FOR
PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Carter Associates, Inc.
In accordance with Contract No. 0725
This Work Order No. 11 is in accordance with the existing AGREEMENT dated November 6m
2007 between Carter Associates, Inc. (SURVEYOR) and Indian River County (COUNTY). This
Work Order No. 11 amends the agreement as follows:
SECTION I — PROJECT LIMITS
This Work Order No. 11 is for the SURVEYOR to perform one combined Boundary Survey of
the 3 parcels indicted below. Boundary and Improvement Location Surveys are directly related
to the CR510 right-of-way acquisition applications.
(Reference attached map)
Site is as follows:
Being property at the North East corner of CR510 and 58th Avenue.
Current Owner: David Eldell
Three (3) parcels within part of the SW 1/4 of the SW1/4 Section 28 — 31 — 39, I.R.C.

SECTION II - SCOPE OF SERVICES
Attached proposal from Carter Associates, Inc.
As agreed upon between Carter Associates, Inc. and Indian River County, the SURVEYOR
shall provide Professional Land Surveying services to complete one combined Boundary Survey
of the three (3) parcels. Survey shall delineate the Boundary lines as per deed of record
(supplied by the COUNTY); locate all above ground and apparent underground improvements.
Underground improvements that are not readily visible or obvious are not included in these
tasks.

SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT
1. Project shall be completed within thirty (30) business days from Notice to Proceed.
2. Deliverables SHALL BE CERTIFIED TO Indian River County and shall include five (5)
signed and sealed 24" x 36" AND three (3) reduced 8'/" x 11" — "legible" versions (for
recording), electronic copies in AutoCAD 2007 and PDF formats.
SECTION IV —COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for services rendered as
identified in Sections I, II, and III of this Work Order No. 11, a lump sum fee of $1,400.00

The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein.
IN WITNESS WHEREOF the parties hereto have executed these presents this 1st day

The SURVEYOR shall provide Professional Land Surveying services to complete a Boundary

locate all above ground and apparent underground improvements. Underground improvements
that are not readily visible or obvious are not included in these tasks. Insert of the proposed
Right of Way expansion of CR510 to subject parcels.
Specific Notes:
a) All horizontal control shall be mapped in (1983 NAD, National Adjusted
Datum 1999 adjusted), Florida East Zone State Plane Grid Network.
b) Proposal does not include any public records searches for ownerships
and/or right of way takings.
COMPLETION & DE LIVERABLES/VVORK PRODUCT:
1. Project shall be completed within thirty (30) business days from Notice to Proceed.

Mapping/GIS Services Agreement, Contract No. 0725 with the County.
TOTAL LUMP SUM PRICE: $1,400.00
