CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY
WHEREAS, the Indian River County Council on Aging has requested authorization to provide Wheelchair Transportation services that
originate within Indian River County;
WHEREAS, the above named service affirms that it will maintain compliance with the requirements ofthe Indian River Code of ordinances (Chapter
304, Life Supportand Wheelchair Services).
THEREFORE, the Board of County Commissioners of Indian River' County hereby issues a Class "E" Certificate of Public Convenience and
Necessity to said Company to provide Wheelchair Transportation services within Indian River County .
Certificate Type: Class E
Date issued : October 2 , 2003 Date of expiration : October 2 , 2005
(unless certificate is sooner revoked or suspended)
Limitations : Wheelchair transportation services only.
Maintain Contract with Community Transportation Coordinator.
