ARDAMAN & ASSOCIATES INC .
All services performed under this contract shall be supervised and certified by a licensed
Professional Engineer ( PE ) registered with the State of Florida and staff employed directly with
your firm .
Scope of Services
All work shall be performed on a unit price basis in accordance with the attached Scope
of
Services dated July 18 , 2008 and shall comply with Standards enumerated in the RENEWAL
OF THE ANNUAL GEOTECHNICAL SERVICES CONTRACT IRC PROJECT NO , 0534 WITH
ARDAMAN & ASSOCIATES INC . Effective Date : September 11 , 2007 .
Additional testing services for 12th Street Sidewalk from 43rd Avenue to 27th Avenue .
All work shall also comply with the current Florida Minimum Technical Standards ,
COMPENSATION ,
The COUNTY agrees to pay , and the Geotechnical Professional agrees to accept , a fee for
services rendered according to " EXHIBIT A " Ardaman & Associates , Inc . , dated June 16 , 2009 ,
A Not4o - Exceed Amount of: $ 827 . 00 to be paid for actual work performed in accordance with
the schedule of services and fees set forth in " EXHIBIT A" per the RENEWAL OF THE ANNUAL
GEOTECHNICAL SERVICES CONTRACT IRC PROJECT NO , 0534 WITH ARDAMAN &
ASSOCIATES , INC . Effective Date : September 11 , 2007 ,

Construction Materials Testing Services
12th Street Sidewalk Project from 43rd Avenue to 27th Avenue
Vero Beach , Indian River County , Florida

Mr . Crain :
As you are aware , Ardaman has been providing construction materials testing services forthe above
referenced project. Prior to the start of the project, we provided a cost estimate to perform testing
services based on estimates of the number of tests and man - hours that were determined using the
project information that was available at the time . Upon completion of a project, the final cost for our
services is a function of the number of tests and man -hours that were actually required during
construction , including additional costs beyond what was originally estimated . Usually, we try to be
slightly conservative with our estimate to prevent change orders and attempt to account for
unforeseen changes in scope , additional items or delays that may occur during the course of
construction , which inevitably occur given the nature of construction .
Various unforeseen , relatively minor items required additional testing during the course of
construction . It is our understanding that items (two road crossings , several in - ground stormwater
collection structures and piping ) were added to this project and subsequently required testing
services beyond what was included in our original estimate . Testing services associated with these
additional items have resulted in our original estimate to be slightly exceeded .
The final cost of our construction testing services forthe project is $ 6 , 527 . 00 , which is $ 827 . 00 more
than our original estimate of $ 5 , 700 . 00 . Therefore , we are requesting a change order for the
amount of $ 827 . 00 for the balance of the cost of our services for the project .
