All services performed under this contract shall be supervised and certified by a licensed Professional Engineer (PE) registered with the State of Florida and staff employed directly with your firm. SCOPE OF SERVICES All work shall be performed on a unit price or hourly rate basis in accordance with the attached Materials Testing Services — Revised Proposal (Exhibit A) dated June 23, 2015 and shall comply with Standards enumerated in the ANNUAL GEOTECHNICAL SERVICES CONTRACT, RFQ 2014007 WITH DUNKELBERGER ENGINEERING & TESTING, INC. Effective Date: October 1, 2014. Minimum Specification Requirements as follows: • Field Observation, Sampling & Testing • Laboratory Testing • Engineering & Reporting All work shall also comply with the current Florida Minimum Technical Standards. COMPENSATION The COUNTY agrees to pay, and the Geotechnical Professional agrees to accept, a fee for services rendered according to the "Materials Testing Services — Revised Proposal" identified in the attached "EXHIBIT A" Fee Schedule and Man Hours. We have revised our proposal based on your email to us on June 15, 2015, requesting that we submit our materials testing proposal on a per test unit rate basis instead of an hourly basis. We are submitting this proposal under Indian River County's Professional Geotechnical Services Agreement 2014-2016, RFQ # 2014007. We have based our scope of work information presented in the project plans provided to us, prepared by Indian River County Department of Public Works Engineering Division, dated November 20, 2014, and our past experience with similar projects for Indian River County. It should be noted that the project has not been let out -to -bid, so a general contractor schedule is not currently available. A. PROJECT INFORMATION We understand that the subject project is located along 87th Street and 91st Street and will involve the installation of new storm drainage pipe/structures; the construction of 9,100 lineal feet (10 of sidewalk; and driveway reconstruction. This project an FDOT Local Agency Project (LAP) Financial Project No. 431728-1-58-01. The purpose of our work is to provide quality assurance (QA) testing to check the conformity of the materials composition and their construction with the project specifications. The governing standards and specifications are the FDOT Standard Specifications for Road and Bridge Construction, dated 2015, and the FDOT "Design Standards" dated 2015, and Technical Specifications Addendum No. 1. SCOPE OF SERVICES Page 2 Based on our understanding of the review of the project plans and specifications, and our past experience, we envision that our field materials sampling/testing services will be required on a periodic basis over the duration of the project. We will provide qualified technicians on a per test unit rate basis. More specifically, our work will involve the following scope outlined below: Field Observation, Sampling & Testing • Earthwork Observations, if necessary, by an engineering technician during the site grading operation including the proof -rolling of "strip grade" for the embankment construction as requested by Indian River County. • Materials Sampling by an engineering technician and delivery to the laboratory for testing of the materials used for embankment, storm drainage backfill, and sidewalk/driveway subgrade. • In-place density testing will be performed during placement of the embankment fill for roadway embankment construction. The density tests will be performed at a frequency of approximately one (1) test for every 200 lineal feet of roadway/embankment within the proposed alignment, for each 1 -foot thickness of backfill placed. • In-place density testing during backfilling of storm drains and catch basins for each 1 - foot lift of backfill placed from the bottom of the structure up to an elevation corresponding to final grade. We have assumed the contractor will be utilizing "thick lift" placement. • In-place density testing during backfilling of the rigid and flexible pipelines. At each roadway or driveway crossing, density tests will be performed for each 6 -inch lift vertically of backfill placed starting at an elevation corresponding to the invert of the pipe and extending up to the final grade. • In-place density tests for the sidewalk and driveway subgrade prior to concrete placement. • Sampling, slump testing, molding, transporting, curing, and laboratory testing to determine the compressive strength of concrete placed during construction (sidewalk, driveways, and concrete endwall). Laboratory Testing • Perform laboratory moisture -density relationship (Proctor) tests (AASHTO T-180 and AASHTO T-99), organic content tests, and sieve analysis (percent fines for soil classification purposes) on the embankment fill and pipe backfill soils in accordance with FDOT specifications. • Limerock Bearing Ratio (LBR) tests, if required, for stabilized subgrade and base course beneath driveways or other select areas. • Concrete cylinder test specimens will be tested following ASTM C39 for their compressive strength at 7 days and 28 days after placement. Our materials and observation testing services will be provided on an as -needed basis as dictated by the pace and methods of construction. C. COMPENSATION The material testing and observation services provided will be billed in accordance with the Indian River County Professional Geotechnical Services Agreement 2014-2016, RFQ# 2014007 and the line item unit rates illustrated on the Projected Scope and Budget (Attachment A). Dunkelberger proposes to complete the scope of services as described herein for an estimated fee of $17,490.00. If additional services outside the listed project services are required, Dunkelberger will provide such under auspice of a Supplemental Proposal using the same line item unit rates as provided herein. The quantity of the work performed and factors beyond our control, such as weather and the contractor's schedule/efficiency, will affect the actual cost for our services. You will be invoiced at regular intervals but not less than monthly for services performed. Invoices for our services will be directed to the addressee of this letter unless we are directed otherwise. Payment is due within thirty (30) days following receipt of an invoice. D. SITE ACCESS AND SAFETY Indian River County (IRC) shall secure all necessary site related approvals, permits, licenses, and consents necessary to commence and complete the services and will execute any necessary site access agreement. Our Pre -Task Plan will identify and prepare our personnel to be able to handle conditions such as but not limited to traffic control, environmental contamination, site access issues, overhead and underground utilities, adverse weather conditions, and personal protection equipment and will continually be reviewed and reevaluated throughout the field work activities. We understand that each site is unique and may contain different safety conditions and as a company to protect our personnel as well as others, we look at each site individually to identify the potential concerns. E. TESTING AND OBSERVATION The above services will be provided under an as -requested basis. Additionally, we request a 48-hour notice to schedule our services. IRC understands that testing and observation are discrete sampling procedures, and that such procedures indicate conditions only at the depths, locations, and times the procedures were performed. Dunkelberger will provide test results and opinions based on tests and field observation only for the work tested. IRC understands that testing and observation are not continuous or exhaustive, and are conducted to reduce — not eliminate — project risk. IRC agrees to the level or amount of testing performed and the associated risk. IRC is responsible (even if delegated to contractor) for notifying and scheduling Dunkelberger so Dunkelberger can perform these services. Dunkelberger shall not be responsible for the quality and completeness of IRC or its contractors work or their adherence to the project documents, and Dunkelberger's performance of testing and observation services shall not relieve contractor in any way from its responsibility for defects discovered in its work, or create a warranty or guarantee. Dunkelberger will not supervise or direct the work performed by IRC or its contractors and is not responsible for their means and methods. F. AUTHORIZATION Dunkelberger appreciates the opportunity to submit this proposal and looks forward to rendering the services described herein.
