PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING /GIS SERVICES
WITH
Carter Associates , Inc .
In accordance with Contract No . 0725 2007 -2009
This Work Order No . 22 is in accordance with the existing AGREEMENT dated November 6th
2007 between Carter Associates , Inc . (SURVEYOR) and Indian River County ( COUNTY) . This
Work Order No . 22 amends the agreement as follows :
SECTION I — PROJECT LIMITS
This Work Order No . 22 is for the SURVEYOR to perform all related field and office Surveying
and Mapping services in connection with the Right of Way Mapping of Old Dixie Highway
( North ) from 69th Street traveling north through CR510 to US1 . Limits include all side streets
intersections for minimum 100 feet and properties associated with the project .
SECTION II - SCOPE OF SERVICES
As agreed upon between Carter Associates , Inc . and Indian River County , the SURVEYOR
shall provide Professional Land Surveying services to complete all tasks as outlined in this Work
Order No . 22 ; specifically detailed in the attached proposal Exhibit A , and Right-of-Way check
list Exhibit B .

SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT
1 . Project shall be completed as follows :
a . Time of 80 % - 90 % PRELIMINARY review submittal shall be 75 calendar Days from
notice to proceed .
b . Time of FINAL project completion shall within 30 calendar days of receipt of the
County ' s review comments .
2 . Deliverables -The SURVEYOR shall provide the COUNTY :
a . PRELIMINARY REVIEW . 80 % - 90 % product for COUNTY review and comments .
One ( 1 ) paper " hardcopy" . COUNTY shall strive to provide review comments within
30 calendar days of preliminary submittal .
b . FINAL two ( 2 ) paper " hardcopy " signed and sealed sets and one ( 1 ) signed and
sealed MYLAR sets in adequate recording fashion ( I . E . Borders ) . Sheeted and
model space ( as applicable ) AutoCad drawing file and PDF formats on CD .
c . Survey set/sheets shall include a cover sheet with location sketch , survey
certifications , and related title and Project No ; a survey notes/legend/abbreviation
sheet , survey horizontal control sheet( s ) , right-of-way plan view sheets and/or as
indicated in the COUNTY supplied Right-of-Way map " checklist" .
d . Work Product and digital versions are to prepared and submitted so that the
COUNTY or other consultants can readily use it for the design and analysis of the
area , as defined . It shall contain all information necessary for third - party surveyor to
independently recreate and/or utilize the survey work . It is acknowledged all final
products become property of Indian River COUNTY and will be available for use by
the public at large .
a . The SURVEYOR ' S work product shall meet or exceed the minimum standards as
defined by Sections ll , III and IV or the COUNTY will not approve the SURVEYOR ' S
request for payment .
SECTION IV — COMPENSATION
The COUNTY agrees to pay , and the SURVEYOR agrees to accept , for the above described
services rendered as identified in Sections I , II , and III of this Work Order No . 22 , for a total
lump
sum fee of $ 37 , 500 . 00 .
All and/or any additional services not described hereon shall be pre - approved by the
COUNTY . Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the contract year( s ) of November 6t" , 2007
through November 5t " , 2009 .
All invoicing shall include project number ( 0934) , Work Order No . (WO 22 ) , Contract Number
( 0725 ) , itemize man - hours and materials expended to complete the scope of services .
Payments shall be in accordance with the original Professional Surveying and Mapping /GIS
Services Agreement , contract No : 0725 with the COUNTY and as stated in Section II , III and IV
hereon ,

The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of
the AGREEMENT shall remain in full force and effect , and are incorporated herein .
IN WITNESS WHEREOF the parties hereto have executed these presents this 18th day
of August 2009 .

Old Dixie Highway Route Survey and Right- of-Way Mapping from 69th Street ( North
Winter Beach Road ) through County Road 510 (Wabasso Road ) to US1 .
Services performed within parts of Section 3 , Township 32 South , Range 39 East and Sections 33 and 34 ,
Township 31 South , Range 39 East , Indian River County , Florida .
Project Scope .
The SURVEYOR shall provide Professional Land Surveying services to include the following :
Project is to ( at a minimum ) adhere to County Supplied EXHIBIT " B " Old Dixie Highway - 69th Street
tthrough
County Road 510 to US Indian River County Right-of-Way Map Checklist ,
1 . ) Establish horizontal control network along right- of-way corridor
2 . ) Tie baseline control to Florida State Plane Coordinate System , East Zone , NAD 83/99
3 . ) Locate block corners , lot corners , and FEC Railroad Right- of-Way monumentation along right-of-way lines
4 . ) Location of FEC Railroad tracks and edge of pavement of Dixie Highway and 100 ' on side streets
5 . ) Locate buildings and/or major concrete slabs or features of concern within 25 ' of right- of-way
6 . ) Establish centerline of right- of-way and set nail and disks ( or sufficient monumentation ) at all
centerline
points of intersection PC ' s , PI ' s , PT ' s and side street centerline intersections , interval not to exceed 1

