AMENDMENT NO. 3
4T" STREET WIDENING, DRAINAGE IMPROVEMENTS
AND INTERSECTION IMPROVEMENTS
This is an amendment to the Professional Engineering Agreement (AGREEMENT) dated January
4. 1995
SECTION If - SCOPE OF SERVICES
The CONSULTANT agrees to perform the additional professional services described below:
A. Widen sidewalks from 5' to 6throughout the project. Relocate sidewalks away from back
of curb, where existing right-of-way permits. Provide additional sidewalk on the south side
of 4'" Street, west of Old Dixie Highway.
B. Revise design drawings and drainage calculations to accommodate restrictions imposed by
actual right-of-way acquired. Add drainage facilities at SE comer of 4'h Street and Old
Dixie. Update design drawing to indicate actual right-of-way and Temporary Construction
Easements acquired in lieu of notations for "proposed right-of-way".
C. Show all buried utilities on cross sections and include utility information.
D. Continue and complete coordination with Florida East Coast Railway for railroad crossing
rehabilitation.
A. Compensation for services outlined above will be in lump sum amount of $9,350.00.
B. Revise Section V.1 of Master Agreement:
The total Not -to -Exceed amount of $73,657.00 is hereby changed to $83,007.00.
A. Receipt of an, executed copy of the Amendment shall constitute tate CO[ NIT ,"S .notice to
the CONSULTANT to proceed with the work.

B. All provisions of the AGREEMENT not specifically notified shall remain in full force and
effect.
C. This Amendment regardless of where executed, shall be governed by and construed
according to the laws of the State of Florida.
IN WITNESS WHEREOF the parties have executed this Amendment this March 7,
2000.
