( Universal ) is pleased to provide this proposal for Construction Materials Testing ( CMT) services at the above- referenced project in accordance with the project specifications and Indian River County requirements . We are confident that our experience , quality of service , professional attitude and proximity to the site will be of value throughout the design and construction phases of the project . PROJECT DESCRIPTION We understand that this � roject is the 2nd phase of roadway improvements on Oslo Road from 43rd Avenue to 27t Avenue in Indian River County , Florida . These improvements include stormwater utility installation , new sidewalk installation , roadway reconstruction , widening and resurfacing . We estimate the following quantities : • +/- 300 LF of 2 foot depth of embankment fill • +/- 57000 LF of stabilized subgrade and base course on Oslo Road • +/- 4 asphaltic concrete driveways with stabilized subgrade and base course +/- 57100 LF of storm drain and 10 cross drains • +/- 42 storm drain structures/inlets • +/- 16 , 100 SY of mill and overlay 1 " SP 9 . 5 • +/- 361500 SY of 2 " SP 12 . 5 and 1 " SP 9 . 5 • +/- 11 , 000 LF of curb and gutter including intersecting streets 0 +/- 67500 LF of sidewalk Oslo Road Improvements from 43"' Ave to 27`h Ave Universal Proposal No. 0310. 0211 . 00012 Page 2 SCOPE OF WORK Based on review of the project documents and our experience in the area , Universal will provide the following : • On-call earthwork , asphalt and concrete testing • Engineering overview of our services The laboratory testing will be performed out of our Fort Pierce , Florida office with additional support from our Rockledge , Florida office , if required . During the duration of the project all technicians and personnel involved with the construction testing and inspection will be supervised by a Professional Engineer ( P . E . ) registered in the State of Florida . The following assumptions were made to develop the estimated scope of services : • Roadway construction will be phased into two separate operations , one per travel direction , and compaction tests will be performed every 500 linear feet . • Curb pad , sidewalk subgrade , and stabilized shoulders will be constructed as separate operations from the mainline roadway construction , and compaction density tests will be performed every 500 linear feet . • Stabilized subgrade will be sampled and tested for Limerock Bearing Ratio ( LBR) every 1 , 000 linear feet per width of roadway operation . Compaction density tests will be performed every 500 linear feet as a single 12 -inch lift . • Base course will be sampled and tested for Limerock Bearing Ratio ( LBR) every 2 , 000 linear feet per width of roadway operation . Compaction density tests will be performed every 500 linear feet as two 4- inch lifts . • Utility pipe trenches and structure backfill will be tested for compaction density for the full depth of fill in 6 - inch lifts at 100 % of a standard Proctor test (AASHTO T99 ) in accordance with the project plans . Tests will be performed at a minimum frequency of one per every 200 linear feet or one per pipe run . • Embankment will be tested for compaction density at a frequency of one per every 500 linear feet . • Asphalt will be tested in accordance with FDOT Local Area Practice ( LAP ) specification for Superpave asphalt . LAP testing specification consists of one extraction gradation and three cores for thickness and density for each day ' s placement . Cost of Services The services listed herein will be invoiced on a unit rate basis per the estimated scope . Note that the budget is an estimate of our services and may change as the result of unforeseen events such as requested standby time , overtime , retesting , etc . Pick up charges are not included in soil or concrete testing rates . A minimum of four density tests are required per trip . The estimated cost of services is separated into three individual phases based on the construction documents .
