ENGINEERING SERVICES WORK ORDER 25
This Work Order Number 25 is entered into as of this 9thday of December -,2025,
pursuant to that certain Continuing Contract Agreement, dated May 2, 2023, ("Agreement"), by and
between INDIAN RIVER COUNTY, a political subdivision of the State of Florida ("COUNTY") and
Kimley-Horn & Associates, Inc. ("Consultant").
The COUNTY has selected the Consultant to perform the professional services set forth on
Exhibit A (Scope of Work), attached to this Work Order and made part hereof by this reference. The
professional services will be performed by the Consultant for the mutually agreed upon lump sum or
maximum amount not -to -exceed professional fee. Any additional costs must be approved in writing,
and at a rate not to exceed the prices set forth in Exhibit B of the Agreement (Rate Schedule) for RFQ
2023015, made a part hereof by this reference. The Consultant will perform the professional services
within the timeframe more particularly set forth in Exhibit A (Time Schedule), attached to this Work
Order and made a part hereof by this reference all in accordance with the terms and provisions set
forth in the Agreement. Pursuant to paragraph 1.4 of the Agreement, nothing contained in any Work
Order shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed
to be incorporated in each individual Work Order as if fully set forth herein.

EXHIBIT A
SCOPE OF WORK
The County desires to expand the existing pedestrian and bike lane networks and
enhance their interconnectivity between communities. The County desires to design and
permit 6 -foot sidewalks and 6 -foot paved buffered bike lanes along both sides of 43rd Avenue
between 6th, Street and 12th Street, approximately 4,000 linear feet. This project will connect
to and expand the pedestrian and bike lane facilities currently under construction within the
43rd Avenue Pedestrian & Bicycle Improvements between 12th Street and 18th Street. In
addition to the pedestrian and bike lane improvements, the County desires to include milling
and resurfacing plans for 43rd Avenue within the project limits.
A. Topographic Design Survey:
The Consultant will provide topographic design survey services necessary to support and
facilitate contemplated design and permitting activities associated with this project. This task
will consist of the preparation of digital base map topographic surveys in accordance with the
Standards of Practice, established by the Board of Professional Surveyors and Mappers, Chapter
5J-17, Florida Administrative Codes, pursuant to current Section 472.027, Florida Statutes, per
5J-17.050 through 5J-17.052. The survey will include:

1. Establishment of a project wide Horizontal Control Network within the PROJECT LIMITS.
A Design Survey Baseline will be established and monumented. A minimum of three
Design Survey Baseline monuments will be installed.
2. Establishment of a project wide Vertical Control Network within the PROJECT LIMITS. All
elevations will be referenced to NAVD 1988 datum.
The limits of the topographic design survey are as follows:
43rd Avenue - between 6th Street and 12th Street, approximately 4,000 linear feet.
Topographic survey shall extend no less than 15 feet beyond the right-of-
way.
B. Geotechnical Investigation:
The Consultant will retain the services of a Geotechnical sub -consultant to assist in
evaluating the existing soil and roadway pavement conditions. The Geotechnical sub -
consultant will provide the following Geotechnical Services:

1. Field Exploration Program
• Six (6) pavement cores along the shoulder alternating sides
• Ten (10) 6 -feet deep hand augers alternating side of the roadway
2. Engineering Analysis and Report
The Geotechnical Sub -Consultant Engineer shall perform analysis of all the data obtained to
evaluate subsurface conditions and develop engineering recommendations to guide site
preparation procedures and foundation support.

2. Plans shall be prepared on 11" x 17" sheets.
3. Each phase submittal shall include drawings in PDF format.
4. Plan sheets shall depict existing right-of-way, section lines, property lines, and
centerline of construction. Horizontal control points with state plane
coordinates for all PC's, PT's, curve radius, curve length and horizontal PI's shall