7 . ) Tie into existing sectional control along and adjacent to Old Dixie Highway right- of-way . Certified Corner
Reports ( CCR ) shall be created or updated on all found , used and or established Sectional Control throughout
the project .
8 . ) Tie into existing control for the U . S . Highway 1 Right- of-way maps at major roadway intersections such as
69th Street , 77th Street , 81St Street and County Road 510 at the extension of Old Dixie Highway north of County
Road 510 . ( Obtain and utilize the preliminary County Road 510 Right- of-way maps prepared by Burdette and
Associates (to be furnished by the County) and the original Department of Transportation Right- of-way maps
for County Road 510 and U . S . Highway 1 right- of-way/centerline control and mapping purposes) .

9 . ) County is to supply any boundary surveys , recorded plats or relevant information helpful to the positioning
of the project from County files within ten ( 10 ) days of notice to proceed .
10 . ) County is to supply a current Title Search of approximately 75 parcels bordering and/or within established
limits along the corridor within thirty ( 30 ) days of notice to proceed .
11 . ) The Right- of-Way mapping project shall be prepared in a fashion as to be easily used by the consultant
or
3rd parties as base map for any additional Topographic Surveys as determined by the County via
separate
work order .
Specific Notes :
Method of horizontal and vertical observations by G . P . S . , RTK [accuracy : horizontal 10mm + 1ppm ( RMS ) ,
vertical 20mm + 1 ppm ( RMS ) ] , North American Datum of 1983/Adjustment of 1999 , Florida State Plane
Coordinates ( East Zone -901 ) and National Geodetic vertical Datum of 1929 .
Component accuracy will not exceed agreed project requirements of 0 . 5 feet .
Project Schedule :
Time of 80 % - 90 % PRELIMINARY review submittal shall be 75 calendar days from notice to proceed .
Time of FINAL project completion shall be within 30 calendar days of receipt of the County ' s review comments .
Carter Associates , Inc . will coordinate with the FEC Railroad for access to the FEC Railroad Right- of-Way .

County shall strive to provide review comments within thirty ( 30 ) calendar days of preliminary submittal .
FINAL — One ( 1 ) paper " hardcopy" signed and sealed set and one ( 1 ) signed and sealed MYLAR set

adequate recording fashion ( I . E . Borders ) . Sheeted and model space (as applicable ) AutoCAD drawing
file
and PDF formats on CD .
Survey set/sheets shall include a cover sheet with location sketch , survey certifications , and related title

view sheets , owner tabulation sheets and/or as indicated in the County supplied Right- of-Way Map " checklist" .
Submittals shall be in the form of cover sheet , survey notes and control sheet ( s ) , followed by
applicable key
sheets and Right- of-Way Map sheets .
Compensation :
Payments shall be in accordance with the original Professional Surveying and Mapping /GIS
Services Agreement , Contract No . 0725 with the County .

All Right-of-way projects shall adhere to Chapter 177 & 472 , F . S . and Chapter 61G17 ,
F . A. C . as minimum and be certified as such .
R/W Maps shall be prepared/submitted in the form of a certified R/W Map will all
applicable certifications and criteria met.
1 . Cover sheet/ Key sheet. (To be supplied by IRC in DWG . )
2 . Survey notes and report sheet.
3 . Key sheet( s )
4 . Survey Control sheet.
5 . Subsequent RM map sheets .

40 . Stations and Angle Ties to ALL Existing Major and Minor Side Streets ( Public Road R/W) as
surveyed and /or calculated .
41 . Show field located P . R . M . /major property corner or parcel ties by station /offset to baseline/control
line .
42 . Current Street Names
43 . Any buildings within 25 feet of proposed R/W line shall be dimensioned to R/W line . If proposed
R/W line is unknown , obtain buildings within +/- 25 ' of existing R/W . Locations of buildings and
large structures can be obtained from field and/or photographic / planimetric methods , note shall be
placed on plan as to origin .
44 . Show and identify all easements on Detail Maps by width , use and ownership (where applicable ) .
45 . Any other items as to meet the goals and objectives established by the County .
