FOR
THE PROFESSIONAL SERVICES MASTER AGREEMENT
DATED APRIL 1, 1997
BETWEEN
DAVID M. JONES, PROFESSIONAL SURVEYOR AND MAPPER
AND
INDIAN RIVER COUNTY SOLID WASTE, DISPOSAL DISTRICT
Pursuant to the Professional Services Master Agreement dated April 1, 1997 between Indian
River County Solid Waste Disposal District (hereinafter referred to as the SWDD) and David M.
Jones, Professional Surveyor and Mapper (hereinafter referred to as the CONSULTANT) for
various surveying, AutoCad and Aerial Photogrammetry services, this Specific Work
Authorization No. 7 is an extension of and hereby becomes a part of that Master Agreement
Contract Number 9701 as follows:
DESCRIPTION OF PROJECT
Indian River County Landfill.
SECTION 1 - BASIC SERVICES OF THE CONSULTANT
A. Survey the locations of the newly installed groundwater monitoring wells around the

coordinates for each well in both systems of Northings — Eastings and Latitude —
Longitude. Establish the NGVD elevation of the top of casing (TOC) and the natural
ground elevation for each monitoring well.
B. Determine and mark the 22.00 NGVD elevation on the staff gage posts, set by others,
in the stormwater lake in the Southwestern pari of the project.

C. Label the benchmark previously placed on the western top of the stormwater outfall
structure, by permanently affixing a brass or bronze tag clearly engraved with the
Northing, Easting, and Elevation of the benchmark disk. Label the benchmarks
previously placed on the "V" Notch Weir of the outfall structure and the northeast
corner of the concrete pad of monitoring well 15D by affixing a brass or bronze tag.
Place a benchmark disk on the concrete base of monitoring wells, 20-D and 17-1.
D. Contractor shall provide the Solid Waste Disposal District with all survey data on
record on a 3-1/2" diskette and in AutoCad release version 14.0.
SECTION 2 - OWNER'S RESPONSIBILITIES
No additions or changes.
SECTION 3 - PERIODS OF SERVICE
The CONSULTANT will begin the survey on August 1, 19989. The CONSULTANT shall
complete the survey and report within August 14 1999.
SECTION 4 - PAYMENTS TO CONSULTANT
The CONSULTANT's fee for performing the surveying service as described in Section "I-
A". thru "I -E" shall be a lump sum amount of $1,800.00
