PROGRAM DESCRIPTION:
1 - The Babies and Beyond program focuses on providing services to every mother, baby, and family
in Indian River County. The Babies and Beyond program offers childbirth education, lactation support
within the hospital, a nurse home visit to postpartum mothers and babies, and community referrals when
appropriate. The program is designed to increase health literacy, strengthen parenting skills, and ensure
that each baby in Indian River County gets the healthy start they deserve. The projected number of
unduplicated children to be served is two thousand, two hundred eighty (2,280) and one thousand, one
hundred forty (1,140) adults.
2 - The Community Doula program supports, educates, and empowers pregnant women to take control
of their health. A Doula is a trained non-medical professional who provides continuous physical,
emotional, and informational support to a mother before, during, and shortly after childbirth, to help her
achieve the healthiest, most satisfying experience possible. More importantly, Doulas have been shown
to have a significant impact on birth outcomes and health literacy. The projected number of unduplicated
adults to be served is one hundred (100).
3 - The Parents As Teachers program (PAT) provides at -risk mothers and fathers with the knowledge
and skills to maximize their child's cognitive, social and emotional development during the most crucial
period of brain development: birth through age three or the first 1,000 days of life. Areas of focus include:
child development, parent-child interaction, play activities designated to target developmental domains,
and family well-being. The projected number of unduplicated children to be fifty-nine (59) and sixty-two
(62) adults.
4 - Indian River County's Healthy Start Coalition's (IRCHSC) Coordinated Intake and Referral (CI&R)
service, which is housed within Partners in Women's Health, informs all program participants about the
various services for which they are eligible and makes referrals as appropriate. The Connections
EXHIBIT A
Specialists follows up with clients regarding services and overall satisfaction. Through community
collaboration, IRCHSC has simplified navigating services and programs available in Indian River
County. One of these services being presumptive eligibility for pregnant women (PEPW) assists eligible
pregnant women to immediately receive prenatal care through Medicaid for up to 45 days, while their
eligibility for full Medicaid benefits is determined. The PEPW program is designed to improve pregnant
women's early access to outpatient prenatal services. The projected number of unduplicated children
to be served is three hundred ninety-six (396) and seven hundred four (704) adults.
5 - The Prevention Education and Community Enhancement (PEACE) program is designed to enhance
the Healthy Start system of care by streamlining initiatives that involve a primary focus on education
and outreach. PEACE will increase local capacity to address the mental, physical, and emotional needs
of pregnant mothers, partners, and families through direct education and outreach. IRCHSC identified
prioritized initiatives through community assessment and FIMR. PEACE education programs for
community members (e.g., mothers, families) and professionals (OBs, pediatricians, mental health
professionals) include perinatal mental health, perinatal bereavement, and empowerment and well-
being of the other parent, and birth and postpartum support. The projected number of unduplicated
adults to be served is five hundred fifty (550).
6 - Healthy Families Indian River County is a home visitation program. Services are initiated during
pregnancy and/or if the infant is 3 months of age or younger, for mothers who are vulnerable for meeting
the challenges of adding a new member to the family. Some mothers have diagnosed mental health
issues and need positive parenting techniques to help raise their baby. Participants also learn about
the importance of healthcare, well -baby checks, childhood immunizations, and family planning. Also,
Family Support Workers (FSW's) conducts regular developmental screenings of the children along with
the families to identify if infants and young children need further evaluation for developmental delays
that affect readiness for school. FSW's help families access quality daycare and services in the
community. Referrals to community partners assist in aiding families to gain assistance in other areas
of need. The projected number of unduplicated children to be served is two hundred twenty (220) and
one hundred thirty (130) adults.

EXHIBIT B
[From policy adopted by Indian River County Board Of County Commissioners on March 15, 2022.1
"D. Nonprofit Agency Responsibilities After Award of Funding
1. Indian River County provides funding to all nonprofit agencies on a reimbursement basis
only.
2. All reimbursable expenses must be documented by an invoice and/or a copy of the canceled
check. Any expense not documented properly to the satisfaction of the Office of Management
& Budget and/or the County Administrator may not be reimbursed.
3. If an agency repeatedly fails to provide adequate documentation, this may be reported to the
Board of Commissioners. In the event an agency provides inadequate documentation on a
consistent basis, funding may be discontinued immediately. Additionally, this may adversely
affect future funding requests.
4. Expenditures may only be reimbursed from the fiscal year for which funding was awarded. For
example, no expenditures prior to October 1st may be reimbursed with funds from the following
year. Additionally, if any funds are unexpended at the end of a fiscal year, these funds are not
carried over to the next year unless expressly authorized by the Board of Commissioners.
5. All requests for reimbursement at fiscal year-end (September 30th) must be submitted on a
timely basis. Each year, the Office of Management & Budget will send a letter to all nonprofit
agencies advising of the deadline for reimbursement requests for the fiscal year. This deadline
is typically early to mid-October, since the Finance Department does not process checks for
the prior fiscal year beyond that point.
6. Each reimbursement request must include a summary of expenses by type. These summaries
should be broken down into salaries, benefits, supplies, contractual services, etc. If Indian
River County is reimbursing an agency for only a portion of an expense (e.g. salary of an
employee), then the method for this portion should be disclosed on the summary. The Office
of Management & Budget has summary forms available.
7. Indian River County will not reimburse certain types of expenditures. These expenditure types
are listed below.
Travel expenses for travel outside the County including but not limited to; mileage
reimbursement, hotel rooms, meals, meal allowances, per Diem, and tolls. Mileage
reimbursement for local travel (within Indian River County) is allowable.
b. Sick or Vacation payments for employees. Since agencies may have various sick and
vacation pay policies, these must be provided from other sources.
c. Any expenses not associated with the provision of the program for which the County
has awarded funding.
d. Any expense not outlined in the agency's funding application.
8. The County reserves the right to decline reimbursement for any expense as deemed
necessary."
