This Amendment Number 3 to Work Order Number 1 ( "Amendment" ) is entered into
as of this 8th day of February 2005 (" Effective Date" ) pursuant to that certain
Continuing Contract Agreement for Professional Services dated entered into as of April 6 ,
2004 ("Agreement" ) , by and between Indian River County , a political subdivision of
the State of Florida ("COUNTY" ) and Brown and Caldwell (" CONSULTANT" ) .
1 . The COUNTY has selected the Consultant to perform the professional services
set forth in Work Order Number 1 dated April 13 , 2004
2 . The COUNTY and the Consultant desire to amend Work Order Number 1 as set
forth in Attachments 1 , 2 and 3 attached to this Amendment and made a part hereof by this
reference , all in accordance with the terms and provisions set forth in the Agreement .
3 . From and after the Effective Date of this Amendment , Work Order Number 1 is
amended as set forth in this Amendment . Pursuant to paragraph 1 . 4 of the Agreement ,
nothing contained in any Work Order shall conflict with the terms of the Agreement and the
terms of the Agreement shall be deemed to be incorporated in each individual Work Order
as if fully set forth herein .
IN WITNESS WHEREOF , the parties hereto have executed this Amendment as of
the Effective Date first written above .

INDIAN RIVER COUNTY UTILITIES DEPARTMENT
OPERATIONS CENTER COMPLEX
SCOPE OF SERVICES
Amendment No . 3 modifies the Scope of Services in Work Order No . 1 for design, permitting,
bidding and construction services of the Utilities Department ' s Operations Complex at the
intersection of 41 " Street and 43rd Avenue to include certain additional work activities associated
with installing approximately 3100 linear feet of off- site wastewater force main from the Operations
Complex site to an existing wastewater force main located on 43rd Avenue at 47t Street .
Installation of this force main will provide a wastewater service connection to the remaining
undeveloped County property along 43rd Avenue as well as additional undeveloped and currently
un- serviced parcels east and west of the 43rd Avenue corridor.
The wastewater force main will be sized in accordance with the Utility Department ' s flow
calculation for the 41St Street and 43rd Avenue corridor. A master lift station will be designed for
installation at the new Operations Complex site . This lift station will serve existing and
undeveloped properties along 41 " Street west of 43rd Avenue and undeveloped properties adjacent
to 43rd Avenue, north of 41St Street. The lift station shall meet normal Indian River County design
standards except that it shall include : variable speed pumps ; flow rate data collection ; biological
odor control ; a self contained, automatic, back-up power supply generator; and additional telemetry
input/output for monitoring/control of the additional equipment .
Amendment No . 3 is being implemented at this time so that the wastewater infrastructure
improvements associated with the Operations Complex site development can be integrated with
wastewater service along the 43rd Avenue corridor. When designed, competitively bid and
constructed along with the Operations Complex, an overall cost savings to Indian River County can
be realized.
A description of the Scope of Services for Amendment No . 3 , broken down into the previously
identified Work Tasks included in the original Work Order No . 1 , is provided below .
TASK 1 — PROJECT MANAGEMENT $ 99000.00
Additional work activities to be completed under Task 1 include the following :
1 . Project Management Plan — Modify Project Management Plan to include the work of
Amendment 3 , identifying the roles of the various members of the BC Team and their
specific scopes of work, a project staffing plan, a communications plan, a QA/QC plan, and
other procedural information relevant to work on the project .
2 . Project Kick-Off Meetings — Conduct project review meetings at regular intervals with
Utilities Department staff. The first meeting will be a project kick-off meeting to review the

objectives of the project and the scope , schedule and budget for completion of work. It is
anticipated that subsequent meetings will be held at approximately monthly intervals
thereafter to review the status of work in progress , to resolve any outstanding issues , and to
provide County staff with the opportunity to have input into the project as it proceeds to
completion.
TASK 2 — SITE INVESTIGATIONS $ 59000.00
Additional work activities to be completed under Task 2 include the following :
1 . Topographic Survey — Survey existing Road and Bridge Lift Station and its surrounding
area. Survey approximately 1500 feet along the west side of the 43rd Avenue right-of-way
lying north of the project parcel to 47th Street. Perform typical cross sections every 300 feet
along the right-of-way and at major breaks and intersections . Identify surface topographic
features between the centerline of pavement to the edge of right-of-way along the entire
length of the survey. Locate and identify any trees in the right-of-way with a diameter at
breast height (dbh) of 4 inches or greater or major vegetative stands not qualifying as
individual trees which might affect the project design.
2 . Topographic Survey Drawing — Prepare a certified "Sketch of Information" of the area to be
surveyed, meeting Florida Minimum Technical Standards and as required to meet Chapter
914 of the Indian River County Land Development Code . An electronic copy of the
topographic survey drawing will be provided to Indian River County in AutoCAD format.
TASK 3 — PRELIMINARY DESIGN PHASE $ 69000.00
Additional work activities to be completed under Task 3 include the following :
1 . Preliminary Meetings — Prepare for and attend up to two "programming" meetings with
Indian River County Utility Department engineering staff or their designated agents to
identify design requirements and constraints .
2 . Design Resort and Conceptual Layout — Perform planning calculations which identify the
areas of service and flows calculated for the project. Determine the appropriate location for
diverting the 41s' Street flows into the proposed collection/transmission system . Design the
diversion and collection system such that the Road and Bridge Lift Station can be taken
"off-line" , if possible . Identify the system component sizes , quantities and anticipated
installed costs .
TASK 4 — SITE PLAN APPROVAL $ 0. 00
Amendment No . 3 does not require any additional work under Task 4 . The master lift station has
been anticipated and permitted in the Operations Complex site plan as a "future" installation.

