In accordance with Contract No . 0390 This Addendum No. 2 to Work Order No . 6 is in accordance with the existing AGREEMENT dated September 23 , 2003 , and the extension thereof, between Kimley- Horn & Associates , Inc . (SURVEYOR ) and Indian River County (COUNTY) . This Addendum No. 2 Work Order No . 6 amends the agreement as follows : SECTION I — PROJECT LIMITS : Kings Highway Extension South (5e Avenue), Phase 1 The SURVEY LIMITS shall begin at a point on Kings Highway that is 1500' south of 17th Street SW and continue south on Kings Highway to a point that is 1000' south of 21 " Street SW (approximately 2 , 100' ) . Lateral B Canal will be surveyed for a distance of 500 ' north and 500' south of the following intersections; 131h Street SW , 17" Street SW and 21s' Street SW . (approximately 3 , 000') . Canal B-8, Canal B-9 and Canal B- 10 will be surveyed for a distance of 200 ' east and 200 ' west of Kings Highway/Lateral B Canal (approximately 1 , 200 ') . Project Survey Control ( Baseline) shall extend to and connect to the County line at 25' street S .W . SECTION II — SCOPE OF SERVICES The existing Kings Highway Extension (58th Avenue) South , Phase 1 Topographic and Control Survey that was completed on October 5 , 2005 by Kimley- Horn and Associates , Inc . will be revised to include the following : 1 ) Additional cross section elevations of Kings Highway will be obtained from a point that is 1500' south of 17th Street SW and continue south on Kings Highway to a point that is 1000 ' south of 215` Street SW (approximately 2 , 100 ') . 2) Additional cross-section elevations of Lateral B Canal will be obtained at 50 ' intervals , for a distance of 500' north and 500' south of the following streets ; 13th Street SW, 17th Street SW and 21 " Street SW . (total distance for Lateral B Canal cross-sections is 3 ,000 ') . 3 ) Retracement of Sections 32 and 33 , Township 33 South , Range 39 East , including the field location of controlling section , tract and individual boundary corner monuments in order to map those right of way and parcel lines within the project limits . 4) Location of the Indian River County Benchmarks within the project limits . 5) Location of the control point' s established/mapped by Nick Miller, Inc . at the corners of Sections 32 and 33 , Township 33 South , Range 39 East. 6) Establishment of field baseline and references along Kings Highway within the project limits. Project Survey Control (Baseline) shall extend to and connect to the County line at 251h street S .W . 7) Preparation of Certified Corner Records for the controlling monumentation within Sections 32 and 33, Township 33 South , Range 39 East. SECTION III - TIME FOR COMPLETION Addendum No. 2 tasks shall be completed within 90 calendar days from Notice to Proceed . Pg2of3 SECTION IV — DELIVERABLES/WORK PRODUCT ( Final product) a) Survey shall be supplied as a topographic survey (3 certified hardcopies) and base map (Cadd) and ( PDF) file to IRC . b ) Said base map file shall include , survey horizontal and vertical control and subsequent topographic data sufficient for anticipated design and planning needs . c) Survey horizontal/vertical control sheet shall be included establishing and referencing control as to facilitate baseline re-establishment and construction . References and Bench Marks shall be logistically placed outside proposed limits of construction and be comprised of durable material . d) The SURVEYOR shall prepare its Work Product Digital versions so that the COUNTY or other consultants can readily use it for design and analysis of the area defined . It shall contain the information necessary for a third-party surveyor to independently recreate and/or utilize the survey work. It is acknowledged that all final products become property of Indian River County and will be available for use by the public at large . e ) The SURVEYOR'S work product shall meet or exceed the minimum standards as defined by ALL sections heron or the COUNTY will not approve the SURVEYORS request for payment. SECTION V — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for the services rendered according to the " Scope of Services " identified in all Sections of this Addendum to Work Order No . 6 , a Lump Sum of $33 , 885 . 00 The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein .
