OF THE GUARDIAN
Seciion 3.1 of Part B of Indian River County's Request For Proposal for dental
insurance stated that any modification to the proposal must be in writing and signed
by the parties. This agreement is intended to modify the minimum participation level
set out in the First Modification to Dental Insurance Proposal of the Guardian dated
March 23, 1999.
Numbered paragraph 1 of the First Modification is hereby amended to read:
1. It is a condition of this contract that Guardian shall be under no
obligation to proceed unless twenty-nine percent (29%) of the
employees of Indian River County (236 employees) enroll in the
dental insurance program prior to June 1, 1999."
Numbered paragraph 2 shall be modified to read:
"2. Employees of the constitutional officers of Indian River County
may piggyback the dental insurance program of the Board of
County Commissioners."
Numbered paragraphs 3, 4, 5 and 6 of the First Modification remain as set out in the
First Modification dated March 23, 1999.
This Second Modification to the contract for dental insurance between the Guardian
Life Insurance Company of Arnerica and the Board of County Commissioners of
Indian River County shall become effective at the end of the enrollment period only if
a twenty-nine percent (29%) voluntary participation rate by employees is attained.
