Indian River County Solid Waste Disposal District Landfill
Dear Mr. Brooks:
Camp Dresser & McKee Inc. is pleased to submit our proposal to Indian River County to
perform engineering services for the referenced landfill project. This proposal includes the
preparation of shallow, intermediate and deep surficial groundwater contour maps for one
semi-annual sampling events in January 1999. This authorization is proposed as an
ammendment to Work Authorizationn No. 17.
The following attachments are provided herein as part of the proposal:

Consulting Engineering services related to preparation of landfill groundwater contour
maps for January, 1999, in accordance with the Indian River County Solid Waste
Disposal District Landfill's current FDEP operation permit, No. 5031-245825.
Reference is made to Attachment B, the attached Scope of Services and the
Professional Engineering Services Master Agreement for Solid Waste".

A. Workers Compensation Insurance in accordance with Florida Statues.
B. Comprehensive and Automotive Liability with a minimum coverage of
$100,000/$300,000 per occurrence for bodily injury or accidental death.
C. Comprehensive General Liability with a minimum limit of $100,000/$300,000
covering property damage.
D. Liability for Property Damage, while operating motor vehicle, with minimum
limits of $100,000 per occurrence.
E. Contractual Liability including limits established for Items III B, C, and D above.
F. Umbrella coverage, excess liability with minimum limits of $100,000 per
occurrence.

Lump Sum Cost Elements
For the basic services performed for a lump sum fee, Indian River County Solid Waste
District agrees to pay Camp Dresser & McKee Inc. (CDM) partial payments to be
made on a monthly basis in proportion to the percentage of work completed.

SCOPE OF SERVICES
INDIAN RIVER COUNTY SOLID WASTE DISPOSAL DISTRICT
PREPARATION OF LANDFILL GROUNDWATER CONTOUR MAPS
Task 100 -Groundwater Contour Maps
Indian River County Solid Waste Disposal District (SWDD) is required by its Florida
Department of Environmental Protection (FDEP) operating permit to provide groundwater
elevation contours semi-annually. According to FDEP Permit No. S031-245825, Exhibit I,
Condition No. 23 SWDD must provide groundwater elevation contour map for each
monitored aquifer zone which is to be submitted semi-annually.
CDM will provide to Indian River County SWDD groundwater elevation contour maps for
the January, 1999 sampling event conducted at the landfill. When CDM receives the
individual monitor well groundwater depth readings from Envirometrics, Inc. the
groundwater elevation (in feet NGVD) will be calculated. Then they will be organized by
aquifer zone into shallow, intermediate and deep surficial aquifer readings and groundwater
contours developed on separate maps of the landfill for each aquifer. These contours will be
developed and reviewed by a registered professional engineer and certified maps provided to
Indian River County SWDD for submittal to the FDEP as required by permit. Any readings
which are not consistent with past observations will be referred back to Envirometrics for
confirmation. Only the January, 1999 event has been budgeted in this proposal.

Groundwater contour maps for Task 100 will be provided to Indian River County SWDD
within three weeks of receiving the groundwater depth readings.
