29tTihis Amendment Number 1 to Work Order 2 ( "Amendment" ) is entered into as of
this day of March , 200 5 ( " Effective Date" ) pursuant to that certin Continuing
Contract Agreement for Professional Services entered into as of 8pril 6 2004
( "Agreement" ) , by and between Indian River County , a political subdivision f the State of
Florida ( "COUNTY" ) and Kimley- Horn and Associates Inc . ( "CONSULTANT " i .
1 . The COUNTY has selected the Consultant to perform th professional
services set forth in Work Order Number 1 dated April 13 , 2004 .
2 , The COUNTY and the Consultant desire to amend Work Order Number 1
as set forth in Attachments 1 and 2 attached to this Amendment and made a part hereof by
this reference , all in accordance with the terms and provisions set forth in the Agreement .
3 . From and after the Effective Date of this Amendment , Work Order Number
1 is amended as set forth in this Amendment . Pursuant to paragraph 1 . 4 oft a Agreement ,
nothing contained in any Work Order shall conflict with the terms of the Agreement and the
terms of the Agreement shall be deemed to be incorporated in each individL al Work Order
as if fully set forth herein .
IN WITNESS WHEREOF , the parties hereto have executed this Amendment as of
the Effective Date first written above .

Proiect Description and Understanding
This Amendment Number 1 modifies the scope of work in Work Order #2 for the survey, design , and
construction tasks of the new Northwest Regional wastewater pump station . The original scope included
the design of a conventional pump station which included a wetwell , submersible pumps , and odor
control system. Through discussions with staff, there is a desire to construct an in-line pump station with
building in lieu of a conventional pump station in order to eliminate odors and reduce maintenance
typically associated with conventional pump stations . Therefore , the following amendment includes
scope modifications to provide additional survey for an alternate site , design for a building to house the
pumps , piping, electrical , generator, and electrical room, and provide construction phase services for the
building and structure .
SCOPE OF SERVICES
The following tasks are included :
Task 1 — Survey
KHA will provide survey for an alternate site located adjacent to the playground on the Northwest corner
of the school on the County common area . The site survey will include a boundary survey , topo , and
other above ground information in that area . Additional survey for an alternate route fort e forcemains
to the west of the site will also be included .
Task 2 - Design
The design of a CBS structure to house the pumps , piping, generator, electrical , and controls will be
included . The approximate size will be 500 sq ft footprint with architectural treatments, ar d include an
access road (shellrock equivalent) to the facility from 64tb Ave .
All other design requirements as stipulated in the original work order #2 will be included .
Task 3 —Construction Phase Services
Consultant will provide additional construction phase services for the construction of the b jilding,
including bidding and contract administration, shop drawing review, coordination and mee Ling
attendance, construction observation , start-up services , preparation of record drawings , inc uding
electronic format in AutoCadd R14 version in real world coordinates on CD, mylars, and f nal
certification of completion of the project. It is anticipated that the duration of construction phase (notice
to proceed to final completion) will be five to six (5 -6) months and that on-site construction observations
will be as appropriate with the level of construction activity.
Consultant will answer contractor questions and prepare addendums, as needed which will be distributed
to all the contract document holders . Consultant will attend a pre-bid meeting at the project site or main
office . Consultant will sell project manuals from our office , including drawings, specifications , and
generation of all addendums .

Shop drawing submittals will be reviewed for conformance with the contract documents , hop drawings
will be submitted directly to Engineer. Consultant will review the submittals and return copies to the
contractor and Indian River County Utilities .
Consultant will provide coordination of project status meetings and observation of construction for the
duration of the project . Consultant will also review pay requests and provide recommenda ions for
payment to Indian River County Utilities for the progress of the work.
Construction observation will be provided by a field inspector . Total observation time will be
approximately 100 hours . The field inspector will observe construction in order to determine if the
project is being constructed in accordance with the contract documents . Work that Consultant believes
does not comply with the contract documents will be brought to the attention of Indian RiN er County
Utilities and the contractor.
Start-up of the new pumps will be facilitated by the pump manufacture ' s representative and by the
Contractor. Consultant will observe start-up of the new pumps once the installation is completed .
All other construction phase observations as stipulated in the original work order #2 will be included.
FEES :
Consultant will provide these Services as described herein on a lump sum basis for the foll wing fees :
