GOK60 FPN 431759-2-54-01 Recipient: Indian River County This Supplemental Agreement ("Supplemental"), dated 525-010-60 PROGRAM MANAGEMENT 12/18 Page 1 of 2 arises from the desire to supplement the State -Funded Grant Agreement ("Agreement") entered into and executed on March 20, 2017 as identified above. All provisions in the Agreement and supplements, if any, remain in effect except as expressly modified by this Supplemental. The parties agree that the Agreement is to be amended and supplemented as follows: On March 20, 2017 the County Incentive Grant Program Agreement was executed. On June 12, 2019, the parties executed Amendment Number One, which increased funding of FIVE MILLION EIGHT HUNDRED EIGHTY THOUSAND SIX HUNDRED THIRTY SIX DOLLARS AND NO CENTS ($5,880,636.00) for the Project, as a result of an increased construction cost estimate; and extended the term of Agreement to December 30, 2020. On October 21, 2020, the parties executed Amendment Number Two, to extend the Agreement to December 31, 2022; and to revise Exhibits C and A, Project deliverables. On October 19, 2022 The parties executed Amendment Number Three to make the Agreement compliant with applicable Florida Statutes; to increase the total funding needed for the Project's construction and CEI services; to update certain Project Exhibits; and to extend the term of the Agreement from December 31, 2022 to June 30, 2023 The purpose of this Supplemental Agreement is to extend the term of the Agreement from June 30, 2023 to December 31, 2023. Reason for this Supplemental and supporting engineering and/or cost analysis: The County experienced delays in the availability of materials and construction activities that resulted in a revised project schedule. This Supplement Agreement extends the terms of the executed State Funded Grant Agreement (SFGA) from June 30, 2023 to December 31,2023 in order to align with the revised construction schedule and to avoid agreement expiration prior to Project completion IN WITNESS WHEREOF, this AMENDMENT is executed by the parties below for the purposes specified herein.
