ADDENDUM TO SUPPLEMENTARY CONDITIONS
AND GENERAL CONDITIONS TO THE
NORTH COUNTY REGIONAL PARK PHASE 2
THIS ADDENDA amends and supplements the Supplementary Conditions
and General Conditions of Construction Contract #0399 , and other provisions of
the contract documents as indicated below . All provisions which are not so
amended as supplemented , remain in full force and affect :
o On page 9 of the General conditions , 1 . 02 D 1 , 2 , and 3 have been
deleted and replaced with new paragraphs . Please see the
Supplementary Conditions .
o Page 16 , General Conditions 5 . 01 C . The references to " paragraph
5 . 01 B " shall be amended to reference Paragraph SC-5 . 01 B .
o On page 17 of the General Conditions 5 . 04 B 4 shall be amended to
include reference to S . C . 6 . 20 .
o On page 18 of the General Conditions 5 . 06 D shall be amended to
include reference to S . C . 5 . 06 .
o On page 19 of the General Conditions 5 . 10 A shall be amended to
include reference to S . C . 5 . 06 .
o On page 22 of the General Conditions 6 . 05 C shall be amended to
include reference to S . C . 6 . 05 A .
o On page 23 of the General Conditions 6 . 08 A The first sentence shall
be deleted .
o On page 27 of the General Conditions 6 . 20 B shall be deleted entirely .
o On page 27 of the General Conditions 6 . 20 C shall be amended to
reference S . 0-6 . 20 A
o On page 28 of the General Conditions 8 . 04 shall be amended to
include reference to S . C . 14 . 02 C 1 and S . C . 14 . 07 C 1 .
o On page 29 of the General Conditions 8 . 05 A shall be amended to
include reference to S . C . 4 . 02 .
o On page 29 of the General Conditions 8 . 08 A shall be amended to
include reference to S . C . 13 . 03 B .

o On page 30 of the General Conditions 9 . 07 . A shall be amended to
include reference to S . C . 6 . 17 G .
o General Conditions , Page 32 , 10 . 05 . 13 . 2 . The reference to "Article 16 "
should be changed to SC - 16A .
o On page 34 , of the General Conditions 11 . 01 . 0 shall be amended to
reference GC Paragraph 12 . 01 .
o On page 40 of the General Conditions 14 . 02 . 13 . 5 . d . shall be amended
to include reference to S . C . 15 . 02 .
o On page 41 of the General Conditions 14 . 02 . D . 1 . D shall be amended
to include reference to S . C . 14 . 02 . B . 5 . e . and f. and also include
reference to S . C . 15 . 02 A .
o On page 41 of the General Conditions 14 . 02 . D . 3 . shall be amended to
reference to S . C . 14 . 02 . C . 1 . not GC 14 . 02 . C . 11
o On page 43 of the General Conditions 14 . 07 . 13 . shall be amended to
reference to S . C . 14 . 09 A .
o On page 43 of the General Conditions 14 . 08 . A . shall be amended to
include reference to S . C . 5 . 01 .
o On page 3 of the Supplemental Conditions SC- 5 . 04 . C . 5 . d . is amended
to include reference to S . C . 6 . 20 .
o On page 9 of the Supplementary Conditions , SC- 13 . 03 . B . the caption
should read , " Delete paragraph G . C . - 13 . 03 . B . in its entirety and insert
the following sentences in its place . "
If needed , please add the signature page .
IN WITNESS WHEREOF , OWNER and CONTRACTOR have signed this
Addendum Agreement in duplicate . One counterpart each has been delivered to
OWNER and CONTRACTOR . All portions of the Contract Documents have
been signed or identified by OWNER and CONTRACTOR or on their behalf.
This Agreement will be effective on January 13 , 200 4 (the date the
Contract is approved by the Indian River County Board of County
Commissioners , which is the Effective Date of the Agreement) .
