INTERSECTION IMPROVEMENTS AT .2d� 07V
CR-510 AND US HIGHWAY I
AMENDMENT NO . 1 TO THE PROFESSIONAL CIVIL ENGINEERING
GEOTECHNICAL ENGINEERING & LAND SURVEYING SERVICES AGREEMENT
BETWEEN McMAHON ASSOCIATES, INC . & INDIAN RIVER COUNTY, FLORIDA
This is an amendment to the existing Professional Civil Engineering, Geotechnical Engineering and Land
Surveying Services Agreement (AGREEMENT) dated May 17 , 2005 , between McMahon Associates, Inc.
(ENGINEER) and Indian River County (COUNTY) . This amendment addresses changes in "Section I — Project
Limits and Description", "Section II — County Obligations", "Section III — Scope of Services", "Section IV — Time
for Completion" and "Section V — Compensation' of this AGREEMENT.
"SECTION I - PROJECT LIMITS AND DESCRIPTION" is being modified to incorporate the following:
Consultant services are required for the preparation of' supplemental design survey, right-of-way maps,
parcel sketches, and legal descriptions, subsurface geotechnical investigations, roadway design plans, signing and
marking plans, signalization plans, and permit applications associated with the design of improvements extending
both west and east of the intersection of SR-5/US- 1 and CR-510 (Wabasso Road), from the present contract limits in
Indian River County, Florida. The roadway improvements identified by the COUNTY consist of the following:
Countv Road 510 Widening
CR-510 will be widened to a four-lane cross-section from the current intersection limits, west to 61 " Drive
(Lows Avenue), and from the current intersection limits, east to the lower bridge crossing the Indian River Lagoon.
Along the Wabasso Scrub Conservation Area, widening is anticipated to be accomplished on the south side with an
anticipated right-of-way width of 130 feet. In conjunction with the improvements to CR-510 , the following
improvements are proposed to the bisecting side streets:
• 59ih Avenue — Proposed paved apron on south approach with no median opening.
• 58'x' Cour( — Proposed paved apron on south approach with no median opening.
• 58°i Avenue/Kings FIighway — Design the south leg to accommodate an exclusive left-tum lane, a
shared lett-through lane, and an exclusive right-tum lane. The left-tum lane on CR-510 to southbound
58" Avenue may be widened to a dual left-tum lane. For purposes of this proposal , this has been
considered. Proposed paved apron on the north approach. Intersection improvements shall include
mast arm signalization components. Approximately 2 ,640 linear feet of improvement.
• 55i ' Avenue - Proposed paved apron on north approach with a median opening and left turn/u-tuns
lanes on CR-510.
• Old Dixie Highway — Construct curb and gutter to eliminate north leg of intersection . Design south leg
with raised concrete to allow right-in-right-out movements only. There will be no median opening at
this location .
• US- I — No change to layout, as proposed in the original agreement.
• 85" Place/47" Avenue — Combined paved apron on north approach with no median opening.
• 86"i Street — Proposed abandonment on north approach with no median opening.
• 46'" Avenue - Proposed abandonment on north and south approaches with no median opening.
• Future PUD location between US- I and Shore Lane — Design signalized intersection. Include mast arm
signalization components. Include exclusive left turn/ u-turn lanes both eastbound and westbound on
CR-510. Include exclusive right turn lane, eastbound to southbound , on CR-510.
• 87"i Street - Proposed paved apron on north approach.
• Shore Lane - Proposed paved apron on south approach.
• 85'" Street — No access to CR-510 or to US- 1 .
The total length of improvements on CR-510 is approximately 8, 172 linear feet with an additional 4,790
linear feet of bisecting side street improvements for a total length of 12,962 linear feet, or 2.45 miles. 3 ,700 linear
feet of the total length of improvements along CR-510 was included in the original contract. Therefore, the total
length of additional improvements is 9,280 linear feet or 1 .75 miles.
The ENGINEER shall provide curbed outside lanes, sidewalks, crosswalks, and ADA compliant curb cut
ramps throughout the project limits. New drainage structures and pipes along these supplemental project sections
will also be required.
"SECTION II - COUNTY OBLIGATIONS" is being modified to incorporate the following:
J . The COUNTY shall be responsible for preparing all signal phasing and timings, queue lengths and
related analysis for the intersection of 58" Avenue at CR-510, and at the intersection where the Future
PUD is proposed, between US- 1 and Shore Lane. The COUNTY shall determine if the intersection of
CR-510 at 581h Avenue warrants a dual left-turn lane from CR-510 west to 58`" Avenue south .
"SECTION III - SCOPE OF SERVICES" is being modified to incorporate the following:
The ENGINEER agrees to perform professional roadway design and related services in connection with the
project as required and set forth in the following:
B . Design Survey
The ENGINEER shall provide design survey services necessary to support and facilitate
contemplated design and permitting activities associated with this supplemental portion of the
project. No subsurface utility locating and no utility designating is included .

