DRAINAGE IMPROVEMENTS

INDIAN RIVER COUNTY, FL
As result of discussion with and comments from Indian River County Staff during the
finalization of the construction plans for Phase IV of the CR-512 Roadway Improvements, it has
been noted that work not included in the original contract or subsequent addendums must be
performed in order to ensure a successful project. The anticipated additional work shall include
turn lanes, the redesign of the 101st Avenue/CR-512 intersection, the evaluation of existing
drainage structures on the Sebastian Middle School site, an updated environmental survey of the
Phase IV alignment, the design and permitting of a sidewalk extension along the north side of
CR-512, and the design of a north-bound left tum lane on 101st Avenue at CR-512.
In order that the project design may be modified and permits obtained, we will perform the
following "Additional Services":
Task A — Phase IV Construction Plan Modifications
We will add the following design elements to the construction plans as requested by the County:
1 . Additional Rip_ht-Turn Lanes: In accordance with the County' s request, we will design right
turn lanes at the following locations:
a. 101st Avenue (east-bound)
b. 104th Avenue (west-bound)
c. 106th Avenue (west-bound)
d. 108th Avenue (west-bound & east-bound)
2. Redesign of 101st Avenue Intersection: The County has proposed to realign 102"d Avenue
north of CR-512 to meet the intersection of 101 " Avenue and CR-512. As such, the Phase
IV construction plans will be revised to illustrate a plateau intersection at that location with a
tumout to the north for the future connection of 102"d Avenue.
3 . Traffic Signal Interconnection: In order to extend traffic signal interconnect capability along
the Phase IV portion of CR-512 we will incorporate the design of double 2-inch conduit with
related pull boxes into the Phase IV construction plans. The design of the traffic interconnect
conduit will be coordinated with the County' s Traffic Engineering department.

4. Existing Stormwater Structure Evaluation: Using as-built information recently provided by
the County, we will evaluate existing stormwater management pipes on the North County
Regional Park and Sebastian Middle School sites. The original stormwater report for the CR
512 project proposed culverts for the Pond B discharge and outfall to a then existing drainage
ditch. The completion of the North County Park site directly north of Pond B piped a portion
of said ditch including the point at which Pond B' s outfall was to connect. Since the size and
length of North County Park project pipes as constructed differ from that included in the
original model, we will revise the model to include the as-built pipe data. The model will
then be run to determine whether the existing structures will accommodate the discharge
flows from CR-512 's Pond B, the North County Regional Park and the Sebastian Middle
School. As part of this task we will discuss the model results with the County Engineer and
make recommendations for any required remedial design.
We propose to perform the Scope of Services outlined under Task A for the following lump sum
fee : $ 22,515.00
Task B — Threatened and Endaneered Species Survey and Wetland Functional Analysis
An environmental assessment was performed for the entire CR-512 Roadway Improvements
project in 1998 by our Sub-consultant, Environmental Services, Inc. (ESI). Since that time, the
Phase IV alignment has been revised to the south of the existing roadway and will pass through
and adjacent to environmentally sensitive areas. In order to address environmental concerns
prior to the commencement of construction the County has requested that an updated Threatened
and Endangered Species Survey and Wetland Functional Analysis be conducted. We shall
provide the following scope of services under this task:
1 . Threatened and Endangered Species Survey
ESI will conduct pedestrian transects through the various vegetative habitats within the
proposed roadway expansion corridor and stormwater management ponds A, B and C
pursuant to the methodologies of the Florida Fish and Wildlife Conservation Commission
(FFWCC), to observe evidence of any local state or federally protected plant or animal
species, specifically any evidence of gopher tortoise (Gopherus polyphemus). A brief letter
report will document the results of the survey.
Note: The limits of the roadway corridor and stormwater management ponds A, B and C will
be staked in the field by others prior to the commencement of the surveys. This task only
assumes one ( 1 ) survey for gopher tortoise. If the survey is conducted more than 90 days
before land disturbance, an updated survey will be required and is not part of this proposal.
ESI will map the locations and status of gopher tortoise burrows to determine the extent of
the on-site gopher tortoise population. If observed, ESI will hang flags on the vegetation
adjacent to a burrow and a map will be prepared indicating the location of the burrow. This
task does not include any gopher tortoise permitting.

2. Functional Assessment of Wetlands
Environmental Services, Inc. (ESI) will assess the proposed wetland impact areas and the
mitigation areas within the Department of Environmental Protection (DEP) owned lands
using the Uniform Mitigation Assessment Method (UMAM), which is utilized by both the
U.S. Army Corps. of Engineers (CE) and the St. Johns River Water Management District
(SJRWMD).
Note: ESI assumes that no additional mitigation will be necessary for SJRWMD as proposed
impacts for Phase IV are approximately less than or equal to the previously proposed impacts
for Phase IV. ESI will be shown the locations of all mitigation previously conducted on DEP
lands used for proposed impacts for Phase III. If proposed impacts to wetlands for Phase IV
are not offset by mitigation previously conducted, additional mitigation may be required that
is not part of this proposal.
3 . Gopher Tortoise Permitting and Relocation (Optional)
In the event transects to be completed under Item # 1 above reveal the presence of tortoise
occupation within the Phase IV limits, ISE will conduct the following tasks :
a. ESI will prepare and submit a gopher tortoise relocation permit application to the
FFWCC for off-site relocation of tortoises if any gopher tortoises are identified within the
roadway expansion corridor or stormwater management ponds A, B and C. ESI will
coordinate with FFWCC staff to facilitate timely permit issuance.
Note: ESI assumes that gopher tortoise(s) will be relocated to another County-owned
property and blood testing to determine the presence or absence of Upper Respiratory
Tract Disease Pathogen will not be necessary.
b. Upon Receipt of the FFWCC relocation permit, ESI will recapture the tortoise (s), either
by hand ( if outside the burrow) or backhoe excavation, and will remove and transport
tortoise (s) to the designated off-site recipient area.
If excavation is by backhoe, the backhoe and operator will be provide by the applicant.
We propose to perform the Scope of Services outlined under Task B for the following lump sum
fees:
1 . Threatened and Endangered Species Survey: $ 1 ,320.00
2. Functional Assessment of Wetlands (if Necessary): $ 19980.00
3 . Gopher Tortoise Permitting and Relocation (ifNecessarv): $ 1 ,650.00

