FOR ANNUAL SURVEYING AND MAPPING SERVICES , CONTRACT 0725

CARTER ASSOCIATES , INC .

This Work Order No . 1 ( Utilities 2009 ) is in accordance with the existing AGREEMENT dated
November 6th , 2007 between Carter Associates , Inc . ( SURVEYOR ) and Indian River County
( COUNTY ) .
This Work Order No . 1 ( Utilities 2009 ) amends the agreement as follows :
SECTION I no,,, PROJECT LIMITS :
Office Tasks Only :
Create 3 new revised legal and sketch of easements of those certain easements segment
located in Section 330031 - 39 at CVS parcel , Graves Brothers and Indian River County , Parcel 3
( id # 31393300001002000004 . 1 ) ; Parcel 1 a ( id # 31392800000005000001 . 0 ) ; Parcel 1 b ( id #
31392800000005000002 . 0 ) ; Parcel 2 ( id # 31392800005000000001 . 0 ) ; Parcel 4 ( id #
3139330001002000010 . 0 ) ; and Parcel 5 ( id # 31392800006011000004 . 0 )
1 . ) CVS segement will be comprised of that portion of the easement located within the
boundaries of the CVS property ( Parcel 3 ) ( id #0004 . 1 ) and adjustments which were
discussed on December 17 , 2008 .
2 . ) Graves Brothers segement will be comprised of that portion of the easement located
within the boundaries of the Graves Brothers property ( Parcel la , 1b , 2 and 4 )
( id # 3139330001002000010 . 0 ) 7 ( id # 31392800000005000002 . 0 ) ,
( id # 31392800005000000001 . 0 ) , ( id # 3139330001002000010 . 0 ) and adjustments
which were discussed on December 17 , 2008 .
3 . ) Indian River County segement will be comprised of that portion of the easement located
within the boundaries of the Indian River County property ( Parcel 5 ) ( id #0004 . 0 . )
4 . ) Utilize the existing 14 page set to create 4 new legal and sketch of easements .
5 . ) Coordinate with Indian River Utilities Department of Indian River County .
SECTION II — SCOPE OF SERVICES :
This Work Order No . 1 ( Utilities 2009 ) is for providing all Professional Survey services related to
the task and services as indicated on in sections I , II , III and IV hereon , the attached exhibit
" A"

SECTION III — TIME FOR COMPLETION :
All tasks outlined shall be completed , in its entirety within 10 working days plus/ minus of
notice to proceed . ( The County shall be notified upon completion of tasks and /or of any
delay beyond specified periods ) .
SECTION IV — DELIVERABLES :
Final ( 100 % ) submittal shall consist of One ( 1 ) certified final survey " hardcopies " in the form
of ( 1 ) Mylar version , and an electronic file/disc in state plane geometry ( AutoCAD 2005 or
previous versions ) shall be delivered upon completion .
SECTION IV — COMPENSATION :
The COUNTY agrees to pay , and the SURVEYOR agrees to accept , a fee for services
rendered according to the " Scope of Services " identified in Section I , Il , III and IV of this
Work Order No 1 ( Utilities 2009 ) , a Lump Sum of $ 1 , 450 . 00 .
All invoicing shall be supported with documentation ( Man hours ) of time and materials spent
prior to payments .
The AGREEMENT is hereby amended as specifically set forth herein . All remaining
sections of the AGREEMENT shall remain in full force and effect , and are incorporated
herein .
