This ADDENDUM to Work Order No . 6 ( Engineering ) is in accordance with the existing
AGREEMENT dated September 23rd , 2005 between Carter Associates , Inc . ( SURVEYOR )
and Indian River County ( COUNTY ) .
This Work Order No . 6 ( Engineering ) amends the agreement as follows :
SECTION I — PROJECT LIMITS :
ADDITIONAL SERVICES related to the 40 ' postion and modificationions of the Boundary
and topographic survey of the +/- 5 acre proposed maintenance complex site :
Reference attached Exhibit "A" scope of services .
SECTION II — SCOPE OF SERVICES :
This Work Order No . 6 ( Engineering ) is for providing ADDITIONAL SERVICE to the original
agreement and shall include all Professional Survey services related to the task and services as
indicated in the attached Exhibit "A" Proposal and scope of additional services supplied by
Carter Associates , Inc .
SECTION III — TIME FOR COMPLETION :
All tasks outlined shall be completed , in its entirety within 30 calendar days of notice to
proceed . ( The County shall be notified upon completion of tasks and/or of any delay beyond
specified periods ) .
SECTION IV — DELIVERABLES :
Three ( 3 ) certified final survey " hardcopies " and an electronic file/disc in state plane
geometry ( AutoCAD 2005 or previous versions ) shall be delivered upon completion .

SECTION IV — COMPENSATION :
The COUNTY agrees to pay , and the SURVEYOR agrees to accept , a fee for services
rendered according to the " Scope of Services " identified in Section I , II , III and IV of this
Work Order No . ( Engineering ) , a Lump Sum of $ 3 , 350 . 00 .
All invoicing shall be supported with documentation ( Man hours ) of time and materials spent
prior to payments .
The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections
of the AGREEMENT shall remain in full force and effect , and are incorporated herein .
IN WITNESS WHEREOF the parties hereto have executed these presents this
22nd day of August 2006 .

1 . ) Revise the boundary of the five ( 5 ) acre site to exclude the additional 40 foot right-of-way
along Hobart Road and to include the additional area along the north property line
( approximately 57 feet - north and south ) to net 5 acres total . Set 5 additional concrete
monuments for revised boundaries .
2 . ) Topographic Survey of the additional area along the northern portion of the five acre site
( approximately 82 feet - includes compensation for the additional 40 foot right-of-way and the
additional 25 feet north of the north boundary line )
3 . ) Tree Location Survey of the additional 82 feet , more or less , along the northern portion of
the five acre site to include all protected trees with a trunk diameter of six ( 6 ) inches or greater .
4 . ) Prepare sketch and description of the additional 40 foot right of way from IRFWCD Lateral J
Canal right-of-way west to Kings Highway right-of-way .

COST OF PROFESSIONAL SERVICES :
Indian River County hereby agrees to pay $3 . 350.00 for the above-stated professional services within 30
days of the date of the invoice, in addition to interest of 1 . 5% per month ( 18% per year) on any balance unpaid after
30 days and any costs of collection including , but not limited to , lien costs , court costs or attorneys ' fees involved in or
arising out of the collection of any unpaid or past due balances .
