UCP 2958) is entered into as of this 13thday of Nov. , 2007 ("Effective Date") pursuant to that certain Continuing Contract Agreement for Professional Services entered into as of April 6, 2004 and amended effective April 7, 2007 ("Agreement'), by and between Indian River County, a political subdivision of the State of Florida ("COUNTY") and Schulke, Bittle & Stoddard, L.L.C., ("CONSULTANT'). I. The COUNTY has selected the CONSULTANT to perform the professional service set forth in existing Work Order Number 4 effective date April 25, 2007 (project no. UCP 2958). 2. The COUNTY and the CONSULTANT desire to amend the above referenced work order (set forth in Attachment 1 (Scope of Work), Attachment 2 (Fee Schedule), and Attachment 3 (Time Schedule) attached to this Amendment and made part hereof by this reference, all in accordance and provisions set forth in the Agreement. 3. From and after the Effective Date of this Agreement, the above referenced Work Order is amended as set forth in this Amendment. Pursuant to paragraph 1.4 of the Agreement, nothing contained in any Work Order shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed to be incorporated in each individual Work Order as if fully set forth herein. Master Planned 12" Water Main and approximately 1,335 If of 6" P.V.C. and H.D.P.E. Water Main, and associated appurtenances to be located and constructed along the following route: A 12" P.V.C. and H.D.P.E. The estimated construction cost of this project is $2,132,898.86. Descrintion of Amendment Schulke, Bittle & Stoddard, L.L.C. has been directed to prepare an Amendment to the Work Order, with changes generally described as follows: The proposed changes will provide for the construction of water mains for additional fire protection for areas within Vero Lake Estates located in the farthest western area of the existing residential development. Six (6) inch water mains are proposed to be added at 87"' St., from 104'' Ave to 105th Ave; at 83`d St. from 104th Ave. to 105'hAve; and a twelve (12) inch water main is proposed to be added at 79"' St., from 104"' Ave. to 105"' Ave. Attachment I Work Order No. 4 with Schulke, Bittle & Stoddard, L LG Pg. 1 of 1 Attachment I (Scope of Work, cont'd) Changes to the Work Order No. 4 will include: TASK 1 - Additional route survey of 87t1i St., 83rd St. and 79" St. (approximately 1995 If) TASK 2 - Add new water mains to the hydraulic model previously developed, and determine fire flow capacity Prepare construction drawings of the new water main routes on 87d' St., 83rd St., and 79'h St. Prepare and submit permit application to Florida Department of Environmental Protection and Indian River County Engineering Department Prepare specifications and change order documents (add new work to construction contract previously awarded) TASK 3 - Additional construction administration and field inspections In order to execute this project in the most cost effective manner with the most aggressive schedule, Schulke, Bittle & Stoddard, L.L.C. is using the following sub -consultants: Houston, Schulke, Bittle & Stoddard, Inc. for the route survey.
