I. Project Description
Provide engineering services for renewal of the industrial waste permit for the North
County Water Treatment Plant (WTP).
II. Scope of Services
Engineering services include preparation of the necessary permit applications and
assemblance of the necessary renewal documentation required in accordance with the
facility permit and F.A.C. 62-620.
111. Consulting Engineer Insurance Requirements
A. Workers Compensation Insurance in accordance with Florida Statutes.
B. Comprehensive and Automotive Liability with a minimum coverage of
$100,000/$300,000 per occurrence for bodily injury or accidental death.
C. Comprehensive General Liability with a minimum limit of $100,000/$300,000
covering property damage.
D. Liability for Property Damage, while operating motor vehicle, with minimum
limits of $100,000 per occurrence.
E. Contractual Liability including limits established for Items IIIB, C, and D above.
F. Umbrella coverage, excess liability with minimum limits of $100,000 per
occurrence.
IV. Compensation for Services
Task 1200 - Engineering Services for Industrial Waste Permit Renewal -
North County WTP $15,000 Upper Limit
Compensation for Task 1200 is to be paid on a salary cost basis plus other direct costs on
an as -needed basis. (See Attachment B-1).

A. Salary Cost Basis with Upper Limit
For the Basic Services performed on a salary cost basis with an upper limit, IRC
agrees to pay CDM as follows:
For engineering services performed by CDM, the payment will be equal to the
salary cost of such services for each employee times 2.3 for overhead and profit
plus actual out-of-pocket expense costs. Salary cost is equal to 1.393 times direct
salary costs and is defined as the cost of salaries (including sick leave, vacation, and
holiday pay applicable thereto) for time directly chargeable to the project; plus
unemployment, excise, and payroll taxes; and contributions for social security,
employment compensation insurance, retirement benefits, and medical and other
group insurance benefits.
Actual out-of-pocket expense costs are all costs other than salary costs that are
incurred during the progress of the work. The actual out-of-pocket expense costs
include: air fare, automobile rental if required, mileage charges, parking tolls, taxi,
meals, lodging, telephone, printing and reproduction costs, and other
miscellaneous costs incurred specifically for this project. The maximum cost for
blueprint sheets is $1.10. The maximum cost for mylar sheet is $11.00 and a sepia
sheet is $5.00. The charges for in-house computer program and word processor
usage will be at CDM's regular rates. For outside computer services, charges will
be made at invoiced cost to CDM.
For the basic services performed on a salary cost basis with an upper limit, IRC agrees to
provide a written request before work is started by CDM.

SCOPE OF WORK
Task 1200 - ENGINEERING SERVICES FOR INDUSTRIAL WASTE PERMIT RENEWAL
NORTH COUNTY WATER TREATMENT PLANT
Background
The North County Water Treatment Plant was completed and started up in June 1998. Raw
water is supplied by three wells constructed into the Floridan Aquifer. Raw water is pumped to
the facility and treated by reverse osmosis (nanofiltration) membranes. A byproduct of the
membrane treatment process is a concentrated solution high in chlorides. The facility is
currently permitted to discharge up to 750,000 gallons of concentrate to the Indian River, a
water body of higher chloride concentration. Toxicity testing is required by the existing permit.
The current facility industrial waste permit expires on February 9, 2001. The permit and Florida
Administrative Code (F.A.C.) 62-620 require that renewal applications and supporting
information be submitted to the Florida Department of Environmental Protection (FDEP) at least
180 days prior to the expiration date.
Services
This authorization would provide engineering services to complete the following permit
application forms with assistance from County operations and administration staff:
■ Wastewater Permit Application Form 1 —General Information (DEP Form 62-620.910(1)).
® ■ Wastewater Permit Application Form 2A for Domestic Wastewater Facilities (DEP Form 62-
620.910(2)).
■ Wastewater Application Form 2CS — Permit to Discharge Process Wastewater from New or
Existing Industrial Wastewater Facilities to Surface Water (DEP Form 62-620.910(5)).
Supporting documentation required by each application form will be assembled and attached for
formal submittal to FDEP. County staff shall provide any information necessary for submittal.
Indian River County shall pay all permit renewal/application fees.
