This Amendment Number 1 to Work Order Number 6 is entered into as of this20th day of
February 200Q7 ("Effective Date") pursuant to that certain Continuing Contract
Agreement for Professional Services entered into as of April 6, 2004 (" Agreement") , by and between
Indian River County, a political subdivision of the State of Florida (" COUNTY") and
Camp Dresser and McKee, Inc. ("CONSULTANT') .
1 . The COUNTY has selected the CONSULTANT to perform the professional services
set forth in existing Work Order Number 6, Effective Date April 18, 2006.
2 . The COUNTY and the CONSULTANT desire to amend the above referenced work
order as set forth in Attachment A (Scope of Work) , Attachment A (Fee Schedule) , and
Attachment A (Time Schedule) attached to this Amendment and made a part hereof by this
reference, all in accordance with the terms and provisions set forth in the Agreement.
3 . From and after the Effective Date of this Amendment, the above referenced work
order is amended as set forth in this amendment. Pursuant to paragraph 1 . 4 of the agreement, nothing
contained in any Work Order shall conflict with the terms of the Agreement and the terms of the
.Agreement shall be deemed to be incorporated in each individual Work Order as if fully set forth
herein.

SCOPE OF WORK
INDIAN RIVER COUNTY
NORTH COUNTY WATER TREATMENT PLANT EXPANSION
ARTESIAN WELL SAMPLING PROGRAM
WORK ORDER NO. 6 - AMENDMENT NO. 1
L Project Description
In response to concerns raised by the Indian River County (County) Soil and Water
Conservation District (SWCD) surrounding the North County Water Treatment (WTP)
Plant Expansion project, the Board of County Commissioners (BOCC) authorized CDM
to perform water quality sampling of offsite artesian wells neighboring the WTP. Work
Order No. 6 was executed by the BOCC on April 18, 2006.
Upon receiving the Notice to Proceed from the County, CDM and County staff met with
members of the SWCD to determine the artesian wells most representative of the area's
hydraulic conditions and those with the most potential to be affected by increased
pumping from the expanded WTP. CDM's original work order was to sample
approximately 14 neighboring wells, the County's 3 production wells, and the 2 deep
monitoring wells at the WTP on a monthly basis for artesian pressure and a series of
field parameters.
A field visit to each of the wells identified by the SWCD revealed that the majority of the
wells were not configured to allow for measurement of artesian pressure or for
collecting a water sample. The only well in the program that was equipped with a
pressure gauge was located at Sandridge Golf Course. This gauge was replaced with a
new one similar to the ones CDM installed on the remaining wells to ensure consistency
and similar calibration. CDM also installed sampling taps on all but one well. These
additional efforts and costs were not anticipated and as such were not included in the
budget of original work order.
Furthermore, the SWCD requested that the County consider increasing the frequency of
sampling to weekly to better represent the seasonal fluctuations (primarily during the
wet season) . The County verbally directed CDM to accommodate the request and begin
sampling on a weekly basis with the next event. CDM has been collecting weekly
samples since May 26, 2006.
ff. Scope of Services
The sampling frequency of once a month set forth in the original Work Order shall be
amended to a weekly frequency under this amendment. CDM will also be reimbursed
for additional services to reconfigure the wellheads of the project wells to include
pressure gauges, sample taps, and valves; where needed. Written reporting frequency
shall remain monthly.

III. Consulting Engineer Insurance Requirements
A. Workers Compensation as required by the State of Florida. Employer's Liability of
$100,000 each accident, $500,000 disease policy limit, and $100,000 disease each
employee.
B. General Liability: commercial general liability coverage, including contractual
liability and independent contractor, with a minimum combined single limit of
$100,000 per occurrence and personal injury coverage of $50,000.
C. Business Automobile Liability : owned, hired, and non-owned vehicles at a
minimum combined single limit of $100,000 per occurrence and personal injury
coverage of $50,000 for bodily injury and property damage for owned and non-
owned vehicles.
D. Professional Liability Insurance: providing coverage for negligent acts, errors, or
omissions committed by Consultant with a limit of $1,000,000 per claim/ annual
aggregate. This insurance shall extend coverage to loss of interest, earning, profit,
use, and business interruption, cost of replacement power, and other special,
indirect, and consequential damages.
IV. Compensation for Services
The additional expense associated with reconfiguring the wellheads to allow for
sampling and the increased frequency have added to the previously anticipated project
cost. The original work order consisted of 20 hours of labor per month for sampling and
reporting for a total of 480 hours. After beginning the sampling process, it was
determined that all wells could be sampled in a 4-hour period, rather than a full 8-hour
day. Increasing the number of sampling events from 24 to 104 is expected to increase the
number of hours required from 480 to 896. A summary of the hours/budget in the
original proposal, as well as the amended amounts to account for the modified scope of
services is provided in Table 1. The additional cost incurred from reconfiguring the
wellheads was approximately $750 for time and materials. The budgeted other direct
costs did not include provisions for purchasing pressure gauges, sampling taps, fittings,
etc. The total requested increase in the upper limit under this amendment is $46,510.
Compensation is to be paid on a billing rate basis plus other direct costs on an as-needed
basis. CDM will submit monthly invoices for services performed.
