Public Health Center Indian River County, Florida Dear Mr. Williams: ORIGINAL 99-3 Spillis Candela & Partners, Inc. (SCP) is pleased to offer the following professional services related to H. R. S. Public Health Center located in Indian River County, Florida. This proposal includes an outline of Engineering services, delivery schedules and professional fees as requested by you on our telephone conversation of November 18, 1999. Based on our conversations we understand the scope of work to apply to a project having the following characteristics: The building is an existing two story building with (6) existing air handlers; (4) constant volume air handlers in the first floor and (2) variable air volume air handlers in the second floor. Basic services to be provided by SCP will include in-house Engineering to produce a set of construction documents for the installation of a 100% outside air roof -top a/c unit dedicated to cool the outside air as outlined on our HVAC System Evaluation Report dated October 28, 1999. As part of the work required to produce this set of drawings SCP will provide (3) visits to the site. The first visit will be performed prior to the production of any drawings in order to verify existing conditions. The second visit will be provided during the construction phase with the third visit to perform a project final punch list at the end of the work. SCP will make submissions to the Owner at the completion of Construction Documents. These services will be incorporated to the Board of County Commissioners Contract with SCP, Inc. dated May 26, 1998, based on contract form AIA B141 (1987). The construction documents will consist of a complete set of mechanical and electrical drawings. Structural drawings if required will also be provided. November 29, 1999 Mr. Lynn Williams Indian River County Page 2 systems. The new work will be drawn on sepia mylars made from the as -built drawings provided by Owner. The reason for this is to avoid having to redraw the complete floor plans and roof plan and therefore minimize the costs to Owner. These services and the corresponding fees will be charged as follows: Preparation of Construction Documents — A Lump Sum of $12,000.00 If additional services are required, the Owner will be billed as per attached SCP STANDARD BILLING RATES CALENDAR YEAR 1999. Our construction administration effort will be limited to review of shop drawings, answering RFI's, a site visit as outlined above, and a final punch list. Bidding will be handled by the Owner. Reimbursable expenses related to the project will include out-of-town travel, long distance communications, reproductions, plotting, and shipping. These expenses will be invoiced at 1.1 times the actual invoice. The project schedule is a critical component of our fee quotation. The following schedule is proposed: SCP will provide a complete set of construction documents (3) weeks after receiving Owner's approval to proceed and the set of as -built drawings. This proposal is based on several assumptions regarding project size, budget, schedule and the requested scope of work. Adjustments to this proposal may be possible after further study or clarification of the scope with the information obtained in the as-builts documents and the initial site verification. We appreciate the opportunity to provide professional services to Indian River County and look forward to working with you. If this proposal is acceptable, we will be pleased to begin work upon receipt of your signed acknowledgement below and the as-builts documents. Payments are due and payable 10 days from the date of invoice. Invoices unpaid 30 days from the date of invoice will accrue interest after 30 days from date of invoice and may cause work to be - - interrupted.
