L Project Description
CDM to provide engineering services to specify well rehabilitation activities necessary at
the South County Water Treatment Plant. Permitting, bidding, and construction
services also included.
II. Scope of Services
Engineering services to assist County staff with preparation of plans and specifications,
permitting, bidding, and construction oversight of well rehabilitation at the South
County Water Treatment Plant (See Attachment B) .
III, Consulting Engineer Insurance Requirements
A . Workers Compensation Insurance in accordance with Florida Statutes ,
B . Comprehensive and Automotive Liability with a minimum coverage of $ 1,000,000
per occurrence for bodily injury or accidental death.
C . Comprehensive General Liability with a minimum limit of $ 1 , 000,000 covering
bodily injury and property damage .
D . Liability for Property Damage, while operating motor vehicle, with minimum
limits of $ 1 ,000, 000 per occurrence .
E . Contractual Liability including limits established for Items IIIB, C, and D above .
IV. Compensation for Services
Well Rehabilitation Services $ 117,075 Upper Limit
Compensation is to be paid on a salary cost basis plus other direct costs on an as-needed
basis . (See Attachment Q . CDM will submit monthly invoices for services performed .

A . Salary Cost Basis with Upper Limit
For the Basic Services performed on a salary cost basis with an upper limit, IRC
agrees to pay CDM as follows :
For engineering services performed by CDM, the payment will be equal to the
salary cost of such services for each employee times 2 .35 for overhead and profit
plus actual out-of-pocket expense costs . Salary cost is equal to 1 . 393 times direct
salary costs and is defined as the cost of salaries (including sick leave, vacation,
and holiday pay applicable thereto) for time directly chargeable to the project; plus
unemployment, excise, and payroll taxes; and contributions for social security,
employment compensation insurance, retirement benefits, and medical and other
group insurance benefits.
Actual out-of-pocket expense costs are all costs other than salary costs that are
incurred during the progress of the work. The actual out-of-pocket expense costs
include : air fare, automobile rental if required, mileage charges, parking tolls, taxi,
meals and lodging, all in accordance with reimbursement rates set forth in Section
112 . 061 Florida Statutes ( 1999), as amended, and telephone, printing and
reproduction costs, and other miscellaneous costs incurred specifically for this
project. The maximum cost for blueprint sheets is $ 1 . 10 . The maximum cost for
mylar sheet is $ 11 . 00 and a sepia sheet is $5 . 00 . The charges for in-house computer
program and word processor usage will be at CDM 's regular rates . For outside
computer services, charges will be made at invoiced cost to CDM .
For the basic services performed on a salary cost basis with an upper limit, IRC agrees to
provide a written request before work is started by CDM.

SCOPE OF WORK
SOUTH COUNTY WATER TREATMENT PLANT
WELL REHABILITATION SERVICES
Backg1ound
Indian River County Utility Services contracted with Camp Dresser & McKee Inc. (CDM) in
May 2000 to investigate accelerated cartridge filter clogging at the South County Water
Treatment Plant. This was the first of four work authorization to assist the County in assessing
the condition of the wellfield and improving its reliability. Records indicated that major
components of these wells had not undergone major service since they were initially installed
up to twenty years ago . Previous work authorizations investigated field conditions and
included removal and inspection of the pumps, specific capacity testing, video inspection of
wells, pump and valve rebuilding, restoration of wells to service, acidization, redevelopment,
well borehole reaming, disinfection, well column extensions, and flow meter acquisition. In
some instances, plans for installation of new equipment or rebuild of existing equipment were
modified as a result of pump failures . CDM and subcontractors worked with County staff to
ensure that the plant's level of service was maintained especially during drought conditions .
CDM prepared and submitted to County Utilities staff a summary of well rehabilitation work
performed to date in August 2002 along with monthly and five-year maintenance
recommendations . Additional recommendations were identified in order to enhance operations
and to account for declining regional water levels .
Services
CDM has prepared this work authorization for well rehabilitation services to assist County staff
in efforts to complete the upgrading of various wellfield appurtenances and equipment.
Task 1 - Rehabilitation of Existing Wells
Well column extension to 100 feet is necessary to account for the declining regional water table
elevations and to reduce the risk of air intrusion. The following is required at each well :

Subtask 1 . 1 — Preparation of Plans and Technical Specifications
CDM will prepare plans and specifications to include the above items to complete the well
rehabilitation efforts for existing wells at the facility. CDM will provide digital drawing files
and technical specifications to the County for bidding purposes . Specifications for telemetry
system / PLC interface and pump rebuilding are to be included . IRC Utilities will prepare the
necessary Contract Forms (Division 0 and 1) for the project.
Subtask 1 . 2 — Bidding Services
CDM will assist the County in obtaining bids for the well rehabilitation, assist the County in
evaluating the bids and in assembling and awarding the contract, and will arrange and preside
over a preconstruction conference . Detailed services include the following:

