MODIFICATION NUMBER 6 TO GRANT AGREEMENT BETWEEN THE
DEPARTMENT OF COMMUNITY AFFAIRS AND INDIAN RIVER COUNTY
(Note: Proposed section changes are
This Modification is made and entered into by and between the State of Florida,
Department of Community Affairs, (the Department"), and Indian River County, Florida, (the
Recipient"), to modify DCA Contract Number 06DB-3C-10-40-01-W 14, award dated August 20,
2005, ("the Agreement").
WHEREAS, the Department and the Recipient entered into the Agreement, pursuant to
which the Department provided a grant of $ 7,500,000.00 to the Recipient under the Disaster
Recovery Initiative Community Development Block Grant ("CDBG") Program as set forth in the
Agreement;
WHEREAS, the Department and the Recipient desire to modify the Agreement;
NOW, THEREFORE, in consideration of the mutual promises of the parties contained
herein, the parties agree as follows:
o Reinstate Agreement
1. The Agreement is hereby reinstated as though it had not expired.
o Extend Agreement
2. Paragraph 3, Period of Agreement is hereby revised to reflect an ending date of
3. The Attachment B, Activity Work Plan section of the Agreement is hereby deleted
and is replaced by the revised Attachment B, Activity Work Plan section, which is attached hereto
and incorporated herein by reference.
4. The Attachment A, Program Budget and Scope of Work section of the Agreement is
hereby deleted and is replaced by the revised Attachment A, the Program Budget and
Scope of Work, which is attached hereto and incorporated herein by reference.

o Change in Participating Parties
5. The Attachment G, Special Conditions section, is hereby modified to delete all
references to " ," as the Participating Party, and replace
them with " " as the Participating Party with the understanding
that the Recipient and the new Participating Party will enter into a Participating Party Agreement
containing provisions and caveats that meet or exceed the conditions agreed to in the Participating
Party Agreement between the Recipient and the original Participating Party.
o Inclusion of an Unmet Need as Addressed in the Original Application
a. The Attachment A, Program Budget and Scope of Work section of the Agreement is
hereby deleted and is replaced by the revised Attachment A, the Program Budget and
Scope of Work, which is attached hereto and incorporated herein by reference..
b. The Attachment B, Activity Work Plan section of the Agreement is hereby deleted and is
replaced by the revised Attachment B, Activity Work flan section, which is attached
hereto and incorporated herein by reference.
o Change in Number of Accomplishments and/or Beneficiaries
8. The Attachment A, Program Budget and Scope of Work section of the Agreement is
hereby deleted and is replaced by the revised Attachment A, the Program Budget and
Scope of Work, which is attached hereto and incorporated herein by reference.
All provisions of the Agreement and any attachments thereto in conflict with this
Modification shall be and are hereby changed to conform to this Modification, effective as of the
date of the execution of this Modification by both parties.
All provisions not in conflict with this Modification remain in full force and effect, and are
to be performed at the level specified in the Agreement.

Amendment Number Six (#6)
Dear Mr. Austin:
Enclosed, please find an Amendment number six (#6) package for the above referenced grant.
With this amendment, the county is proposing to modify the Administration, Potable Water
Lines, and Sewer activities as follows:
Administration
a. Decrease the existing budget from $432,500.00 to $415,047.89 to reflect the
actual amount that will be expended under this line item.
2. Potable Water Lines
a. Decrease the existing budget from $670,928.00 to $660,928.00 to reflect the
actual amount expended under this line item;
b. Increase the total number of beneficiaries from 161 to 164 individuals and the
total number of Low and Moderate Income (LMI) beneficiaries for Phase I of the
Wabasso Water project from 141 to 144 individuals to reflect the actual number
of persons that were hooked into the water system as a result of this project; and
c. Increase the total number of beneficiaries from 59 to 46 individuals as well as the
total number of LMI beneficiaries for Phase II of the Wabasso Water project from
48 to 43 individuals to reflect the actual number of persons that were hooked into
the water system as a result of this project.
3. Sanitary Sewer
a. Increase the existing budget from $4,177,024.97 to $4,204,477.08 to reflect the
amount of the construction contract that CDBG-DRI funding will be paying
toward the installation of the new sanitary sewer collection system; and
b. Increase the number of linear feet`of sewerlirie installed in this project from
21,500 to 23,384 to reflect the actual lineal feet installed.
The amendment will not result in an adjustment to the application score. The Amendment
package includes the following items:

o Amended Attachment -A — Program Budget -and -Scope -of Work - - - - - ---
o Amended Attachment B — Project Work Plans, four (4) pages
■ DCA Form — Request for Amendment Form 07.02 (one original)
