The following changes are hereby made .
Decreased the amount of 12 and 8 " PVC Pipe installed , decreased the number of Fire
Hydrants installed , eliminated fittings , eliminated wet tapping sleeve and valve , added
bends , eliminated directional bores and decreased amount of sod .
Original CONTRACT PRICE ( LABOR ONLY ) $ 472 , 714 . 50

I . GENERAL
This change order is necessary to cover changes in the work to be
performed under this Contract .
II , REQUIRED CHANGES
Increase/decrease certain item quantities to reflect actual field conditions
III . JUSTIFICATION
To accommodate existing utility conflicts encountered in the field .

Funds for the proiect have been approved by the BCC from the Utilities Department
Fund , Account No . 472- 169 -000- 02529 .
WAIVER
This Change Order constitutes full and mutual accord and satisfaction for
the adjustment in contract price or time as a result of increases or
decreases in costs and /or time of performance caused directly and
indirectly from the change .
This Change Order represents an equitable adjustment to the contract and
CONTRACTOR hereby waives all rights to file a claim on this Change
Order after it is properly executed .
IV . APPROVAL AND CHANGE AUTHORIZATION
Acknowledgements :
The aforementioned change , and work affected thereby , is subject to all
provisions of the original contract not specifically changed by this Change
Order; and ,
It is expressly understood and agreed that the approval of the Change
Order shall have no effect on the original contract other than matters
expressly provided herein .
