Vliabasso Bridges Hanging Reuse Water Main Crossings Inspection
This Work Order Number 2 is entered into as of this 15 day of DecembeF201 5 pursuant to that
certain Continuing Contract Agreement for Professional Services, dated November 15, 2011, and that
certain Extension and Amendment of Continuing Contract Agreement for Professional Services entered into
as of this 4th day of November, 2014 (collectively referred to as the "Agreement"), by and between
INDIAN RIVER COUNTY, a political subdivision of the State of Florida ("COUNTY") and
Carter and Associates, Inc. ("Consultant").
The COUNTY has selected the Consultant to perform the professional services set forth in
the Proposal (Scope of Work), attached to this Work Order and made part hereof by this
reference. The professional services will be performed by the Consultant for the fee schedule set
forth in the Proposal (Fee Schedule), attached to this Work Order and made a part hereof by this
reference. The Consultant will perform the professional services within the timeframe more
particularly set forth in the Proposal (Time Schedule), attached to this Work Order and made a
part hereof by this reference all in accordance with the terms and provisions set forth in the
Agreement. Pursuant to paragraph 1.4 of the Agreement, nothing contained in any Work Order
shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed to
be incorporated in each individual Work Order as if fully set forth herein.
IN WITNESS WHEREOF, the parties hereto have executed this Work Order as of the date
first written above.

It is Carter Associates, Inc.' s (CAI) pleasure to present for your acceptance, the following revised
Proposal for professional engineering and surveying services associated -With the above mentioned
project. The Wabasso Bridge Reuse Main crossings consist of double 12 -inch diameter ductile iron
mains hanging under the bridge decks. There are 4 bridge crossings consisting of 3 low elevation
structures and a high causeway structure. Together, the total length of exposed pipe is
approximately 3,400 linear feet. Since installation, the metal pipes have experienced several small
failures, but have had no complete inspection for the weathering process caused: by the salty
environment. A quick visual view of the hanging pipes indicates the need for such an examination
based on the pictures we observed with you.
With the pipes being hung under each of the bridge decks, access is limited .to;a. mobile platform
deck. Rather than using a multi -boom lift crane, which would require drive lane closure for a.
significant time, CAI has experience, on this very bridge, using a Tolling scaffold for pipeline
inspection. During 2001-2003, CAI conducted: inspection; design; bidding; and inspection and
subsequent re -coating services for the 16 -inch diameter cast iron pipe currently hanging under the
south side of the Wabasso Bridge decks. Unfortunately, the inspection scaffold for that project no
longer exists. CAI would propose the same successful inspection techniques used on the south side
to inspect the County's Reuse Mains located under the north side bridge decks. Please find attached
our Engineering and Surveying scope of services and budget outlining the steps required to prepare
for the inspection, conduct the: field inspection; data review; engineering analysis; and eporting.
Our total price for the services as outlined and estimated Reimbursable Expenses shall not exceed
$56,048.42. These services would be furnished under our existing Engineering Contract with the
County, dated November 4th, 2014. Invoices would be billed only -for actual hours and costs as
identified hereafter.

Any additional services requested by the Owner, but not included in this Project Scope, may be
provided at our hourly rates, a copy of which is attached. Reimbursable expenses, such as permit
fees, express mail, mileage, reproduction, etc., will be billed at cost, except for the manufactured
inspection platform and tapping services which will be cost plus 15 and 10 percent (to cover design
services and administrative costs). The scaffold design sheets as well as costs for the scaffold
(which has been bid by CAI), scaffold platforms and the tapping services are attached in this
Proposal for your review.