Task C — Additional Sidewalk Design & Permitting
Masteller & Moler, Inc. shall provide professional engineering services for the design,
permitting, bidding and services for the construction of sidewalk improvements to the north side
of County Route 512 from 108'h Avenue west under the 1-95 overpass to the western limit of the
FDOT Limited Access Easement, a distance of approximately 1 .750 linear feet.
This work is being implemented to provide professional engineering services by Masteller &
Moler, Inc. associated with the project described above. The work to be performed will
generally include the preparation of base plans, construction plans, permit applications and cost
estimates to support the construction of the sidewalk.
The Scope of Services for Task C is provided below:
I — SurvevinQ Data Coordination
The proposed sidewalk extension is beyond the limits of Phase IV roadway
improvements. Masteller, Moler, Reed, & Taylor, Inc. (MMRT) has performed a route
survey of both Phase IV and Phase V of CR-512 to support the design of roadway
improvements. The survey data illustrates all improvements along CR-512 at the I-95
interchange. Under this task, we will coordinate with MMRT to obtain their route survey
and incorporate it into our base plans.
2 —Base Plans
The proposed sidewalk extension is beyond the limits of Phase IV roadway
improvements. Base Plans will be developed from the existing CR 512 Phase IV and
Phase V Route Survey and Design Plans for Phase IV CR-512 Roadway Improvements.
The base plans shall include coordinate points with reference to the State Plane
Coordinate System to facilitate the incorporation of the plans into the County' s GIS
system.
3 — Construction Plans
Masteller & Moler, hie. will utilize the previously completed route survey information
and the CR 512 Phase IV roadway designs for preparation of construction plans for the
sidewalk. Our plans will show the sidewalk, cross-walk striping at roadway crossings,
and cross-sections at pertinent locations.
4 - Permitting
Masteller & Moler, Inc. shall prepare and submit applications for the following permits:
• Florida Department of Transportation (FDOT) General Use Permit
• Florida Department of Transportation (FDOT) Drainage Permit
• Indian River County — Right of Way Permit
Permit application fees are not included in this Proposal.

We propose to perform the Scope of Services outlined under "Task C" as follows:
1 - 4 (Survey, Base Plans, Construction Plans, & Permitting): $ 11 ,510.00
Task D — Improvements to 101't Avenue at CR-512
Masteller & Moler, Inc. shall provide professional engineering services for the design,
permitting, bidding and services for the construction of a north-bound left tum lane on 101 "
Avenue at CR-512. This tum lane will replace the existing left tum lane at the subject location
since the existing lane will be truncated by the proposed CR-512 improvements.
The proposed tum lane will be designed as a center widening similar to the existing turn lane,
and will provide 2801f of storage with a 501f taper. The transition to the two-lane section will be
designed in accordance with the design speed of 101 " Avenue and discussion with the County
Engineer and Assistant Public Works Director.
The Scope of Services for Task D is provided below:
1 — Route Survey
Using data from their survey of the CR-512 right of way, our partner company, MMRT,
will conduct a survey of the 101 " Avenue right of way to a point approximately 1 ,000 If
south of the proposed CR-512 south edge of pavement. The survey will illustrate the
horizontal and vertical location of all visible improvements including existing pavement,
residential driveways, drainage features, utility poles, etc. This survey will serve as the
basis of the 101 " Avenue roadway design.
2 —Base Plans
Using the route survey provided by MMRT we will develop Base Plans for the 101 't
Avenue Improvements to match the proposed CR 512 Phase IV Roadway Improvements.
The base plans shall include coordinate points with reference to the State Plane
Coordinate System to facilitate the incorporation of the plans into the County' s GIS
system.
3 — Construction Plans & Cost Estimate
We will develop Construction Plans to detail the proposed improvements along 101 "
Avenue and the connection to proposed CR-512 improvements at the north end of project
and the existing 101 " Avenue roadway at the south end of the project. Our plans will
include a summary of quantities, typical cross-section, plan view, centerline and swale
profiles, cross-sections at each roadway station, and details of construction elements. We
will also prepare a Construction Cost Estimate as part of this subtask.

4 - PermittinQ
Discussion with SJRWMD indicates that the improvements to 10151 Avenue will qualify
for a Permit Exemption since the improvements will not increase the amount of untreated
impervious area. Additional stormwater permitting is not included.
Masteller & Moler, Inc. shall prepare and submit applications for the following permits:
• Indian River County — Right of Way Permit
• SJRWMD Permit Exemption
Permit application fees are not included in this Proposal .
We propose to perform the Scope of Services outlined under "Task D" as follows:
I Route Survey: $ 3 , 150.00
2 - 4 Base Plans, Construction Plans, & Permitting: $ 13 , 150.00

We will complete the above described Scope of Services in a diligent manner after our receipt of
written authorization to proceed from Indian River County.