■ CDM will provide digital files of plans and technical specifications to the County. County
will advertise for bids, and provide the documents to prospective bidders .
■ CDM will coordinate the advertisement of the bid, respond to communications from
Contractors, and issue addenda, as necessary.
■ CDM will prepare a detailed final opinion of probable cost.
■ CDM will attend the bid opening, evaluate bids, and recommend a bidder for Award of
Contract to the County.
■ After issuance of a Notice of Award by the County, CDM will assist the County in
preparing the construction contract, request and review information from the Contactor,
and advise the County on finalizing the Contact.
■ CDM will arrange for a preconstruction conference to be attended by the Contractor, the
County, other interested parties, and CDM's representatives . Preconstruction conference
minutes will be prepared and distributed to all attendees . The Notice to Proceed will be
prepared by CDM for issuance to the Contractor .
Subtask 1 . 3 - Services During Construction
These services shall include the following parts :
Part A
CDM will consult with and advise the County and act as its representative as provided in the
General Conditions of the construction contract. Consult with and advise the County as to the
acceptability of substitute materials and equipment proposed by the Contractor when
substitution is permitted by the Contract Documents .
Part B
CDM will make visits to the site at intervals appropriate to the various stages of construction to
observe the progress and quality of the executed work of the Contractor and to determine in
general if such work is proceeding in accordance with the Contract Documents . During such
visits and on the basis of on-site observations; CDM shall keep the County informed of the
progress of the work, shall endeavor to guard the County against defects and deficiencies in
such work and may disapprove or reject work as failing to conform to the Contract Documents ,
Part C
CDM will review and approve Shop Drawings and samples, the results of tests and inspections
and other data that the Contractor is required to submit.

Part D
CDM will issue County instructions to the Contractor and issue necessary interpretations and
clarifications of the Contract Documents . Have authority as the County's representative, to
require special inspection or testing of the work. Act as initial interpreter of the requirements of
the Contract Documents regarding the execution and progress of the work.
Part E
Based on CDM's on-site observations and on review of applications for payment and the
accompanying data and schedules, CDM will determine the amounts owing to the Contractor
and recommend in writing payments to the Contractor in such amounts .
Part F
CDM will conduct a final check to determine if the work has been completed in accordance with
the Contract Documents .
Part G
CDM will undertake project quality management to meet budget and time constraints; generate
correspondence to the County, Contractor, regulatory agencies, and other parties; provide
monthly project status reports; prepare monthly progress meeting minutes; and maintain
comprehensive project records and documentation on file .
Part H
CDM will provide services in connection with preparing change orders to reflect changes to the
construction project.
Task 2 - Replacement of Well No 3
The County has recently experienced problems with production Well No . 3. County staff
authorized a video log of the well that indicated a casing break. Repair of the casing is possible;
however, due to the break location and age of the well, full replacement is recommended .
Letter modification of the SJRWMD water use permit will be required to authorize installation
of the replacement well and abandonment of the existing well . The new well would be tied into
existing transmission piping near the existing well. Preliminary discussions with SJRWMD staff
indicate that obtaining the required letter modification of the permit is feasible .
Subtask 2. 1 - Design and Permitting
CDM will provide configuration of the new production well based on the existing well
construction details, input from IRC staff, and SJRWMD requirements . Casing sizes, depths,

and materials will be selected . The wellhead design completed under the existing contract will
be incorporated into the construction specifications . The well will connect to the raw water
transmission main that conveys water to the plant. Specifications for materials, equipment, and
drilling techniques will be prepared .
CDM will prepare contract drawings for construction of the new well and appurtenances .
Proper siting of the well and associated appurtenances will be critical to the success of the
project. The connecting piping will be PVC or other suitable material acceptable to County staff
and will be provided with isolation valves as required . CDM will obtain the services of a
professional land surveyor to provide horizontal control of the proposed well and associated
appurtenances, if necessary.
The production well design is anticipated to be similar to the existing wells; however, the casing
size may be changed to facilitate pump equipment installation and removal . The casing size
will be coordinated with SJRWMD staff as required . The control system concept and
supervisory control and data acquisition system for the existing well will be used for the new
well . The electric utility service for the existing well will be used for the new well . The
telemetry system obtained in Task 1 will be installed as well as new pressure sustaining valves
and flow meter (purchased through existing contract) .
In the event that SJRWMD and IRC staff negotiate that the new well is to be located in a
different location, additional services may be required and are not included in this work
authorization.
Contract documents and specifications will be prepared for submittal at the completion level .
One design review workshop / meeting will be held with County staff.
Part A — Design Drawings
Detailed design will consist of preparation of the well and piping drawings . Standard details
will also be prepared . Construction drawings will be prepared using AutoCAD format.
Part B — Contract and Technical Specifications
The technical specifications will detail the materials and methods of construction to be utilized .
Consultant's standard master construction specifications will be used as the basis for the
technical specifications for the project. Preferred manufacturers and suppliers will be listed in
the specifications . IRC Utilities will prepare the necessary contract forms (Division 0 and 1 ) for
the project. Specifications for well abandonment will also be prepared.
Part C - Design Progress Submittals
Construction drawings and specifications will be produced and submitted for County final
review at the completion level . The drawings will be provided on bound 11 x 17-inch sheets .
The final design submittal will be defined as a set of drawings and specifications that is
complete and biddable, but subject to final comment from FDEP and the County.

