A . M . ENGINEERING & TESTING , INC .
All services performed under this contract shall be supervised and certified by a licensed
Professional Engineer ( PE ) registered with the State of Florida and staff employed directly with
your firm .
SCOPE OF SERVICES
All work shall be performed on a unit price basis in accordance with the attached Proposal
( Exhibit A ) dated April 3 , 2009 and shall comply with Standards enumerated in SECTION III of
the RENEWAL OF THE ANNUAL GEOTECHNICAL SERVICES CONTRACT RFQ 2007081
WITH A . M . ENGINEERING AND TESTING INC . Effective Date : November 18 , 2008 .
All work shall also comply with the current Florida Minimum Technical Standards .
COMPENSATION .
The COUNTY agrees to pay , and the Geotechnical Professional agrees to accept , a fee for
services rendered according to the Proposal ( Exhibit A ) as attached .
A Not4o - Exceed Amount of: $ 13 , 980 . 00 to be paid on a unit price basis for actual work
performed in accordance with the schedule of services and fees set forth in SECTION V of the
RENEWAL OF THE ANNUAL GEOTECHNICAL SERVICES CONTRACT RFQ 2008071 WITH

Per our discussions and correspondence , A . M . Engineering and Testing , Inc . is pleased to
submit this proposal for performing geotechnical services for the referenced project . The project
consists of mast arm foundations at 6 intersections . We understand that our field services ,
geotechnical reports , and construction CADD sheets must be completed by May 1 , 2009 . The fee
estimate for our services is attached .