be included on the Plan or summarized in an alignment table.
5. Plans shall include spot grades adequate to describe any proposed grading. Spot
grades will be provided to delineate ADA regulation compliance.
6. Match lines shall not be located within the limits of an intersection.
7. All quantities shall reference FDOT Pay Item Numbers.
8. All details shall reference FDOT Index Numbers.
9. All specifications shall reference to FDOT Specifications for Road and Bridge
Construction. Any deviations are special specifications not included in FDOT
Specifications are required in the Technical Specifications.
10. Preliminary (60% completion), interim (90% completion) and Final Plan (100%
completion) Submittal shall include the following:
i. One (1) set of drawings in PDF format.
ii. One (1) Opinion of Probable Construction Cost
The plans will be prepared based upon English units. Final design files will be submitted in
AutoCAD format.
D. Signing and Pavement Marking Plans:
Signing and Pavement Markings will be included within the Roadway Plan set. Quantities will
be provided in the Summary of Pay Items sheet. All signing and pavement markings are to be
prepared in accordance with the latest design standards and practices (MUTCD), FDOT
Standard Specifications, Indexes, and shall be accurate, legible, and complete in design.
E. Type I Categorical Exclusion:
The Consultant will attend one (1) kickoff meeting with the County to discuss the project
details. The Consultant will review readily available GIS databases to determine listed species
known in the area or those species that could potentially occur within the project corridor or
immediate vicinity. This will also include a review of the existing land use/land cover as mapped
by the St Johns River Water Management District (SJRWMD) and the existing soil survey. The
Consultant will also request information from the Florida Natural Areas Inventory (FNAI) and
the US Fish and Wildlife Service (USFWS) IPaC Trust Resources Report. Lastly, the Florida
Department of Environmental Protection (FDEP) OCULUS database will be researched for
existing contamination data within and adjacent to the project corridor. Following this desktop
review, the Consultant will conduct field reconnaissance to review the project corridor.
Representative photographs will be taken, and a photographic log will be prepared.
The Consultant will complete a Type I Categorical Exclusion checklist along with the

environmental support document and accompanying maps. The maps prepared will include a
location map, US Geological Survey (USGS) Topographic map, Natural Resources Conservation
Service (NRCS) soils map, land use/land cover map, Federal Emergency Management Agency
(FEMA) flood zone map, and a listed species map, if applicable. Upon approval from the County,
the project will be entered into the Statewide Environmental Project Tracker (SWEPT).
F. Cultural Resource Assessment Su
These services will be conducted in compliance with Section 106 of the National Historic
Preservation Act (NHPA) of 1966, Chapter 267 of the Florida Statutes (F.S.), Chapter 1A-46
(Archaeological and Historical Report Standards and Guidelines), Florida Administrative Code
(FAC), the conditions and specifications in the Florida Division of Historical Resources (FDHR)
Module 3: Guidelines for Use by Historic Preservation Professionals (2003), and the FDHR June
2022 CRAS Sufficiency Checklist. The Principal Investigator will meet the Secretary of the
Interior's Professional Qualification Standards (48 FR 44716) for archaeology, history,
architecture, architectural history, or historic architecture.
Archival Research
Background research will include a review and analysis of the Florida Master Site File (FMSF)
data to identity previous surveys and previously recorded resources within the project area and
vicinity. An analysis of available historic and modern aerial photographs, historic General Land
Office (GLO) plat maps and associated surveyors' notes, United States Geological Survey (USGS)
7.5 -minute topographical maps, historic soil surveys, and pertinent environmental variables will
also be conducted. This analysis will be used to reconstruct past land use and assess the
potential for archaeological sites and historic resource within the project areas. Precolumbian
period and historic period contexts will also be reviewed and developed to provide a
framework within which the archaeological and historical records can be understood. A
summary of the project area's archaeological and historic contexts and pertinent environmental
features will be prepared from the results of the archival research. This information will inform
the survey strategy and provide the context in which the significance of any resources identified
during the project can be evaluated.
Cultural Resource Assessment Survey
The CRAS will include a survey to identify both archaeological sites and historic resources.
Based on the proposed improvements, the area of potential effect (APE) will be narrowly
defined and only resources directly within the APE will be documented.
Archaeological Survey
The archaeological survey will include a reconnaissance level survey to document existing
conditions and determine where archaeological subsurface testing is feasible. Any area where
archaeological subsurface testing is possible will be marked and coordination with the Sunshine
811 Call Center will be conducted to locate any underground utilities. Areas where excavation is
not possible will be documented with field notes and photographs. Subsurface tests will be .5 m