INSPECTION, SURVEYING, AND REPORTING SERVICES

PROJECT SCOPE, PARAGRAPHS A THROUGH H
A. Inspection Scaffold - Design, bid and have new inspection scaffold manufactured
specifically for newly constructed bridge railings. At the termination of this project, the Owner
may, if desired, claim ownership of the newly manufactured scaffold upon execution of a release of
liability. The scaffold may then be used for subsequent inspections on the Bridges. Design of the
Scaffold is included as "Attachment No. 1". Quotes for the Scaffold and Scaffold Platforms are on
Pages 12 and 13 of "Attachment No. 2".
B. Survey Crew (3 man) - Using tape measure and temporary grease pencil , the crew shall
layout stationing on each bridge wall for the 3,400 E linear feet. The crew shall also be used to
transport and assist with the scaffold: installation; relocation; and removal.
C. FDOT MOT Plans and Permitting - CAI shall prepare and obtain a FDOT, District 4,
General Use Permit for the Field Inspection Work and the Maintenance of Traffic (MOT) Plans.
D. Field Maintenance of Traffic - The CAI Inspection method anticipates no lane closures
for the Survey or Inspection Services. All Inspection and Survey work, except surveying on the
causeway bridge, shall be behind existing constructed barriers. CAI shall stagger the causeway
surveying to limit exposure to less than 60 minutes at a time. These measures will reduce the
signage requirements and project costs. Temporary MOT signage shall be provided, installed and
removed each day while personnel and equipment are on the Bridges.
E. Pipeline Inspection - The entire existing exposed length of both 12 -inch diameter Reuse
Main pipes (approximately 3,400 linear feet each) shall be visually inspected, with pictures and
notes taken of abnormal exterior wear and/or significant lengths of routine excessive wear. Pictures
shall be related to the surveyed stationing. Additional inspection using ball -peen hammer for
soundness and electronic coating thickness tester shall be utilized. In addition, the inspection shall
include the tapping of each Main at two locations to produce "coupons" of the metal sidewall
profile. Locations shall be coordinated with the Owner and taken as close to the top of pipe as can
be conducted with the tapping equipment. We have made arrangements with Mr. Rick Sheltra of
PRP Construction (license GC1510570) to supply the tapping materials and equipment needed to
obtain the "coupons". A quote for, the Tapping Supplies and Services is on Page 14.
F. Pre -Report Meeting — Upon engineering review of the field data, conducting
alternatives analysis and preparing suggested conclusions, this material shall be presented to the
Indian River County Department of Utility Services in raw data and calculations. CAIshall attend
a review meeting •to discuss the raw data, calculations and conclusions with the Department
representatives. Questions regarding any of these topics shall be discussed and if not resolved atthe
meeting shall be clarified prior to Final Report production. Any new topics or analysis requested of
the Engineer shall be considered additional services.

G. Conclusions Report - The project engineer shall download and log each day's pictures
and data notes. An engineering summary evaluation of the Reuse Mains pipe conditions shall be
conducted. if pipe rehabilitation by exterior coating is deemed a reasonable option, a cost estimate
for that task shall be determined. The cost of re -coating shall be compared in an economic analysis
against other options such as internal relining or replacement with new pipe. Possible alternatives
to re -coating shall be discussed with the Owner's Representative before proceeding with follow-up
analysis Before drafting the Final Report, CAI shall attend the Pre -Report Meeting and resolve
outstanding issue. The activities and results of the Inspection and Engineering tasks shall be
provided to the Owner with up to four original copies of a Conclusions Report.
H. Reimbursable Expenses - The cost of direct, non -personnel expenses that are not routine
overhead expenses are included in this Item. These costs are known or close estimates based upon
anticipated final quantities and represent not to exceed amounts. Reimbursable expenses shall be
invoiced on actual quantity used amounts only. If quantities are not used, they will not be billed. As
indicated in the cover letter accompanying this Scope I Budget, there is no mark- up on any
Reimbursable items except for the invoiced Scaffold and Mainline Tapping costs. Quotes for the
Scaffold and Platforms, and the Mainline Tapping are included in "Attachment No. 2"

2) WELDS - ALL BUTT AND LAP JOINTS SHALL BE MADE USING FULL LENGTH, FILLET WELDS. WELDS SHALL COMPLY
WITH APPLICABLE AWS STANDARDS. CASTER PLATES SHALL BE WELDED TO FRAMING ON ALL 4 SIDES. GUSSET
PLATES SHALL BE FULL WELDED TO EACH FRAME MEMBER THEY TOUCH. WHERE GUSSET PLATES ABUT EACH
OTHER, THEY SHALL BE WELDED TOGETHER. WELD SLAG SHALL BE REMOVED BEFORE BLASTING FOR COATING.
3) DIMENSIONAL DEVIATIONS - CASTER LOCATION IS CRITICAL AND THEY SHALL BE SIZED AND LOCATED WITHIN A
MAXIMUM 1/4 INCH DEVIATION FROM THE DESIGN. FRAMING DEVIATIONS SHALL BE MINIMIZED TO ASSURE
COMPLIANCE WITH THE ABOVE CASTER DEVIATION LIMIT AND SMOOTH SLIDER FRAME INSTALLATION.
4) FINAL FRAME TOUCH-UP - PRIOR TO BLASTING FOR COATING ALL BURRS, SHARP EDGES, ETC. SHALL BE GROUND
OFF ALL GREASE, OIL, LAITANCE, ETC. SHALL BE REMOVED.
5) PRE COATING PREPARATION -THE ENTIRE FRAME SHALL BE PREPARED FOR COATING BY REMOVING ALL RUST
AND MILL SCALE AND PROVIDING A SURFACE PROFILE USING: "POWER TOOL CLEANING" PER SSPC - SP3
SPECIFICATIONS OR "COMMERCIAL BLAST CLEANING" PER SSPC - SP6 SPECIFICATIONS.
6) COATING - COATING SHALL TAKE PLACE INA CLEAN. DRY, INTERIOR ENVIRONMENT THE EPDXY COATING SHALL
BE APPLIED IN TWO. 5 MIL (DRY FILM THICKNESS) COATS, IN ACCORDANCE WITH THE COATING MANUFACTURER'S
