Architectural Services Proposal
Owners Representative
Indian River County Department of Emergency Services
Design/Build Fire Station Projects
Background :
Indian River County Department of Emergency Services is seeking services of a Design/Build
Team to provide professional services for the site planning, design and construction of new
Emergency Services Stations, the renovation of existing stations and the removal demolition of
Hurricane damaged stations . Indian River County envisions a phased approach to the overall
program for renovation and construction .
The County has retained Donadio & Associates Architects , P . A to perform the services of
Owners Representative During the Design, Bidding and Construction Phases of the renovation
and construction .
SCOPE OF SERVICES
Design Phase .
During the Design phase of each fire station project the Owners Representative will provide
services for the following :
1 . Review of the Design/Build Agreement.
2 . Attend Design Criteria meetings .
3 . Review of the Design Criteria.
4 . Review of drawings, specifications and cost estimates at the 30% , 50% and 0%
completion stages of the construction documents .
5 . Monthly review of the design schedule .
6 . Review of monthly Design/Build Team invoices .
7 . Attend Design/Build Team meetings .
8 . Provide written monthly reports to the Emergency Services Director.
9 . Attend pre-bid conference .
10 . Bid evaluation of proposals .
11 . Review Design \Build Guaranteed Maximum Price Agreement (GMP) .
12 . Meetings with Owner on an as needed basis .
13 . Attend County Commission meetings on an as needed basis .

Construction Phase :
During the Construction phase of each fire station project the Owners Representati a will
provide services for the following :
1 . Attend pre-construction meeting .
2 . Attend Design/Build Team meetings .
3 . Perform regular on-site visits to observe the progress of the project.
4 . Review monthly application for payment.
5 . Regular review of submittal, RFI and change order logs .
6 . Review of proposal request and change orders .
7 . Monthly review of construction schedule .
8 . Provide written monthly reports to the Emergency Services Director.
9 . Meetings with Owner on an as needed basis .
10 . Attend County Commission meetings on an as needed basis .
11 . Review of punch list for substantial and final completion .
12 . Review project closeout documents and warranties .
13 . Review and recommendation for final payment.
Compensation :
The Owners Representative will be compensated for services performed under this Work Order
Number 1 , for Phase I Projects in accordance the following fee schedule :
Design Phase Services : * *

* The above Design and Construction Phase Services fees are a per building fee for new and
renovated facilities for Phase I work only. Progress payments will be invoiced on a
monthly basis for work performed.
* * Design and Construction Phase Services fees are a maximum not to exceed fee billed
on an hourly basis as per the hourly rate schedule in the Continuing Contract
Agreement for Professional Services dated October 19 , 2004 .

Work Order Number 1
This work order number 1 is entered into as of this 29th day of , 2005
pursuant to that certain Continuing Contract Agreement for Professional services entered
into as of October 19 , 2004 (AGREEMENT) , by and between Indian River County, a
political subdivision of the State of Florida (COUNTY) and Donadio & Associates
Architects , P . A . (CONSULTANT) .
The COUNTY has selected the CONSULTANT to perform the professional services set
forth in Attachment A , attached to this Work Order and made part of hereo by this
reference . The CONSULTANT will perform the professional services fort e fee set forth
in Attachment A , attached and made part hereof by this reference . The CONSULTANT
will perform the professional services within the timeframe more particularly set forth in
Attachment A, attached to this work order and made a part hereof by this reference all in
accordance with the terms and provisions set forth in the AGREEMENT . Pursuant to
paragraph 1 . 4 of the AGREEMENT, nothing contained in any Work Order shall conflict
with the terms of the AGREEMENT and the terms of the AGREEMENT shall be deemed
to be incorporated in each individual Work Order as if fully set forth herein
IN WITNESS WHEREOF , the parties hereto have executed this Work Order as of the
date first written above .
