Ircu South County Ro Water Plant Storage Tank Interconnect Project
This Amendment 1 to Work Order Number 5 is entered into as of this 17th day of
ninrPn )Pr , 201.a. pursuant to that certain Continuing Contract Agreement for
Professional Services entered into as of November 15, 2011 (the "Agreement" ), by and
between INDIAN RIVER COUNTY, a political subdivision of the State of Florida ( "COUNTY" ) and
MBV Engineering, Inc . ( "Consultant" ) .
1 . The COUNTY has selected the Consultant to perform the professional services set
forth in existing Work Order Number 5, Effective Date February 19, 2013 .
2 . The COUNTY and the Consultant desire to amend this Work Order as set forth on
Exhibit 1 (Scope of Work ) attached to this Amendment and made part hereof by this reference .
The professional services will be performed by the Consultant for the fee schedule set forth in
Exhibit 1 ( Fee Schedule ), and within the timeframe more particularly set forth in Exhibit 1 (Time
Schedule ) , all in accordance with the terms and provisions set forth in the Agreement.
3 . From and after the Effective Date of this Amendment, the above- referenced Work
Order is amended as set forth in this Amendment . Pursuant to paragraph 1 .4 of the Agreement,
nothing contained In any Work Order shall conflict with the terms of the Agreement and the
terms of the Agreement shall be deemed to be incorporated in each individual Work Order as If
fully set forth herein .
IN WITNESS WHEREOF, the parties hereto have executed this Work Order as of the date
first written above .

Engineer's Project Number: 12-276 Addendum 1
Dear Mr. Burke .
We are hereby submitting our proposal to provide professional services for the above subject
project. Per your request, we reviewed the bids that were submitted for the above referenced
project. Just two bids were submitted (RJ Sullivan and Close Construction) . Furthermore , there
were substantial differences in the line item costs of both bids. For these reasons, we have
recommended that the project be re-bid, and that a mandatory pre-bid meeting be held to ensure
that all contractors have a clear understanding of the specifications . Some additional services will
be required to put the project out to bid a second time .
Based on our understanding, our additional scope of work would include the following services ,
1 . Revision of the specifications to include all addendurns to the original specifications
2 . Attendance at the pre-bid meeting
3 . Coordination with contractors and purchasing to address any questions
4 . Preparation of engineer' s cost estimate
We propose to provide the above described services for a total fee of $ 1 ,5004
