BRIDGE REPLACEMENTS AND WIDENING OF 66"' AVENUE
FROM SOUTH OF SR 60 TO 59TH STREET
PROFESSIONAL CIVIL ENGINEERING, GEOTECHNICAL ENGINEERING
AND LAND SURVEYING SERVICES
AMENDMENT NO, 4 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING
SERVICES AGREEMENT BETWEEN ARCADIS AND INDIAN RIVER COUNTY,
FLORIDA,
This is an amendment to the existing Engineering/Land Surveying Services Agreement
(AGREEMENT) dated September 6, 2005 between ARCADIS (ENGINEER) and Indian River
County (COUNTY). This amendment addresses changes in "Section I — Project Limits and
Description", "Section III - Scope of Services", and "Section V - Compensation".
Amendment Description
This Amendment includes the following:
"SECTION I - PROJECT LIMITS AND DESCRIPTION" is being modified to incorporate
the following:
Consultant services are required for the preparation of construction plans and permit
applications for design of improvements to 66`h Avenue from South of SR 60 to 33`d Street. The
scope of services included in this amendment consist of:
Traffic Signal Plans
Traffic signal plans will be prepared for the intersection of SR 60 and 66`h Avenue. The
traffic signal plans will detail the construction of a mast arm traffic signal at the intersection. The
mast arms will be located at the ultimate intersection configuration.
Construction Plans — South of SR 60 to 33 Id Street
Indian River Con desires to expedite the construction of the section of 66`h Avenue
from south of SR 60 to 33 Street. ARCADIS will prepare a separate set of stand alone plans for
construction of the section of 66`h Avenue from south of SR 60 to south of 33`d Street. The plans
will include a transition for the proposed 4 -lane divided roadway section to match the existing 2 -
lane roadway section south of 33`d Street.
"SECTION III — SCOPE OF SERVICES" is being modified to incorporate the following:

"SECTION IV - TIME FOR COMPLETION" is being modified to incorporate the following
The time for completion of the 90% design drawings, permit applications, and Engineer's
Opinion of Probable Construction Cost for the project shall be three (3) months from the date of
execution of ADDENDUM No. 4. Once permits are received, 100% design drawings shall be
completed 30 days thereafter.
"SECTION V — COMPENSATION" is being modified to incorporate the following:
The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered
pursuant to this Agreement fees in accordance with the following:

The AGREEMENT is hereby amended as specifically set forth here in. All other sections of the
AGREEMENT shall remain in full force and effect and are incorporated herein.
This Amendment No. 4 to the AGREEMENT regardless of where executed, shall be governed by
and constructed by the laws of the State of Florida.
