Dear Commissioners :
As we have in the past three years, The Treasure Coast Homeless Services Council Inc. is
requesting that you vote to allow the Chairman of the Board of County Commissioners to
sign, as applicant, for two projects which will be submitted to HUD for funding under the
2003 HUD SuperNofa grant request.
We ask that you consider this item on the Agenda for the meeting of July 8, 2003 .
The Treasure Coast Homeless Services Council, Inc . is not requesting financial assistance
from Indian River County to meet the services or cash match of these two projects .
The First Project is Indian River County Tenant Rental Assistance for 13 Chronically
Homeless Adults, under the Shelter Plus Care Program. This is a five year award.
The Second Project is Treasure Coast HMIS Project to expand the Homeless
Management Information System in use in our Continuum of Care. This is a three year
award.
A synopsis of both projects follows :

consolidated application is requesting up to $400, 140. 00 in Tenant Based Rental
Assistance to provide for permanent affordable rental housing for at least thirteen
chronically homeless mentally ill and/or substance abusing adults from Indian River
County. The Indian River County Department of Human Services will administer the
tenant based rental assistance and will oversee the delivery of services to the assisted
tenants to assure that the required services match is met. New Horizons of the Treasure
Coast, The Mental Health Association, The Homeless Assistance Center and The
Homeless Resource Center will use the services they provide to these clients to meet the
in-kind services match requirement. This is a five year grant period.

The Indian River County Board of Commissioners is serving as the applicant in this
consolidated application requesting up to $75 , 568 . 00 in Supportive Housing Program
funds to develop a new project to expand an existing dedicated Homeless Management
Information System (HMIs) Project. The County Department of Human Services will
administer the grant funds and the Treasure Coast Homeless Services Council, Inc. will
act as the sponsor. The new project will provide for additional licenses for new HMIS
users, and expand technical assistance availability and training for new programs and
services participating in Service Point, the Continuum of Care' s dedicated FMS . The
Treasure Coast Homeless Services Council will provide the statutory match amount of
$ 192 392. 00. Funds have been set aside in this amount for this project, if awarded This is
a three year grant period.

EXHIBIT THREE : PROJECT PRIORITY # TWO
Section A. Project Narrative
1. Project Summary :
The Indian River County Board of Commissioners as the statutory applicant in this
consolidated application is requesting up to $400, 140. 00 in Tenant Based Rental
Assistance to provide for permanent affordable rental housing for at least thirteen
chronically homeless mentally ill and/or substance abusing adults in Indian River County.
The Indian River County Department of Human Services will administer the tenant based
rental assistance and will oversee the delivery of services to the assisted tenants to assure
that the required services match is met and that tenants maximize the access to
mainstream resources other affordable housing to which they are or may become
entitled. Selected tenants will be expected to rent units in Indian River County which
meet the threshold for housing quality standards under 24CFR982 . 401 .
Indian River County Department of Human Services delivers a wide range of services to
indigent and homeless individuals and families in Indian River County. As the Social
Services arm of the Indian River County local government, it provides information and
referral, access to entitlements, rental assistance, funds for emergency prescriptions,
transportation vouchers, case management, referrals for medical assistance and funds for
indigent burial .
2. Homeless Population to be served.
Shelter Plus Care funds are being requested to develop and deliver a new tenant based rental
assistance project over a period of five years to house and serve these chronically homeless
individuals who are mentally ill, some of whom also exhibit chronic substance abuse. These
clients are primarily the long term chronic population served by New Horizons of the
Treasure Coast, The Mental Health Association and Family Preservation Services who have
difficulty maintaining housing and continually end up on the streets due to the nature of their
disabilities. Many of these clients are living in camps in the woods behind public buildings in
the city of Vero Beach. Others are "living" in abandoned housing which is condemned but
has not yet been torn down, just outside the city limits in Gifford. These buildings have no
plumbing, no water and no electricity. These funds will meet the clients ' needs for clean,
safe, affordable housing and should improve their ability to stay engaged in treatment.
This population primarily exhibits severe mental illness, often with chronic substance abuse.
Many also have serious physical health problems. The Shelter Plus Care Committee of the
Treasure Coast Continuum of Care will select the tenants and coordinate the long term
supportive services required to keep these individuals in the housing. The Shelter Plus Care
Committee is chaired by the County, who is acting as applicant. Recipients of the rental
assistance will be required to remain housed in Indian River County over the five year period
of the rental assistance in order to assure access to their major service providers who monitor
their medication and provide other outpatient services .
Clients will come from New Horizons of the Treasure Coast, the Mental Health
Association, The Source Ministerial Outreach Program and the Homeless Assistance

