II. SCOPE OF SERVICES Reference is made to Attachment B, the attached Scope of Services, and the "Professional Engineering Services Master Agreement for Solid Waste." III. CONSULTING ENGINEER INSURANCE REOUIREMENTS The following insurance requirements will be provided in accordance with the Master Services Agreement: A. Worker's Compensation Insurance in accordance with Florida Statutes. B. Automotive Liability with a coverage of $1,000,000 for a combined single limit. C. (CDM) partial payments to be made on a monthly basis in proportion to the percentage of work completed in accordance with the referenced contract. A-2 Je309A. 40 • ATTACHMENT B SCOPE OF SERVICES AND SCHEDULE INDIAN RIVER COUNTY SOLID WASTE DISPOSAL DISTRICT ENGINEERING SERVICES FOR RENEWAL OF LANDFILL OPERATION PERMIT AND STORMWATER PERMIT Project Understanding The Indian River County Solid Waste Disposal District (SWDD) operates a Class I Landfill and other solid waste management facilities for Indian River County residents. The Florida Department of Environmental Protection (FDEP) issues operation permits which must be renewed every 5 years for the operation of solid waste disposal facilities. The SWDD permit needs to be renewed and updated for the next 5 years. This FDEP Operations Permit also includes the closure of Segment 1, the partial closure of Segment 2 and the Waste Tire Processing Operation. Task 1.0 - PREPARATION OF PERMIT RENEWAL APPLICATION FOR THE LANDFILL The solid waste renewal application consists of the 41 page Chapter 62-701.900 FAC checklist and all other supporting information, The major information for the permit application is the updating of all information since the last permit modification. The applicant must complete Parts A, B, D, E, F, H, I, K through R, and T of the application. The anticipated effort required for each of the parts is described in the following paragraphs. Parts A, B, D, E, and F consist of data forms and general facility information (location of airports, floodplain, zoning/land use, solid waste projections, airspace volume, etc.). Most of the information developed for the December 1995 operations permit modification is usable for the permit renewal, with some revisions for changes in facility operations since January 1996, and for required updates. Part H of the permit application reflects the results of the hydrogeological investigation for the site. Much of the hydrogeological information for the site can be taken from the hydrogeological report submitted with the February 1994 operations permit renewal. "-1 w*ll i 6iei unuc and summarize the geotechnical investigation performed for the lateral and vertical expansions of Segment 2 and the Infill. Part K information is an operations plan which describes in detail the operations, safety, and training procedures which are used on a daily basis to minimize the risk of environmental degradation. Most of the operations manual prepared for the Infill operation permit modification will be used for this renewal submittal. Part L describes the water quality and leachate monitoring plan for the site. A comprehensive report describing hydrogeology, the groundwater and surface water monitoring system, and leachate sampling has been prepared and will be submitted prior to this renewal being submitted. This report is the Indian River County Landfill Biennial Water Quality Report for the 1997 and 1998 water quality data. 03W•Wo August 11, 1999 B—I 40 Parts N, O, and P relate to landfill closures. These parts will be updated as needed for both Segment 1 and the partial closure already taken place on Segment 2. Information submitted previously to FDEP will be referenced as much as possible. Part Q is a requirement to provide financial assurance for closure and for 30 years of long-term care. A detailed closure cost estimate for the Segment 2/Infill Segment Closure and a sinking fund financial analysis to determine annual deposits to a landfill closure escrow account, based on the remaining airspace has been performed and will be included in the submittal. This will include the most recent detailed cost estimate for operation and maintenance of Segment 1, Segment 2, and the Infill segment. The Tire Processing Operation will also be renewed as part of this permit application. Task 2.0 - FDEP STORMWATER PERMIT RENEWAL CDM has contacted FDEP regarding the renewal of the stormwater permit. The stormwater permit was issued to construct and operate a stormwater system at the site and now must be renewed. An Environmental Resource Permit (ERP) is now required for stormwater permitting. FDEP staff has indicated that the conversion of this permit will require submittal of an ERP application. The ERP application form will be completed, however, some information which has already been submitted will be referenced in the form and not resubmitted. Some information will have to be updated with the submittal of this ERP application. The Environmental Resource Permit (ERP) application form consists of Sections A — F and attachments 1-6. For the renewal of the current MSSW permit Sections A, C and E will be completed. Information previously submitted which is still applicable will be referenced to the submittal in which it was included. Section A of the ERP is a general information section consisting of 8 parts. Information regarding the proposed activities, type of permit, ownership, general description of activities and signature pages are part of this section. Section E requires detailed information regarding any site improvements or stormwater improvements which affect wetlands or surface waters. Since construction of additional stormwater facilities was recently completed, some limited information will be supplied regarding the landfill area and stormwater facilities. A map showing the entire property boundary of the landfill on a recent aerial will be included as part of the submittal. Aerial photography will be performed by a subconsultant to CDM to show the existing and additional property owned by the SWDD at the landfill. The aerial photography will include a rectified color aerial, an infra -red aerial and a digital file of the aerial photograph. jeM.wpa August 11, 1999 D-2 do s Task 3.0 - RESPONSES TO FDEP REQUESTS FOR ADDITIONAL INFORMATION Due to the complexity of the construction process for Class I lined disposal areas, it is anticipated that FDEP may request additional information regarding the permit renewal. It is anticipated that each round of responses will require from 45 to 60 days depending on the number and complexity of FDEP comments. Responses to FDEP comments will be in the form of letter reports with attached supplemental information.
