Subject: Amendment No. 2 — Work Order No.1 Donadio & Associates, Architects, P.A.
On August 13, 2019, the Board approved Work Order No. 1 for Donadio & Associates, Architects,
Inc. (Donadio) to perform work under continuing services contract (RFQ 2018063) for the design and
engineering related to elevating the Jones Pier Conservation Area's Main House (1930s Bungalow
Cottage). These services are required to complete the design and engineering review by the Florida
Division of Emergency Management (FDEM) and subsequent State Historic Preservation Office
(SHPO) in order to qualify and receive FEMA mitigation funding to assist with the work necessary to
elevate and protect the historic home.
Donadio and sub -contractor Schlitt Engineering Services, provided fmal plans on March 16, 2020. A
review from the Hazard Mitigation Grant resulted in a necessary change to the design plans which
was completed under Amendment Number 1 in the amount of $1300.00. This amendment was
approved on May 11, 2020, under the County Administrottor's signing authority.
As a result of ongoing site work, the architect has recommended a necessary grading plan be
completed by an engineer prior to bidding. This will allow for necessary as-builts and estimating
material quantities. Because MBV, Inc. is the engineer of record for Phase 1 improvements to the
site, it was determined that they would be able to expedite the grading plan quickly which would also
tie in the other site improvements seamlessly. This Amendment Number 2, in the amount of $850.00
will complete the plans necessary for the bid documents.
The total cost of the Work Order with both amendments will be $17,900.00.
FUNDING:
Funding for the amended work order in the amount of $850.00 is available through 004121072-
066510-18010 — MTSU Fund/Parks/Jones' Pier Improvements.
RECOMMENDATION:
In order to expedite the plan set completion in preparation for bidding as soon as possible, staff
requests that the County Administrator approve Amendment No. 2 in the amount of $850 within
his delegated authority limit.

CONTINUING ARCHITECTURAL SERVICES AMENDMENT 2 TO WORK ORDER 1
JONES PIER CONSERVATION AREA HISTORIC RESIDENCE
ARCHITECTURAL/STRUCTURAL ENGINEERING
This Amendment #L to Work Order Number #1 is entered into as of this _ day of December, 2020, pursuant
to that certain Continuing Architectural Services Agreement for Professional Services entered into as of the 13th
day of November, 2018 (collectively referred to as the "Agreement"), by and between INDIAN RIVER COUNTY, a
political subdivision of the State of Florida ("COUNTY") and Donadio & Associates, Architects, P.A.
("Consultant").
1. The COUNTY has selected the Consultant to perform the professional services set forth in existing Work Order
Number 1, Effective Date August 13, 2019.
2. The COUNTY and the Consultant desire to amend this Work Order as set forth on Exhibit A (Scope of Work)
attached to this Amendment and made part hereof by this reference. The professional services will be
performed by the Consultant for the fee schedule set forth in Exhibit B (Fee Schedule) in accordance with the
terms and provisions set forth in the Agreement.
3. From and after the Effective Date of this Amendment, the above -referenced Work Order is amended as set
forth in this Amendment. Pursuant to paragraph 1.4 of the Agreement, nothing contained in any Work Order
shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed to be
incorporated in each individual Work Order as if fully set forth herein.

Dear Mr. Donadio:
At you request, we are hereby submitting our proposal to provide professional services for the
above project. Furthermore, we understand you are looking to lift an existing historical house on
the property onto new pilings and provide access through the way of stairs and ramps. As such, a
site grading plan and quantity of fill is required in order to provide proper drainage away from
underneath the building and to existing stormwater management areas.
We propose to provide these services for a fee of $ 750.00, excluding direct costs.
The following services are excluded from this proposal _and are not considered part of the basic
services: Coordination with the surveyor to provide the as -built information required as part of the
above-mentioned services.
Construction inspection and additional services, including but not limited to meetings, modified
site layouts, and environmental coordination will be provided on an hourly rate basis, as an
addition to the Cost of Work / Services fee at the latest billing rates. Current billing rates are as
