SUPPLEMENTAL No. 2 to WORK ORDER NO. 1
TO THE
PROFESSIONAL CIVIL ENGINEERING SERVICES AGREEMENT
WITH
CARTER ASSOCIATES, INC.
This Supplemental No. 2 to Work Order No. 1 is in accordance with the existing
AGREEMENT dated January 9, 2007 and the extension thereof, between Carter
Associates, Inc. (ENGINEER) and Indian River County (COUNTY).
This Supplemental No. 2 to Work Order No. 1 amends the agreement as follows:
SECTION I — PROJECT LIMITS
Consultant services are required for the re -design of the stormwater management system
and the potable water service connection for the construction of the Indian River County
Parks Maintenance Complex.
SECTION II — SCOPE OF SERVICES
The Scope of Services for this Supplemental No. 2 to Work Order No. 1 is to perform the
following tasks as requested by the County:
Stormwater Management System to include:
• Revise the Paving, Grading and Drainage Plan to call out revised stormwater pond
control structure weir elevations and show revised permanent pool contours.
• Revise the control structure detail.
• Revise and update the stormwater calculations report.
• Submit revised plans and calculations to the SJRWMD and IRC Engineering Dept.

Water Service
0 Revise the Utility Plan to add a fire hydrant and call -out connection to the
converted 16" water main.
• Modify water supply system permit with the FDEP.
• Modify utility construction permit with the IRCDUS.
SECTION III - TIME FOR COMPLETION
Supplemental tasks shall be completed within 30 calendar days from the date of
execution of this Supplemental No. 2 to Work Order No. L
SECTION V - COMPENSATION
The COUNTY agrees to pay, and the ENGINEER agrees to accept, a fee for services
rendered according to the "Scope of Services" and further identified in all the Sections
of this Supplemental No. 2 to Work Order No. 1, a Lump Sum of $15,740.00.
All and/or any additional services not described hereon shall be pre -approved by the
COUNTY. Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the 2006 — 2007 contract year.
All invoicing and payments shall be in accordance with the original Professional Civil
Engineering Services Agreement with the COUNTY and as stated hereon. Invoices
shall include the work order and project number, invoice number related to this work
order, original contract amount, current billing amount and retainage amounts.
The AGREEMENT is hereby amended as specifically set forth herein. All remaining
sections of the AGREEMENT shall remain in full force and effect, and are incorporated
herein.

Supplemental No. 2 to Work order No. 1 to the Professional Civil Engineering Services Agreement
Dear Chris:
We are pleased to present this proposal for additional services relative to re -designing the stormwater
management system and the potable water service connection for the project. All provisions of the original
Work Order No. 1 to the Professional Civil Engineering Services Agreement (dated January 9, 2007),
including Supplemental No. 1, shall remain in full force and effect, unless otherwise specifically modified
herein. A man-hour fee estimate is attached as Exhibit "A" for your use. The following describes our
proposed scope of services and terms of agreement between Carter Associates, Inc. (Engineer) and Indian
River County (County):
Stormwater Management System:
The stormwater management system will be revised to comply with the phosphorus loading analysis
requirements stipulated as item L in the SJRWMD's letter of 2/21/08. This analysis requirement has been
applied by the SJRWMD to this project since the North Canal drainage basin has recently been classified as
an impaired water body, This requirement is a change in the permitting process that could not have been
anticipated and was not included in the original scope of professional services.
The stormwater management system will also be revised to utilize the wet detention system methodology in
lieu of the stormwater re -use alternative method in order to remove the automatic shut-off/turn-on switch
requirement for the irrigation pump, as is stipulated as item 21 in the SJRWMD's letter of 4/16/08. This
requirement is also a change in the permitting process that could not have been anticipated and it requires a
re -design of the previously designed and submitted plans and calculations. The project documents will be
modified as follows:
• Revise the Paving, Grading and Drainage Plan to call out revised stormwater pond control structure
weir elevations and show revised permanent pool contours.
• Revise the control structure detail.
• Revise and update the stormwater calculations report.
• Submit revised plans and calculations to the SJRWMD and IRC Engineering Dept.
Water Service:
A fire hydrant will be added at the project frontage along 77`h Street and the water service connection will be
re -designed to connect to a 16" diameter water main on the north side of 77`h Street. This water main is
currently shown on the project plans and the Utilities Department as-builts as a brine line. The brine line will

be converted to a potable water main by others. Professional services to be provided under this supplement
are as follows:
• Revise the Utility Plan to add a fire hydrant and call -out connection to the converted 16" water
main.
• Modify water supply system permit with the FDEP.
• Modify utility construction permit with the IRCDUS.
Compensation:
Supplemental No. 2 fees will be invoiced on a percent complete lump sum basis. The total amount to be
billed shall not exceed $15,740.00 unless authorized by an additional written agreement.