(20 inches) in diameter and dug to a minimum depth, subsurface conditions permitting, of one
meter (3.3 feet). All excavated soils will be sifted through 6.4 mm (% inch) metal hardware cloth
screen suspended from portable wooden frames. Any cultural materials recovered will be
stored in plastic bags with all provenience data recorded. Field notes on each test performed
will be recorded, and the location of all tests will be clearly marked on aerial photographs of the
project area. A Florida Master Site File form will be completed for any archaeological site
identified during the survey.
Historic Resources Survey
A historic structures survey will be conducted to ensure that historic resources constructed in
1974 or earlier are identified and documented. Resources directly within the APE will be
identified and documented on Florida Master Site File forms (FMSF). Photographs will be taken
and a log kept recording the resource's physical location and compass direction of each
photograph. Each resource's individual significance will then be evaluated for its potential
eligibility for listing in the National Register of Historic Places (National Register). Historic
physical integrity will be determined from site observations, field data, and photographic
documentation.
Documentation and Report Preparation
A draft report presenting the methods, findings, evaluations, and recommendations of the
cultural resource assessment will be prepared and submitted to Kimley-Horn for review and
comment. The draft report will be submitted electronically. Any comments will be incorporated
into the final report. The Consultant will prepare and submit the final report and associated
documentation to the Florida State Historic Preservation Officer (SHPO)/FDHR.
G. Jurisdictional Permitting:
The Consultant shall prepare jurisdictional permit applications necessary to support the
proposed improvements. The following agencies are anticipated to require applications/review
to facilitate the proposed improvements:
• Saint Johns River Water Management District (SJRWMD) —Environmental Resource
General Permit
• Indian River Farms Water Control District (IRFWCD) — Connection Permit
• Florida Department of Transportation (FDOT) — State Electronic Review Comment
(ERC) system
The Consultant has included $2,500 as a reimbursable expense to address SJRWMD and
IRFWCD application fees. The County will be responsible for any mitigation bank costs
necessary to mitigate potential environmental impacts, if necessary.

In accordance with your request CivilSury Design Group, Inc. is pleased to propose to provide a Topographic Survey for a portion
of 43rd Avenue, Vero Beach, Florida.
The Limits of the Topographic Surveys are as follows:
1) The Northern limit is South edge of pavement of 43Rd Avenue and 1211 Street
2) The entire width of 43rd Avenue Right -of -Way and 10' East and West of the existing Right -of -Way
3) The Southern limit is the South Edge of pavement of 611 Street.
The Survey shall adhere to the Minimum Technical Standards as set forth by the Board of Surveyors and Mappers pursuant
to Rule 5J-17 of the Florida Administrative Code.
Horizontal datum shall be based on the East Zone of the Florida State Plane Coordinate System.
1) Have 4 vertical control points for future construction. Vertical datum will be dependent on nearest vertical control to project.
2) Provide cross sections ant 50' intervals
3) Locate all striping, culverts etc. within the project limits
4) Locate all trees 6" or greater
5) Spot elevations at 50' intervals including intermediate highs and lows
The Lump sum fee per for the Topographic Survey is $25,750.00.
Shipping fees are not included.
Deliverables will include an AutoCAD drawing and points text file of the Topographic Survey as well as a PDF file.

Dear Mr. Good:
Ardaman & Associates, Inc. is pleased to submit this proposal for performing a subsurface soil
exploration and pavement cores for the above referenced project site. We understand that the
project entails the design and construction of approximately 4,000 linear feet of concrete sidewalk
along 43rd Avenue from 6th Street to 12th Street. The purpose of our field-testing program is to
explore shallow soils and groundwater levels in the vicinity of the proposed sidewalk and to obtain
thickness measurements of the existing pavement materials (asphalt, base material and stabilized
subgrade) at the various locations on 431d Avenue.
The following summarizes the proposed scope of work and associated fees for conducting the
subject exploration.
FIELD EXPLORATION
Based provided information, our field exploration will be performed within existing residential
roadways with heavy traffic volume. Therefore, maintenance of traffic (MOT) is necessary to
perform the requested field exploration, and we have estimated one (1) day of MOT.

