I. PROJECT DESCRIPTION - TASKS

Consulting Engineering services related to preparation of the annual full cost accounting
study for Fiscal Year 1998-99 in conformance with the requirements of Chapter 62-708,
Florida Administrative Code, and draft public notice of full cost.
II. SCOPE OF SERVICES
Reference is made to Attachment B, the attached Scope of Services and the "Professional
Engineering Services Master Agreement for Solid Waste".

III. CONSULTING ENGINEER INSURANCE REQUIREMENTS
A. Workers Compensation Insurance in accordance with Florida Statutes.
B. Comprehensive and Automotive Liability with a minimum coverage of
$100,000/$300,000 per occurrence for bodily injury or accidental death.
C. Comprehensive General Liability with a minimum limit of $100,000/$300,000
covering property damage.
D. Liability for Property Damage, while operating motor vehicle, with minimum limits
Jof $100,000 per occurrence.
E. Contractual Liability including limits established for Items III B, C, and D, above.
�I F. Umbrella coverage, excess liability with minimum limits of $100,000 per occurrence.

Engineer Services:
Task 100 - Prepare Full Cost Accounting Report for
Fiscal Year 1998-99, and Draft Public Notice (Lump Sum) $6,500.00

Lump Sum Cost Elements
For the basic services performed for a lump sum fee, Indian River County Solid Waste
Disposal District agrees to pay Camp Dresser & McKee Inc. (GDM) partial payments to
be made on a monthly basis in proportion to the percentage of work completed.

SCOPE OF SERVICES
INDIAN RIVER COUNTY SOLID WASTE DISPOSAL DISTRICT
FULL COST ACCOUNTING REPORT FOR FISCAL YEAR 1998-99
PROJECT SCOPE OF SERVICES
Task 100 - Full Cost Accounting Study and Report for Fiscal Year 1998-99
Under provisions of Chapter 62-708.300(3), Florida Administrative Code, Indian River County
Solid Waste Disposal District (SWDD) is required to determine and publicly disclose the full
cost of solid waste management within its service area for Fiscal Year 1998-99.
CDM will prepare a report setting forth the full cost of solid waste management for Fiscal Year
1998-99 in accordance with the requirements of Chapter 62-708, Florida Administrative Code.
CDM will also prepare a draft public notice based on the full cost of solid waste management in
Indian River County for Fiscal Year 1998-99.
SCHEDULE
CDM will provide a draft report and public disclosure notice to the County by March 10, 2000
assuring receipt of all data required to perform the study by March 1, 2000. After receipt of
comments, CDM will submit the final report and public disclosure notice within one week.