Center Shelter. They may come directly from detox units, short term residential treatment
stays, outpatient treatment and the streets . New Horizons of the Treasure Coast'.s
professional case management staff will engage the target population in the program. The
Source will assist with engagement, through sober support groups and meeting client
basic needs. The Treasure Coast Homeless Services Council ' s Resource Center will
support this project by providing furniture and household item and by working with
professional staff to secure entitlements for this population.
3 . Discharge Planning Changes ,
As part of it ' s ongoing effort to participate in discharge planning which prevents
discharge into homelessness, the County Commission Chair and the Director of the
County Department of Human Services are part of a new Law Enforcement Partnership
Committee, convened by the Treasure Coast Homeless Services Council, Inc, in
February of 2003 . This Law Enforcement Partnership includes city and county law
enforcement for both Indian River and St. Lucie County, two county judges and a wide
variety of service providers, especially New Horizons of the Treasure Coast which
provides residential mental health and substance abuse treatment to homeless
individuals. The Source, which works exclusively with chronically homeless adults is
also a significant member of this Committee. While no new written policies have
emerged, as yet, because the applicant has no jurisdiction, several "practices " have been
agreed upon among the committee participants and several new mutual understanding of
the "issues " on both side have emerged. This ongoing committee is anticipated to have
an impact on eliminating the revolving door syndrome chronically. homeless people
experience .
As in the past, Indian River County still has no direct jurisdiction over prisons, jails,
hospitals or institutions but continues to work with the Department of Children and
Families and FACT Teams to monitor the housing status and ongoing community
maintenance of clients released from State Mental Institutions.
By serving as the applicant for this project, Indian River County is also taking steps to
facilitate access to permanent affordable housing for individuals who would otherwise
be discharged from the county jail, local treatment programs and hospitals into
homelessness .
4. Housing where participants will reside.
Participants will be assisted in selecting their own housing arrangements by the
Continuum of Care ' s Shelter Plus Care Committee which locates apartments in safe
neighborhoods for Shelter Plus Care and other hard to place clients . Various living
arrangements , including single units and group homes have been pre-identified and
approved by the Committee to ensure that tenants will have access to housing that suits
their needs and preferences and is also accessible to community amenities including
public transportation. Accessibility to the principal service provider managing the client ' s
care remains as a key factor. Apartments will be inspected by the Indian River PHA to
ensure safety and compliance with all threshold requirements following the Fair

Housing guidelines . Clients in Indian River County will also have access to the Housing
Coordinator through the Homeless Resource Center to assist them with finding housing
when they are ready to move from TRA to Section 8 or other permanent housing options .
However, the key factor in determining if a unit or a placement is optimum will be the
client ' s ability to access ongoing services and whether or not the proposed housing
arrangement supports the client' s ongoing re-socialization and supportive services needs.
b. Community amenities and transportation :
Basic community amenities and transportation are readily available in the County,
especially in the City of Vero Beach. Free public transportation is available on fixed bus
routes throughout the City of Vero Beach. New Horizons of the Treasure Coast clients
can also access transportation through the F. A. C . T. Team case managers. Indian River
Memorial Hospital and the County Health Department, the only providers of indigent
health care in the County, are located in the City of Vero Beach and are both within
blocks of the Resource Center. All facilities are accessible by public transportation.
c . That the apartments will be accessible to persons with disabilities.
There are a number of group homes and converted assisted living facilities which have
been serving as landlords for in Indian River County' s Shelter Plus Care clients when
handicapped accessibility is required. There are additional units available if necessary. It
is reasonable to expect that an individual with a need for handicapped accessibility will .
not have difficulty locating an appropriate unit.
Ideally, tenants should be located in apartments around the City of Vero Beach which
provides the best accessibility to Indian River Memorial Hospital, The Health
Department, numerous churches and food pantries, public transportation and the
Homeless Resource Center. The Homeless Resource Center and the New Horizons FACT
Team also have cars and vans available to transport clients to services and appointments.
5. Supportive services the participants will receive
Supportive services will be secured and offered by New Horizon' s of the Treasure
Coast 's professional treatment staff. These include ongoing medication management,
mental health counseling and support and substance abuse counseling and support. In
some cases, representative payees will be appointed to manage client funds . Residents
will be encouraged to participate in the CoC ' s specialized intensive job rehabilitation
program, which was originally designed with this population in mind. It is anticipated
that most of the tenants will opt to stay actively engaged in long term supportive services
like those described above due to the benefits derived from housing stability. Therefore,
they will become more willing and able to work toward appropriate social and
employment goals . The Shelter Plus Care Committee will monitor each tenant ' s case plan
on a monthly basis to assure that case managers continue to work with each tenant to
maximize their income levels through the receipt of all appropriate entitlements related to
their indigence and disability. Residents will be asked to sign an agreement with their primary