The auger borings will be drilled in the green areas using a hand-held 3 -inch diameter bucket
auger. Each sample will be removed from the auger in the field and then examined and visually
classified by our crew chief. Representative portions will be sealed and packaged for
transportation to our laboratory for further analysis as required. Water level observations will be
made in the boreholes during the drilling operation. It is noted that the hand augers may terminate
at depths shallower than 6 feet if refusal to penetration and/or borehole collapse during drilling.
The asphalt cores will be performed on the shoulders using a portable coring machine equipped
with a diamond -tipped core barrel. The locations of the cores will be selected by an Ardaman
engineer based on site observations. The thicknesses of the asphalt, base material, and stabilized
subgrade (if present) will be measured within the core holes and recorded by our field personnel.
Representative samples will be sealed and packaged for transportation to our laboratory for
further analysis as required. The core holes will be filled with cold -patch asphalt. We have
assumed that one day of Maintenance of Traffic (MOT) will be required for the job.
LABORATORY TESTING
Routine laboratory visual classification will be performed along with specific classification tests
deemed necessary (i.e., percent fines and organic content). We note that no asphalt content
testing, extraction/gradation testing or Limerock Bearing Ratio (LBR) tests are being proposed at
this time. All laboratory tests will be performed in general accordance with applicable ASTM
standards.
ENGINEERING ANALYSIS AND REPORT
Engineering analysis of all data obtained will be made to evaluate general subsurface conditions
and to develop engineering recommendations to guide site preparation within the proposed
sidewalk area. We will also provide the pavement core results. Our recommendations, together
with data developed during the exploration, will be submitted in a written report upon conclusion
of the study.
COST ESTIMATE
The costs associated with the aforementioned tasks will not exceed below estimates without prior
authorization from the client.

CLOSURE
This proposal is subject to the following conditions: (1) access to boring locations is to be readily
available to our equipment and field personnel, (2) 1 day of MOT will suffice, (3) the proposed
number of borings and the boring depths will be adequate, (4) undisturbed samples and
consolidation tests on fine grained soils are not budgeted into the total cost, (5) Ardaman &

Associates will not take responsibility for damages to underground structures and/or services that
are not located by Sunshine One -Call; their locations are to be provided by the client or owner
prior to commencement of the field work, (6) exploration or evaluation of the environmental
(ecological or hazardous/toxic material related) condition of the site and subsurface is not
included, and (7) permits, including right of use and well, are not required.

Our basic services include a Cultural Resources Assessment Survey (CRAS) to idertify archaeological sites and historic
sources within the Area of Potential Effect, which is assumed to be within the 43rd Avenue ROW. The CRAS will be conduct
accordance with FDHR's Cultural Resource Management Standards and Operational Manual (February 2003). Requests for
restigations that exceed FDHR standards and requirements will be subject to a supplemental scope of work and associated

2. Background research to include review of Florida Masser Site File (FMSF), historic maps, historical aerials, GLO field
surveyors notes, sods, USGS topographic maps, local designations, development of archaeological probability and development
of archaeological and historic contexts.
3. APE will be limited to footprint of improvements.
4. Per the Underground Facility Damage Prevention and Safety Act,' Chapter 556, Florida Statutes, coordination will be
conducted with the Sunshine 811 Center to locate underground utilities prior to any archaeological subsurface testing
5. Archaeological Field Survey; 2 people /3 days to include travel, Sunshine 811 white lining and utility locates coordination ,
subsurface testing, where feasible, documentation of existing conditions, preparation of field maps, and pre and post field
briefing. Preliminary research suggests a low potential for archaeological sites. Subsurface tests will be in accordance with
SHPO/FDHR standards for a maximum of 10 shovel tests.
6. No historic resources are anticipated.
7. CRAS Report will be in accordance with Chapter 1A-46 (Archaeological and Historical Report Standards and Guidelines), and
the CRAS Sufficiency Checklist. A draft and final report will be prepared as well as a SHPOIFDHR submittal package (CD with
electronic riles, photographs, GIS shapefiles, hard copies of report, hard copies of survey log).
8. Please note that hours include one round of responses to SHPO/FDHR coordination and one rounds of revision to the report,
excluding additional field time or research.
9. If human remains are identified, this scope includes the initial coordination with the State Archaeologist required by Chapter
872, F.S. Costs for additional investigation or coordination will be negotiated, as needed.
10. Costs for additional research or excavations deemed necessary to evaluate National Register of Historic Places (National
Register) eligibility, Section 106 Determination of Effects for National Register -eligible resources, Section 106 consultation, or
the development of a Memorandum of Agreement (MOA) are not included in this scope and will be negotiated later, if required.
