ENGINEERING WORK AUTHORIZATION
Date: Work Authorization No. 15 for Consulting Services
County No. 9901-15 Camp Dresser & McKee Inc. (Consultant) CDM Project No.
I. PROJECT DESCRIPTION
Consulting Engineering Services for the installation of a new groundwater monitoring well,
the abandonment of two monitoring wells, and the installation of a continuous water stage
monitor as required by Florida Department of Environmental Protection permit conditions.
Surveying services including verifying the elevations of monitoring wells and observing the
coordinate of Aerial Targets are included in this project.
II. SCOPE OF SERVICES
Reference is made to Attachment B, the attached Scope of Work and the "Professional
Engineering Services Master Agreement for Solid Waste."
III. CONSULTING ENGINEER INSURANCE REQUIREMENTS
The following insurance requirements will be provided in accordance with the Master
Services Agreement:
A. Workers Compensation Insurance in accordance with Florida Statues.
B. Automotive Liability with a coverage of $1,000,000 for a combined single limit.
C. General Liability with a limit of $2,000,000 for general aggregate coverage.
D. Professional Liability with an aggregate limit of $3,000,000.
IV. COMPENSATION FOR SERVICES
Task 1. Including Sub -Tasks 1.1-1.7

Lump Sum Cost Elements
For the basic services performed for each task for to a lump sum fee, Indian River County Solid
Waste Disposal District Agrees to pay Camp Dresser & McKee Inc. partial payments to be made
on a monthly basis based on the percentage of work completed.

I. PROTECT DESCRIPTION
Consulting Engineering services related to the preparation of a biennial water quality
report covering the last two years (1999-2000) of landfill water quality data. The report
is to be submitted to the Florida Department of Environmental Protection (FDEP), as
required by Chapter 62-701 of the Florida Administrative Code and the SWDD's FDEP-
issued landfill operation permit.
II. SCOPE OF SERVICES
Reference is made to Attachment B, the attached Scope of Services, and the
"Professional Engineering Services Master Agreement for Solid Waste."
