Home My WebLink About2005-196 OC� o `1os� INDIAN RIVER COUNTY 5`h STREET SW BRIDGE OVER LATERAL " C " CANAL 43 " AVENUE BRIDGE OVER SOUTH RELIEF CANAL AND OLD DIXIE HIGHWAY BRIDGE OVER SOUTH RELIEF CANAL AMENDMENT NO, 2 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING SERVICES AGREEMENT BETWEEN BRIDGE DESIGN ASSOCIATES , INC. AND INDIAN RIVER COUNTY, FLORIDA. This is an amendment to the existing Engineering/Land Surveying Services Agreement (AGREEMENT) dated March 18 , 2003 between Bridge Design Associates, Inc . (ENGINEER) and Indian River County (COUNTY) . This amendment addresses changes in "Section I" — "Project Limits and Description", " Section III - Scope of Services " and " Section V - Compensation" . Amendment Description This Amendment includes the following : The ENGINEER will perform the necessary engineering services to combine the glans that have been prepared for the intersection improvements at the intersection of 43 `d Avenue and 5 Street SW with the plans for the replacement of the 43 `d Avenue Bridge over the South Relief Canal, and to widen 43 `d Avenue to a three lane section from north of Oslo Road to Vineyard Lane SW. "SECTION I — PROJECT LIMITS AND DESCRIPTION The south limit of the project is located approximately 757 feet north of the intersection of Oslo Road and 43 `d Avenue where the existing 3 - lane section starts to transition north to a 2-lane section. The north limit of the project is at Vineyard Lane SW where the existing 3 -lane section transitions south to a 2-lane section . The total length of the project on 43 `d Avenue is approximately . 82 miles . The east-west project limits on 5t" St SW are outlined in the original AGREEMENT. " SECTION III — SCOPE OF SERVICES " is being modified to incorporate the following : The plans for the improvement of the intersection of 43 `d Avenue and 5th SW and for the replacement of the 43 `d Avenue Bridge over the South Relief Canal will be combined into one set of construction plans suitable for bidding and construction. Additional plan sheets will be prepared covering the new project limits and detailing the widening and reconstruction of 43 `d Avenue . A Stormwater Pollution Prevention Plan (SWPPP) will be developed in conjunction with this project. The site specific SWPPP is a requirement of both the Environmental Protection Agency (EPA) National Pollutant Discharge Elimination System (NPDES) General Permit for Stormwater Discharges from Construction Activities and the State of Florida Department of Environmental Protection (DEP) Generic Permit for Stormwater Discharges from Construction Activities. A traffic signal will be designed to accommodate the intersection improvements to the 5th Street SW/43rd Avenue intersection. A mast-arm signal configuration will be designed to meet current COUNTY and FDOT standards . A :\03 -537-38 -39 Additional Services . doc The construction plans shall include the following plan sheets : • Cover Sheet • Typical Sections • Quantities and General Notes • Horizontal and Vertical Control • Details • Plan and Profile Sheets • Cross Sections • Signing and Marking Sheets • Utility Plans • Construction Phasing • Signalization Plan at 43 `d Avenue and 5th St. SW • Bridge Plans • Intersection Plateau Plan & Details • Stormwater Pollution Prevention Plan Permit applications will be prepared and submitted to the St. Johns River Water Management District, Indian River Farms Water Control District, Corps of Engineers, Department of Environmental Regulation and any other applicable permits as outlined in the original AGREEMENT . "SECTION IV — COMPENSATION " is being modified to incorporate the following : The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered pursuant to this Agreement fees inclusive of expenses in accordance with the following : Field Survey $ 55500 . 00 Construction Plans and Specifications $219423 . 00 43 `d Avenue Bridge Modifications $ 189000 . 00 Stormwater Pollution Prevention Plan $ 5 ,240. 00 Permitting Roadway & Bridge $ 2 , 500 . 00 Signalization $ 12 , 800 . 00 Signalization Permitting and Utility Coordination $ 1 ,200 . 00 Mast Arm Foundations $ 6 , 000 . 00 Meetings and Coordination $ 29500. 00 Soil Investigations (mast arms & 6 roadway borings) $ 91000 . 00 TOTAL $ 84 , 163 . 00 There are no sketches and legal descriptions, raster images, or as-built plans included in the above fee . The AGREEMENT is hereby amended as specifically set forth here in . All other sections of the AGREEMENT shall remain in full force and effect and are incorporated herein .
