In accordance with Contract No. 0725 This Work Order No. 5 is in accordance with the existing AGREEMENT dated November 6th 2007 between Carter Associates, Inc. (SURVEYOR) and Indian River County (COUNTY). This Work Order No. 5 amends the agreement as follows: SECTION I — PROJECT LIMITS ( Reference attached map) This Work Order No. 5 is for the SURVEYOR to perform Professional Surveying and Mapping services related to the study of the positioning and alignment of the Old Dixie Highway corridor lying south of CR512, traveling to US Highway No. 1, approximately 1.3 miles. SECTION II - SCOPE OF SERVICES Goals and objectives: Provide Professional Surveying and Mapping services in an effort to prepare specific findings related to the name, historical time line and alignments of the current and historically positioned Old Dixie Highway within the northern section of the County. Specifically: 1. The Old Dixie Highway currently posted and lying south of CR512 to US Highway No. 1. 2. The Dixie Highway(s) which may have followed along the existing Federal Highway (US Highway No. 1) and/or the roads currently named Indian River Drive, Central Avenue and possibly others. (Cont) F:SEngineeringlCapital Projects\0725- 2007 Annual Prof. Survey Contract\2007-2008 Projects\Carter\Old Dixie\0725 WO 5 Old Dixie Study 1-8-08.doc WO 5 Carter Old Dixie Study Page 2 of 4 As agreed upon between Carter Associates, Inc. and Indian River County, the SURVEYOR shall provide Professional Land Surveying Services as follows: 1. Review documentation supplied by the County, including but not limited to, deeds, title documents, plats and maps. 2. Perform adequate research of documents supplied by the County and in-house (Carter Associates, Inc.) records pertaining to the project. 3. Perform adequate research with existing and historical governmental agencies. 4. Perform mapping (in AutoCAD) of project alignments from the north line of Indian River County through the project area. 5. Plot (Map course and distances) the 1895 (Brevard County Surveyors) county road alignment. 6. Plot (Map courses and distances) the 1920's mark and post 66' foot centerline description. 7. Plot (Map courses and distances) other possible or alignments related to the scope of the project. 8. Time lines and text projections to be included in the mapping. 9. Prepare a Surveyors Report of the findings including: • Detailed text reports on items related to the road in question • Supporting graphics and or maps as exhibits to text items. • A report conclusion. 10. Other related assignments and/or task to assist in achieving the projects goals and objectives. SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT 1. Project shall be completed within thirty (30) CALENDER DAYS from Notice to Proceed. 2. Deliverables SHALL BE CERTIFIED to Indian River County and shall include three (3) signed and sealed 24" x 36" "specific purpose" survey of related maps and alignments, electronic copies in AutoCAD 2007 and PDF formats. 3. A signed Surveyor report. One original bound version and PDF. SECTION IV — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for services rendered as identified in Sections I, II, and III of this Work Order No. 5, on an hourly basis (listed heron) as established by the existing contract with Indian River County NOT TO EXCEED amount of $152000.00. All invoices shall provide actual man-hours and related cost prior to payments. All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein.
