Between
MIIC Countryside at Vero Beach , LLC
And
Indian River County Utilities
For the Maintenance of the Water and Server System Internal to the Mobile Home
Park otherwise known as Countryside North
MIIC Countryside at Vero Beach LLC is in process of connecting the property known as
Countryside North Mobile Flolne Park to County Water . Both parties recognize that tile
water distribution system and the wastewater collection system within the property
rePCITCCI to as Countryside North N/lobile hank Pall: is a private system . The system Will
remain the property and responsibility of the OWI1Cl-, MIIC Countryside North , LLC
( Owner) . Indian River County Utilities ( County) will be responsible for the maintenance
of the water distribution System boll Outside Of the property Lip t0 the point Of Connect10I1
at the master nlCters and the wastewater collection system upstream of [lie lift Station .
Both parties agree that if authorized by the OWIICIP, ill all eIllC1-9cilCy , file COL1Ilty will
make any Minor I'epairs to the private System , so as t0 minimize interruption of service to
the community . The County Will bill the Owner for the cost of the repairs to the private
system and the Owner will reimburse the County for such repair. MIIC Countryside
North LLC will Supply the County With a 24 lout' emergency Contact naI11e and phone
number that Will be utilized to obtain authorization for the repair. The county will not
make any repairs [Chill authorization Is granted . If the county Is Unable to obtain such
authorization either because the contact is unavailable , or the contact has not granted such
permission , the county will iiliOrlll Clio residents Of Such Inability t0 Obtain permission .

Region at 813 - 282-6754 . In the future , MIIC Countryside at Vero Bedell , LLC Will
notify the Utility Director, in writing, of any changes in contact information .
The Owner agrees to allow the County to read individual motets assigned t0 each lot and
bill each lot in accordance with the usage for said lot . "]'lie Owner also gives Indian River
County Utilities permission to discontinue service for any account that becomes
delinquent as defined by Indian River County Ordinance 201 . 24 . Indian P`iver County
will honor leases between N/11-IC Countryside North LLC and its tcriants . No aCCOLIlItS
will be established for sub lessees . `]'Ile COLlnty will bill the Owner from file Master
IVleter accounts and any individual lots held in the name of the Owncr. The SLIIII Of all
meter reads on the 648 individual lot meters will be deducted from the master meter reads
and only the difference will be billed on the master meter accounts .
Each lessee will be responsible for Submitting a $ 50 . 00 deposit on their water account as
in accordance with Indian River County Ordinance 201 . 08 . The deposit Will be due on
the same date as the first water bill for etch lot .
Under Indian River County Ordinance 201 . 22 the Owner of the property is responsible
for base charges also ]mown as service availability charges , including talose belonging to
a tenant , if not paid by that tenant . Therefore , once any service availability charges
surpass 90 days from the billing date ; they will be transferred to one of the master meter
accounts in accordance with Indian River County Ordinance 201 . 22 . Indian River County
utilities will first strive to collect these past due charges from the lessee by disconnection

Follow up to FDEP 4-log Virus Treatment .Request For Additional Information
North and South RO Water Treatment Plants
Project Understanding
Florida Department of Environmental Protection has requested additional information to
demonstrate 4- log virus removal that requires recalculation of the way 4 -log credit for virus
reduction is considered . Due to this and what is believed to be unreasonable requests for some
additional information, additional clarification is needed which is beyond the scope of Work
Order # 19 .
SCOPE OF SERVICES
Consultant will recalculate 4 - log credit methodology considering free chlorine as the primary
mode of virus removal and achieving 99 . 99% virus removallinactivatio 1 Consultant will prepare
revised calculation, prepare additional process flow diagrams, and submit a response to FDEc
request dated July 6 , 2010, for conditional approval of 4-log credit .
SCHEDULE
Consultant will prepare a draft demonstration of 4 - lag removal with attachments for Owner
review within two (2) weeks of notice to proceed .
FEES
We will provide these services in accordance with our Continuing Services Contract Agreement
for Professional Services dated April 7 , 2007 with Indian River County,
The Consultant will provide professional services on a lump sum fee basis of $ 3 , 100 .
Additional Services :
The following services are not included in the Scope of Services for this project , but may be
n"equired depending oil circunistatlCes that may arise during* the execution of this project .
Additianal services include , but may not be limited to the following :
m Design drawings of any addition to the facility as requested by FDE, P .
a Pennit applications for any plant component additions which may be necessary.