TASK 5 — CONSTRUCTION PLANS AND SPECIFICATIONS $ 25,000. 00
Additional work activities to be completed under Task 5 include the following :
1 . Gravity Collection Main - Redesign the Operations Complex wastewater collection system
to flow from the Road and Bridge Lift Station to the proposed master lift station . Size the
system for diversion of flow from the 41St Street transmission system into the new collection
system. Provide gravity extensions to the north and east under 43rd Avenue to service areas
which are currently undeveloped. Prepare plan and profile drawings for installation of the
gravity sewer. Prepare technical specifications for placement on the construction plans .
2 . Wastewater Force Main — Prepare plan and profile drawings indicating the proposed
installation and restoration for the wastewater transmission main. Prepare typical and project
specific details for installation of the main. Prepare technical specifications for placement
on the construction plans .
3 . Master Lift Station — Using Indian River Standard Utility Details , design a master
wastewater lift station to service the regional area. Provide design plans and details for the
following additional items : variable speed pumps ; flow rate data collection; biological odor
control ; a self contained, automatic, back-up power supply generator ; and additional
telemetry input/output for monitoring/control of the additional equipment. Prepare technical
specifications for placement on the construction plans .
4 . Design Review and Cost Estimates — Conduct design reviews with Indian River County
Utilities Department staff at approximately the 50, 90 and 100 percent design completion
levels . An Engineer' s Opinion of Probable Cost will be prepared as part of each design
review submittal .
TASK 6 — PERMITTING $ 69000.00
Additional work activities to be completed under Task 6 include the following :
1 . Wastewater System Permitting — After completion of the 90 percent drawings, prepare an
individual FDEP wastewater permit application, submit the application and attachments , and
assist the County in obtaining the permit from FDEP . Prepare an Indian River County right-
of-way application, submit the application and attachments, and assist the County in
obtaining the right-of-way permit . Prepare an Indian River Farms Water Control District
utility crossing application, submit the application and attachments, and assist the County in
obtaining the utility crossing permit.
2 . Permit Fees — Payment of permit fees is included in this Scope of Work. A budget
allocation of $ 1 ,250 has been included for fees associated with the FDEP wastewater permit,
the Indian River Farms Water Control District permit, and the Indian River County right-of-
way permit.

TASK 7 — BIDDING SERVICES $ 49500.00
Additional work activities to be completed under Task 7 include the following :
1 . Provide one set of hard copy reproducible plans and specifications and one electronic copy
on compact disc . Coordinate with Indian River County in preparation of Bid Form as
applicable . Indian River County shall be responsible for preparing contract documents,
including "Front End Bid Documents", as required .
2 . Attend Pre-Bid Meeting.
3 , Answer Bidders questions and draft Addendum(s) for the wastewater improvements as may
be required.
4 . Review bids for accuracy, reasonableness , and proposed sub-contractors . Assist Indian
River County in negotiating a Contract with low Bidder.
TASK 8 — CONSTRUCTION PHASE SERVICES $ 219000.00
Additional work activities to be completed under Task 8 include the following :
1 . Construction Administration - Attend Pre-Construction Meeting . Act as liaison between
Contractor, Jurisdictional Agencies and County for construction issues . Review shop
drawings of the wastewater system structures and materials . Respond to "requests for
information" from the Contractor for construction related issues . Attend monthly progress
meetings and prepare progress report after each meeting.
2 . Project Inspection - Provide part time Project Inspector for duration of time required to meet
permit conditions and provide reporting of substantial construction activities to the
Engineer. Site Inspector time is based upon efficient site work and project infrastructure
installation. Engineer is not responsible for time overruns during construction . This scope
is based upon the provision of 120 hours for an Inspector during the wastewater system
construction period (two hours per day for three months) . Engineer shall perform up to 6 site
inspections to substantiate Contractor ' s application for payments and to attend substantial
and final completion inspections with County staff. These services are in addition to those
required for inspection of the original on-site gravity collection system .
3 . Project Completion and Certification — Using "as-built" survey data provided by the
Contractor and inspection notes, prepare a punch list . Upon completion, revise plans to
indicate "as-built" conditions . Complete and submit certification forms and "as-builts" to
permitting agencies . Complete Indian River County 15 -point operating permit checklist for
sewer and submit with required attachments . These services are in addition to those required
for inspection of the original on- site gravity collection system
