66th Avenue/ County Road 510 Roadway Widening
AMENDMENT NO . 1 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING
SERVICES AGREEMENT BETWEEN KIMLEY-HORN AND ASSOCIATES , INC. AND
INDIAN RIVER COUNTY, FLORIDA.
This is an amendment to the existing Engineering/Land Surveying Services Agreement
(AGREEMENT) dated February 22d, 2005 between Kimley-Horn and Associates , Inc .
(ENGINEER) and Indian River County (COUNTY) . This amendment addresses changes in
" Section I - Project Limits and Description" , " Section III - Scope of Services " , " Section V -
Compensation" and " Section XVI — Duration of Agreement" of the AGREEMENT .
Amendment Description
This Amendment includes the following :
" SECTION I — PROJECT LIMITS AND DESCRIPTION" is being modified to incorporate the
following :
Consultant services are required for the preparation of design surveys , right-of-way maps,
parcel sketches and legal descriptions , subsurface geotechnical investigations , roadway
improvement plans associated roadway improvements along 66th Avenue and County
Road 510 . The COUNTY desires to construct 66th Avenue as a four-lane divided
roadway section between 77th Street (Hobart Road) and Barber Street, and CR 510 to a
four- lane divided section approximately 2 , 000 feet east and west of 66th Avenue .
Signalization and/or intersection modifications will be necessary at the Barber Street, 81St
Street and 77th Street intersections to accommodate the proposed four-lane divided
section. It is understood that COUNTY desires the additional lanage to operate as drop
lanes as to not require a roadway transition north and south of the Barber Street and 77th
Street intersections, respectively. The County also desires that northbound, southbound
and westbound left turn lanes be added to the 81St Street intersections improvements .
It is understood that the Indian River County School Board wishes to expand the
intersection at 81 st Street for Liberty Magnet School which is scheduled for opening
August 2006 . Since additional right-of-way is required for the proposed Liberty Magnet
School, the COUNTY intends to acquire the full right-of-way required for the four- lane
divided section and extend the design of the four-lane divided section to 77th Street .

Based upon the proposed limits and improvements described above the total project
length is 15 , 840 linear feet or 3 . 0 miles .
"SECTION III — SCOPE OF SEVICES " is being modified to incorporate the following :
Control Survey:
Will consist of a Cover Sheet, one Key Sheet, and 6 Detail Sheets depicting existing right
of way lines, monumentation, etc , in compliance with the attached "Exhibit B ", as
provided by Indian River County.
Right of Way Survey:
Will consist of a Cover Sheet, one Key Sheet, and 7 Detail Sheets depicting existing right
of way lines, and proposed right of way lines, etc, in compliance with the attached
"Exhibit B", as provided by Indian River County. The Right of Way Survey will also
include a Tabulation Sheet depicting Parcel Take information.
Detail Sheets will depict existing planimetric features obtained via FDOT Electronic
Field Book Practices .
Key Sheets will be at a scale of 1 "= 300 ' and Detail Sheets will be at a scale of 1 " = 50 ' .
The surveys will be delivered on mylar.
It is assumed the County will provide Title Reports , in a timely manner, and other
available survey control information in the possession of the County. Legal Descriptions
and Sketches will be prepared for approximately 61 parcels at a per unit fee of $ 375 .
Design Topographic Survey:
From 77th Street to 1500 ' South of CR 510, and from Barber Street to 1500 ' North of CR
510 for development of Digital Terrain Model (DTM) .
Drainage survey of structures within the corridor to include pipe size, type, and invert .
The County will be responsible to insure that structures are clear of debris . No sub
surface utilities are included, and no location of utility designating is included.
Opinion of Probable Construction Cost (OPC) will be submitted with 30% , 60%, 90%
and 100% plan submittals .
" SECTION V — COMPENSATION" is being modified to incorporate the following :
The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered
pursuant to this Agreement fees inclusive of expenses in accordance with the following :

The basic compensation mutually agreed upon by the ENGINEER and the
COUNTY is as follows :

" SECTION XVI — DURATION OF AGREEMENT" is being modified to incorporate the
following :
The time for completion of the 90% design drawings and permit applications for the
scope of services detailed in this amendment will be fifteen ( 15 ) months following receipt
of the notice to proceed from the COUNTY . Once permits and 90% design comments
are received from the COUNTY, 100% design drawings will be completed 30 days
thereafter.
The AGREEMENT is hereby amended as specifically set forth here in. All other sections of the
AGREEMENT shall remain in full force and effect and are incorporated herein.

This Amendment No . 1 to the AGREEMENT regardless of where executed, shall be governed
by and construed by the laws of the State of Florida.
