CALL TO ORDER 10:29 A.M. SPECIAL DISTRICTS AND BOARDS A. Emergency Services District 10:29 1. Approval of Minutes Meeting of November 3, 2015 a.m. 2. Approval of Minutes Meeting of November 17, 2015 ON MOTION by Commissioner O'Bryan, SECONDED by Vice Chairman Flescher, the Board unanimously approved the Regular Meeting Minutes of November 3, 2015 and November 17, 2015, as presented. 3. FY 2014-2015 Records Disposition Compliance Statement and Records Management Liaison Officer Form — Emergency Services District (memorandum dated December 2, 2015) PACKET PAGES 270-291 Emergency Services District Page 1 December 8, 2015 ON MOTION by Commissioner O'Bryan, SECONDED by Vice Chairman Flescher, the Board unanimously authorized the Chairman to execute the Records Management Compliance Statement for Fiscal Year 2014-2015 for the Indian River County Emergency Services District. 4. Approval of FY 2015/16 EMS County Awards Grant: Purchase of Capital/Operating Equipment Using Non -Matching EMS Grant Funds and Grant Resolution (memorandum dated December 1, 2015) ON MOTION by Vice Chairman Flescher, SECONDED by Chairman Solari, the Board unanimously approved: (1) the Fiscal Year 2015-2016 Emergency Medical Services Grant; (2) Resolution 2015-01 ESD, of the Emergency Services District Board of Commissioners, Indian River County, Florida, authorizing the application for funding County Emergency Medical Services (EMS) Grant Awards to be submitted to the State of Florida Department of Health, Bureau of Emergency Medical Services; (3) authorized the Chairman to execute the necessary documents to obtain funds from the Department of Health in the amount of $29,309.00 and authorized budget amendments as required to receive and expend the grant funds; and (4) authorized staff to establish a unique accounting code designator for all County Awards Grant deposits, disbursements, interests accrual and rollover of funds. Assistant Fire Chief Brian Burkeen announced that the new engines would probably be delivered by mid-January 2016.
