II. SCOPE OF SERVICES Reference is made to Attachment B, the attached Scope of Work and the "Professional - Engineering Services Master Agreement for Solid Waste." III. CONSULTING ENGINEER INSURANCE REQUIREMENTS A. Worker's Compensation Insurance in accordance with Florida Statutes. B. Comprehensive and Automotive Liability with a minimum coverage of $100,000/$300,000 per occurrence for bodily injury or accidental death. C. Comprehensive General Liability with a minimum limit of $100,000/$300,000 covering property damage. D. Liability for Property Damage, while operating motor vehicle, with minimum limits of $100,000 per occurrence. E. Contractual Liability including limits established for Items III B. III C. and III D above. F. Umbrella coverage, excess liability with minimum limits of $100,000 per occurrence. Rli l�1�94 -6 40 40 ATTACHMENT B SCOPE OF WORK CONSTRUCTION AND DEMOLITION (C&D) DEBRIS PERMITTING SERVICES INTRODUCTM Indian River County currently operates a Construction and Demolition (C&D) Debris Landfill facility under a Florida Department of Environmental Protection (FDEP) general operating permit (Permit No. S031-236459). CDM is currently working with FDEP to renew the C&D Debris Operating permit for the landfill. Camp Dresser & Mckee Inc. submitted a groundwater monitoring plan, which includes installation of additional monitoring wells, as part of the permit application package. The previous proposal was budgeted for installation of upgradient and downgradient groundwater monitoring wells based on the installation of four (4) shallow groundwater monitoring wells. During the permitting process the FDEP is requiring an additional 4 intermediate wells and 1 deep monitoring well. This proposal covers the cost of installation of 4 additional intermediate depth monitor wells and 1 deep monitoring well. The new monitoring wells will be tied into existing horizontal and vertical control using Indian River County's contract professional land surveyor. The survey of the monitoring wells is outside the scope of this contract. Task 100 - INSTALLATION OF MONITORING WELLS CDM will subcontract the installation of the monitoring wells to a qualified geotechnical engineering firm. A total of four intermediate and one deep groundwater quality monitoring wells are budgeted in this proposal as required by FDEP. The monitor wells will be installed in accordance with the specific conditions of the current FDEP operating permit for the Landfill. A figure showing the approved number and location of monitoring wells is attached. The specific conditions from are restated below: 15. New or replacement monitoring well design or placement must be approved by the Department. Proposed well construction details based on site specific borings must be submitted with all supporting data (grain size, analyses, in-situ hydraulic conductivity testing, death to water, etc.) for Department annroval prior to well installation- I Ne! of hollow Story, auger equipment is recommended. Other drilling methods must be approved by the Department prior to well installation. 18. Well completion reports for new monitor well(s) must be submitted to the Department on the attached Ground Water Monitoring Well Completion Report Form within thirty (30) days after installation. Note that the latitude and longitude in degrees, minutes and seconds of each well must be provided on the form. In addition, as -built well construction diagrams and soil boring logs that cover the entire depth of the monitoring wells must be submitted to the Department. 1,0169.wpd n.1 ELI O O 19. A survey drawing must be submitted within sixty (60) days following monitor well installation showing the location of all monitor wells (active and abandoned), water bodies and waste filled areas. The location of features on the survey drawing must be horizontally located by standard surveying techniques. The survey drawing shall include the monitor well name and identification number as well as location and elevation, referenced to NGVD, of all wells, permanent benchmark(s) and/or corner monument marker(s) at the site. The survey shall be conducted and certified by a Florida Registered Surveyor. Prior to installing the monitoring wells, a pilot standard penetration test (SPT) boring will be drilled at each location. CDM will review the soil boring log prepared by the driller and visually classified in the lab according to the United Soil Classification System to select a preliminary screen interval for each monitoring well. CDM will then specify where the grain size analysis should be completed to properly size the sand pack and screen slot size for each monitoring well. During construction of the monitor wells, CDM will provide oversight of the drilling and classification of soil types from the pilot soil borings. CDM will also provide construction oversight during monitor well installation. After the monitoring wells are constructed, they will be developed by pumping until a clear water sample is obtained. After all of the new monitoring wells are installed, a registered professional land surveyor will i locate each monitoring well relative to the State of Florida Plane Coordinate System (NAD 1983). The land surface elevation and top of casing elevation for the five additional wells will be tied into the National Geodetic Vertical Datum (NGVD) of 1929 by a professional land surveyor. The surveyor under contract to the Indian River County SWDD will perform this work, as directed by Indian River County. IV 11:0 MM After the monitoring wells have been installed, developed and surveyed, CDM will prepare a summary letter report for Indian River County to submit to FDEP. CDM will combine the results of the monitoring well installation with the results of the survey work. The report will include a map showing the final location of the five additional monitoring wells. A FDEP completion report will be developed for the monitoring wells. 'YY t' understand that it is planned to install four (4) intermediate wells to 50 feet and one (1) deep well to 75 feet at locations around the landfill to be identified. The wells will be 2 -inch in diameter with concrete pads and lockable caps. Protective steel posts will be installed around the concrete pad. The deep well will be double cased using 6 -inch diameter PVC casing. In addition, an SPT boring will be performed at each well la .o prior to installation of the wells. We will also extend the riser for one well where riser is about 18" below the casing. We estimate that we can provide the scope of services outlined above for the sum of $11.045.00. The cost estimate is outlined on Attachment A. This cost estimate is based on the assumption that the site is accessible by 4 -wheel drive drilling rig and that the boring locations are identified to the field prior to mobilization. We estimate that the field work will require 5 to 7 days to complete. The well completion report will be available within 3 days following the completion of the field work.
