WORK ORDER NO. 4 TO THE PROFESSIONAL CIVIL ENGINEERING SERVICES
AGREEMENT BETWEEN ARCADIS U . S . INC . AND INDIAN RIVER COUNTY, FLORIDA.
This Work Order No . 4, entered into this 3RD day of Mflrch , 2009 , by and between
INDIAN RIVER COUNTY , a political subdivision of the State of Florida, hereinafter referred to as the COUNTY ,
and
ARCADIS U . S . , INC . , Vero Beach , Florida, hereinafter referred to as the ENGINEER, modifies the original Agreement as
follows :
" SECTION III — SCOPE OF SERVICES "
Consultant services are required for Environmental Assessment and Regulatory Permitting in
support of the preparation of road design plans and permit applications for the construction of the
widening of 43rd Avenue from 25 `" Street S . W . to Oslo Road (9`" Street S . W . ) .
B . Scope of Services :
The preparation of environmental assessment and regulatory permitting documents in response to the US Army
Corps of Engineers request for additional information. The environmental assessment and regulatory
permitting documents will consist of documentation relative to wetland jurisdictional services and endangered
and threatened/species of special concern including:
• Field determination of approximate wetland lines
Approximate wetland boundary depiction
Preparation of a wetland jurisdictional summary memorandum
• Endangered & threatened /species of special concern surveys
• Agency coordination
If required by the USACE, a permitting submittal package will be prepared. The submittal package will consist
of.•
• UMAM
• Mitigation alternatives analysis
• Environmental Resource Permit (ERP) Application
• Coordination with Army Corps of Engineers
If required by the USA CE, a mitigation plan will be prepared.
The flagging and surveying of wetland boundaries in the field, if required by the regulatory agencies, is not
included in this work authorization. This work will be included in a subsequent work authorization if requested
by the County.
" SECTION IV - TIME FOR COMPLETION" is being modified to incorporate the following :
The time for completion of the Environmental Assessment and Regulatory Permitting for the
project shall be two (2) months from the date of execution of this Work Order No . 4 .

" SECTION V — COMPENSATION" is being modified to incorporate the following :
The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered
pursuant to this Agreement fees in accordance with the following :

the State of Florida.
