In accordance with Contract No. 0725 Cqoo k-'- 156 This Work Order No. 10 is in accordance with the existing AGREEMENT dated November 6"', 2007 between Carter Associates, Inc. (SURVEYOR) and Indian River County (COUNTY). This Work Order No. 10 amends the agreement as follows: SECTION I — PROJECT LIMITS This Work Order No. 10 is for the SURVEYOR to perform additional continuing surveying services for the proposed Oslo Road and Boat Ramp Facility located in a portion of Sections 19, 20, 29 and 30, Township 33 South, Range 40 East, Parcel ID #33401900000700000002.0, Site address: 9th Street SW — East of U.S. Highway #1. SECTION II - SCOPE OF SERVICES As agreed upon between Carter Associates, Inc. and Indian River County, the SURVEYOR shall provide additional (continuing) Professional Surveying and Mapping, Topographic/ Hydrographic / Environmental Professional Land Survey Services in support of the design and permitting applications. Additional (continuing) survey tasks shall consist of the following services: TASK 1: 1. Update the existing Topographic Survey (Carter Associates, Inc. Project Number 03- 422S, Drawing Number 17006 C, dated August 2003) to include additional cross sections and topography southwest of the existing boat ramp for a newly proposed vehicle staging area. 2. As -built two existing culvert locations located southerly of Oslo Road right-of-way. 3. Coordinate with G.K. Environmental to identify and perform topographic cross sections of two proposed culvert locations located approximately 750 and 1670 feet southerly of Oslo Road right-of-way along the westerly shore of the Indian River Lagoon. Establish monumentation on the north line of the County owned lands (Parcel ID # 33401900000700000002.0) lying north of Oslo Road and the Mean High Water line as requested by G.K. Environmental 5. Additional locations/verification of newly established wetland/upland lines. 6. Locate and delineate existing Red and Black Mangroves for proposed dock location. 7. Project management, onsite meetings and coordination with G.K. Environmental, I.R.C., S.J.R.W.M.D. and D.E.P. TASK 2: Perform various surveying services in support of the design and construction as directed by the County Engineer or County surveyor at time and materials rates in accordance with the existing AGREEMENT dated November 6'", 2007 between SURVEYOR and the COUNTY SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT 1. Task 1: Project shall be completed within thirty (30) business days from Notice to Proceed. 2. Task 2: On an as needed and/or requested basis. 3. Deliverables SHALL BE CERTIFIED TO Indian River County and shall include five (5) signed and sealed 24" x 36" surveys (in state Plane Geometry), electronic copies in AutoCAD 2007 and PDF formats. SECTION IV — COMPENSATION Task 1: The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for the Task 1 services as identified in Sections I, II, and III of this Work Order No. 10, a lump sum fee of $3,860.00. Task 2: The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for the Task 2 services as identified in Sections I, II, and III of this Work Order No. 10, on a Time and Material basis, Not to Exceed fee of $5,000.00.