7. The DESIGN LIMITS shall be extended to include the following :
• 3, 700 linear feet along CR-510, west of the original survey limits
• 1 ,000 linear feet along CR-510, east of the original survey limits
• 2,640 linear feet, south of CR-510, along 58" Avenue
• 250 linear feet along the following side streets: 59" Avenue, 58"i Court, 58'h Avenue
(north), 55ih Avenue, 85'h Street, 47" Avenue, 87'" Street, and Shore Lane
• 50 linear feet along the following side streets: 86ih Street, 46" Avenue, and 44`"
Avenue
B1 . Right-of-WaySurvev
It is assumed that the FDOT will provide the County with a Control Survey/30% RIW Maps for
this project.
The Right of Way Survey will not be prepared for any roadways other than CR-510. The Right-of-
Way Survey will consist of a Cover Sheet, one Key Sheet, and seven detail sheets depicting
existing right of way line, and proposed right of way lines, etc, in compliance with the attached
"Exhibit B ", as provided by Indian River County. The Right-of-Way Survey will also include a
Tabulation Sheet depicting parcel take information.
Detail sheets will depict existing planimetric features as existing in the current design topo by
FDOT.
Key Sheets will be at a scale of F = 300' and Detail Sheets will be at a scale of I " = 50' . The
surveys will be delivered on mylar.
It is assumed that the County will provide Title Reports, in a timely manner and other available
survey control information in the possession of the County. Legal Descriptions and Sketches will
be prepared for approximately 70 parcels at a per unit fee of $375 .
B2 . Control Survey
The Control Survey will consist of a Cover Sheet, one Key Sheet, and 7 Detail Sheets depicting
existing right-of-way lines, monumentation, etc. , in compliance with the attached "Exhibit B", as
provided by Indian River County.
Cl . Geotechnical Investigation — Supplemental Services Amendment I
To aid in the design of the additional roadway sections, stormwater management facilities and
signalization foundation designs an outline for the proposed geotechnical services, as provided by
our subconsultant Ardaman & Associates, Inc. , is shown below:

1 . Conduct visual reconnaissance along the proposed roadway corridors. Include
photographic record of typical, key or anomalous site features noted during the
reconnaissance. Review aerial photographs, information from the Soil Survey of Indian
River County, Florida, and any geotechnical data furnished by the COUNTY. The
Geotechnical subconsultant may rely upon information furnished by the COUNTY
without independent review and verification.
2 . Perform up to four Standard Penetration Test (SPT) borings to determine soil parameters
(i . e., unit weight, friction angle and cohesion), an estimated wet season water table level
and recommendations for drilled shaft construction for use in design of drilled shaft
foundations, relative to the design of mast anti signal poles. The borings will be
performed to a depth of approximately 35 feet.
3 . Perform up to 38 five-foot auger borings within the proposed alignment to explore the
shallow soil stratigraphy and groundwater depths. Eight auger borings are expected to be
performed relative to intersection improvements at 58th Avenue. Thirty auger borings are
expected to be performed along CR-510.
3 . Conduct up to 10 South Florida Water Management District (SFWMD) exfiltration tests
to obtain soil hydraulic conductivity values for the design of runoff water retention and
detention areas, as required by the Client. The SFWMD tests will be performed in
accordance with methods described in the SFWMD Permit Information Manual, Volume
IV.
4 . Obtain three cores from the existing roadway at 58th Avenue. The cores will be obtained
using a four-inch diameter diamond-tipped core barrel. The thicknesses of asphaltic
concrete, base and subgrade will be measured, and the core holes will be backfilled and
patched upon completion.
5 . Obtain soil samples from the auger and SPT borings for laboratory testing. This includes
routine classification tests including standard sieve analysis, Atterberg Limits, organic
contents, etc.
6. Additional borings, if needed, will be obtained by amendment to delineate any found
muck/organic deposits, plastic soils or other deleterious subsurface conditions. This work
element may also include the performance of additional field exftltration testing to further
explore permeability and hydraulic conductivity values of soils.
7 . Note any rubbish, trash, debris or auger refusal, if encountered. Also note any
contamination with petrochemicals evident visually or through odor or the soils
encountered . If petrochemicals are encountered, notify the COUNTY immediately.
Approximately backfill all borings after recording ground water levels. Work shall be
done at times that will minimize traffic disruptions (avoid AM and PM peak traffic
hours). Provide appropriate lighting and safety traffic control devices such as flashing

message/arrow boards during the work. Coordinate with utilities prior to start of coring
activities.
8 . Prepare a report including all data developed during the laboratory and field programs,
and our recotmnendations relative to soil and groundwater parameters to be used in the
design.
The typical Geotechnical Report will include:
• Discussion of site regional geologic or physiographic setting.
• Discussion of general and specific subsurface conditions encountered by the
exploration.
• Discussion of any adverse soil conditions (such as muck, high plasticity clays,
etc.) encountered during the exploration. Include recommendations for remedial
actions (removal, stabilization, etc .) for any adverse conditions found.
• Discussion of any petroleum or other hazardous contamination encountered by
the borings. Identify and map locations of concern based on the reconnaissance.
• Boring profile reports.
• Recommendations for additional study, if appropriate.
• Recommended Limerock Bearing Ration (LBR) for pavement design subgrade
and base courses. No LBR testing will be performed as a part of this work
effort.
• Recommendations for soil construction for roadway including base material,
subgrade and embankment preparation and compaction.
• Estimated hydraulic conductivity of soils encountered on left and right side of
roadway based on performance of exfiltration tests performed in the field, and
typical relations between relative density, grain size, and soil type.
D. Roadway Desien Plans
Plans shall be prepared to include :

