AMENDMENT NO . 1 TO WORK ORDER NO. 5 TO THE PROFESSIONAL BRIDGE
DESIGN SERVICES AGREEMENT BETWEEN BRIDGE DESIGN ASSOCIATES , INC.
AND INDIAN RIVER COUNTY, FLORIDA.
This is an amendment to Work Order No . 5 to the existing Bridge Design Services Agreement
(AGREEMENT) for Annual Bridge Design Services , dated October 6 , 2006 between Bridge
Design Associates , Inc . (ENGINEER) and Indian River County (COUNTY ) .
Amendment Description
This Amendment includes the following :
The ENGINEER performed the necessary engineering services due to site conditions ; GRL
Engineers , Inc . ( a sub - consultant) made two site visits to perform their work for dynamic pile
testing . As such their expenses exceeded the amount originally anticipated .
" SECTION I — PROJECT LIMITS AND DESCRIPTION
53 `d Street Bridge over the Indian River Farms Water Control District Lateral "H"Canal .
" SECTION II — SCOPE OF SERVICES " is being modified to incorporate additional dynamic
pile testing .
" SECTION IV — COMPENSATION " is being modified to incorporate the following :
The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered
pursuant to this Agreement fees inclusive of expenses in accordance with the following :
Additional dynamic pile testing $ 1 , 500 . 00
TOTAL Amendment No. 1 $ 19500.00
The AGREEMENT is hereby amended as specifically set forth here in . All other sections of the
AGREEMENT shall remain in full force and effect and are incorporated herein .
This Amendment No . 1 to the AGREEMENT regardless of where executed , shall be governed
by and constructed by the laws of the State of Florida .

Dear Mike :
In our original proposal to you for construction phase services at the above referenced
project we allotted $4 , 000 . 00 for dynamic pile testing .
Due to site conditions , GRL Engineers , Inc. made two visits to the project site to monitor
an additional test pile . As such , their expenses exceeded the amount originally anticipated .
We had allotted $4 , 000 . 00 and their invoices totaled $5 , 500 . 00 (copies attached ) .
Therefore , we request an additional $ 1 , 500 . 00 be added to our contract .