case manager, before entering into the project, that they will stay in Indian River County and will
comply with their treatment plan.
Specialized training for this population's disability needs will include :
Medications Management
Mental Health Counseling and Ongoing Case Management
Drop-In Center Re-Socialization
Participation in Self-Help Recovery Support Groups
Consumer Skills
Nutrition Education
6. Self-Sufficiency
A comprehensive plan will be created for each client by . the primary case manager. A
new case record will be opened and maintained for each client using the Continuum ' s
new FMS system, Service Point. It will track all efforts to determine and secure
entitlements for which the clients are eligible. At a minimum, ongoing medication and
treatment requirements and potential for increased income through employment and
mainstream entitlement will be addressed. Entering this information will be the
responsibility of the particular case manager who is responsible for assisting the client
through the process . Every client in the Shelter Plus Care Program is automatically
personally assisted with securing Medicaid, SSI, SSDI, Vet Benefits, Food Stamps and
Supplementary Medication Assistance by the primary case manager who recommends the
client to the program. The Resource Center provides additional assistance to the clients
until the process is complete. The process includes( 1 )automatically applying for Section
8 on behalf of each client, (2) completing simultaneous paperwork with the client for
various entitlements all at once, (3 ) making the appointments, (4) following up on
appointments, (5) securing transportation if necessary, (6) selecting representative
payees, when necessary, (6) advocating on behalf of the client with social security and
disability determination staff and (7) securing free legal advocacy for clients who are
denied, when necessary. The responsible service provider(s) will be identified in the
plan, as well as other agencies who will participate in service delivery. A monthly report
of services received by each client is provided to the Shelter Plus Care Committee which
monitors service activity and the level of increase in income and entitlements for each
client. Where feasible, employment training and job finding assistance will also be
provided by the Homeless Resource Center to these clients to support a goal of at least
40 % of these clients moving into Section 8 or their own housing during the five year
grant period.

Section B . Experience Narrative
1 .The specific type and length of experience of all organizations involved in implementing the proposed project.
The applicant is the Indian River County Board of Commissioners. Indian River County' s
Total Expenditures of Federal Awards for the Year Ended September 30, 2002 is
$4,390,296 . 00 . The Total Department of Housing and Urban Development Expenditures for
Fiscal Year Ended September 30, 2002 is $ 1 ,274, 712 . 00 .
This project will be administered on behalf of Indian River County Board of Commissioners
by the Indian River County Department of Human Services. This Department has twenty
eight years of experience in grants and program administration. The Department is proficient
in intake, needs assessment, information and referral, eligibility determinations and record
keeping. They provide a variety of direct services to meet the emergency health and financial
needs of the indigent and homelessness in Indian River County. The Department administers
emergency financial assistance, utilities assistance, temporary housing assistance (rent
subsidy) , emergency medications funds and other health related financial assistance to the
poor and needy in the County, including the indigent burial program. Screening for food
stamps, TANF and other AFDC is conducted at the Department ' s offices. This Department is
also the administrator of the Children ' s Services Advisory Council Grants Program and is
responsible for the RFP, selection process and grants awards and administration. The
Department' s annual operating budget is $446, 000 . 00 for case management and emergency
services and $ 1 ,000, 000 . 00 for Children' s Services.
Since August of 2002, This Department has administered the HUD2001 Shelter Plus Care
Tenant Rental Assistance Program which now successfully provides rental assistance for
fourteen formerly homeless disabled adults who are living in apartments around Indian River
County and is prepared to administer the HUD 2002 Shelter Plus Care Tenant Rental
Assistance Program award which begins on July 1 , 2003 . The County successfully managed
the HUD 2000 award for SHP, Transitional Housing (Rehabilitation). Renovations and
additions are successfully completed. The building has been issued a certificate of occupancy
in May 2003 . Five families are occupying the transitional housing beds added through this
HUD 2000 TH award. Indian River County also administered $ 1 ,205,683 . 00 in Section 8
rental assistance vouchers for the Fiscal Year Ended September 30, 2002.

F. 1 . Tenant" based Rental Assistance (TRA) Project Budget
Applicants requesting TRA must complete the chart below showing the number of units expected to be used in
your program. Multiply the applicable existing fair market rents (FMRs) as published in the Federal Register (FR) on
September 30, 2002, by the number of units of a given size by 60 months. [Please be advised that the actual FMRs

Discharge Policy
Required of all State and local government applicants. Submit this certification along
with the HUD form HUD424. (you may submit a single certification covering all of
your projects .)
I hereby certify that as a condition for any funding received as a result of this competition,
our government agrees to develop and implement, to the maximum extent practicable and
where appropriate, policies and protocols for the discharge of persons from publicly
funded institutions or systems of care (such as healthcare facilities, foster care or other
youth facilities, or correction programs and institutions) in order to prevent such
discharge from immediately resulting in homelessness for such persons. I understand that
this condition for award is intended to emphasize that States =4 units of general local
government are ,primarily responsible for the care of these individuals, and that
McKinney-Vento Act funds are not be to used to assist such persons in place of State and