Part D - Final Plan Preparation
The final design, 100 percent complete documents, will incorporate comments from the County
and FDEP. The final design documents will consist of the County front-end documents,
technical specifications, and engineering drawings . The final design documents will
incorporate designs for the production well and related wellfield facilities .
Part E - Permitting and Agency Coordination
This task will establish the permitting requirements for the proposed production well
replacement project and where appropriate, initiate the permit application process .
Construction permit applications or modification applications will be prepared and submitted
to the following agencies on behalf of the County:
■ Indian River County Building Department (Assist contractor with obtaining required
permits)
■ Florida Department of Environmental Protection
• St. Johns River Water Management District
It is assumed that the County will pay all permit application fees . Up to two responses for
additional requests for information will be provided during the permitting process .
Subtask 2.2 — Bidding Services
CDM will assist the County in obtaining bids for the well replacement, assist the County in
evaluating the bids and in assembling and awarding the contract, and will arrange and preside
over a pre-construction conference . Detailed services include the following:
■ CDM will provide digital files of plans and technical specifications to the County. County

Subtask 2 . 3 — Services During Construction
Construction services to be provided for the construction of the replacement well consist of the
following:
Part A - Detailed Observation of Construction
Provide full-time, on-site observation of construction activities by one or more representatives
as authorized and approved by the County. CDM will provide on-site construction observation
services during the construction of the production well only. These services consist of daily
observation of construction activities, collection and analysis of drill cuttings and water
samples, testing, and certification of construction. CDM's on-site representative will prepare
and submit weekly progress reports to the County during construction. The weekly progress
reports shall include project status, contractor manpower, materials and equipment received,
work performed, problems encountered and rectification, and weather conditions . CDM will
also responsible for on-site construction observation services during pump installation, testing,
flushing, connections at both ends of the new piping from the well, and
electrical /instrumentation appurtenances .
Part B - Review Pay Requests
CDM's representative will note actual quantities of materials used, depths drilled, well
development time utilized, etc . Contractor requests for payment will be reviewed to verify the
requests are consistent with the amount of work accomplished .
Part C - Issue Interpretations
CDM will issue necessary interpretations and clarifications of the construction contract
documents, in writing, and prepare change orders as required . CDM will interpret the
requirements of the construction contract documents, evaluate the work performed thereunder,
and make recommendations on any claims by the Contractor.
Part D - Report
CDM will prepare a report documenting the well construction activities, well construction
details, water quality, and well performance.
Part E - Certificate of Completion
CDM will prepare a certificate of completion for the new well and raw water piping facilities .
Record drawings of the new facilities will be prepared . A mylar set will be included along with
four signed and sealed blueline copies .

Subtask 2 . 4 — Operational and Start-up Services
During this phase, CDM will provide services to the County to conduct a systems checkout,
including troubleshooting of equipment and instrumentation installed as part of the project.
CDM will provide on-site advice and troubleshooting throughout the start-up .
Once the new well and associated facilities are fully operational, CDM will assist County staff
with operation fine-tuning. All contract documents and computer disks will be catalogued and
delivered to the County.
Task 3 — Sand Separator
County staff indicated that micron filter elements upstream of the membrane skids required
frequent change due to sand from the raw water production wells . Field investigations by
CDM and County staff identified wells that indicated elevated frequency of sand . CDM has
investigated various sand separation equipment for potential use at the facility. CDM assisted
County staff with pilot testing of a small-scale unit and determined that installation of a full-
scale system would be of great benefit to the operation of the facility.
Subtask 3 . 1 — Technical Specification Preparation
CDM will prepare a technical specification for a full-scale sand separator system for raw water
feed to the facility. The specification will be provided to the County for incorporation into
ongoing work currently contracted .