The following additional data shall be utilized for development of the plans.
Ia. Typical Sections:
Supplemental portions of CR-510 will be designed as a 4-lane divided section in
coordination with the FDOT PD&E Study, and as accepted by the COUNTY. At the
intersection of 58°i Avenue, the cross-section will be as described above. The Engineer
will submit typical sections for final review and approval, prior to preparation of the
roadway plans.
10. Stormwater Pollution Prevention Plan (SWPPP) :
A SWPPP will be developed in conjunction with this project. The site specific SWPPP is
a requirement of both the Environmental Protection Agency (EPA) National Pollutant
Discharge Elimination System (NPDES) General Permit for Stormwater Discharges from
Construction Activities and the State of Florida Department of Environmental Protection
(DEP) Generic Permit for Stormwater Discharges from Construction Activities. Note.
SJRWMD administers permit for DEP.

E. Drainage Design
Major impacts to the existing drainage system will occur with the development of the four lane
divided section. The increase in impervious area will require a drainage analysis to be performed
and permitting will be required by several public entities. The following will be required to
prepare the drainage plans :
2 . Drainage Improvements:
The existing drainage system within the CR-510 limits will be completely redesigned. All
proposed drainage improvements/ modifications shall be shown on the roadway plans.
G. Signal Plans
Ia. The ENGINEER shall prepare traffic signal plans showing, in detail, the locations and
pay items of all new traffic signal equipment including mast arms, signal heads, pull
boxes, conduit, interconnect, etc. associated with the improvements at the intersection of
CR-510 and 58" Avenue and at the intersection where the Future PUD between US- 1 and
Shore Lane is located. Each mast arm will include a light standard.
H1 . Landscape Plans
1 . General
Landscape and irrigation plan sheets shall be prepared to include the following: Key
Sheet, Tabulation of Quantities sheet(s), Landscape Plan sheet(s), Irrigation Plan and
Detail sheet(s), and Landscape Detail sheet(s). The approximate length is 8,290 feet, and
will include areas in the median and roadside within the corridor right-of-way.
The landscape plans will also include coordinating the design of the electrical system and
service point locations for irrigation pumps. A Service Point Detail shall be included in
the plan set showing the required conduits, conductors, meters, and all necessary
electrical equipment.
2. Sutmnary of Pay Items
Pay items and quantities will be developed and will be incorporated into the overall
project master quantities list and opinion of probable construction cost.

3 . Special Provisions
Technical Special Provisions will be provided for any items of work not standard FDOT
items, and do not conform to FDOT' s Standard Specifications for Road and Bridge
Construction, dated 2004 .
4. Landscape Maintenance Agreement
As an additional service, information required for the Maintenance agreement shall be
provided to the COUNTY . Written or graphic guide for care of the plantings, irrigation
system and maintenance after the warranty period. Maintenance details and specifications
shall recommend : a mowing schedule and maintained grass height; fertilization
schedules, formulas, rates and methods of application; weeding schedule and method
chemical, mechanical, or manual ; edging schedule; herbicide schedules formulas, rates,
methods of application, precautions; pruning schedule and methods to maintain health and
clear sight requirements; mulching materials, thickness and replacement frequency;
irrigation schedule and warranty period maintenance (flushing, adjustments, clean-up);
litter pick-up, and hardseape care. This Maintenance Plan will be developed in
coordination with the local government entity who assumes the maintenance obligation.
1. Construction Phase Assistance
Construction phase assistance in excess of that which is included in the original agreement shall be
submitted to the COUNTY under a Supplemental Work Order, if so requested, once the
construction documents have been finalized and construction duration hours been determined .
"SECTION IV - TIME FOR COMPLETION" is being modified to incorporate the following:
The time for completion of the 90% design drawings and permit applications for the scope of services
detailed in this amendment will be ( 14) months. No format plan submittals for the original scope of work
will be made until the work associated with the plan submittals in this amendment is complete. Once
permits are received, 100% design drawings shall be completed 90 days thereafter.
"SECTION V — COMPENSATION" is being modified to incorporate the following:
The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered pursuant to this
Agreement fees in accordance with the following:

SECTION XVII - ENTIRETY OF AGREEMENT
This writing embodies the entire AGREEMENT and understanding between the parties hereto, and there
are no other agreements and understandings, oral or written, with reference to the subject matter hereof that
are not merged herein and superseded hereby.
No alteration, change, or modification of the terms of this Agreement shall be valid unless made in writing
and signed by both parties hereto.
This AGREEMENT, regardless of where executed, shall be governed by and construed according to the
laws of the State of Florida.
IN WITNESS WHEREOF the parties hereto have executed these presents this 7th day of
