PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Carter Associates, Inc.
In accordance with Contract No. 0725
This Work Order No. 9 is in accordance with the existing AGREEMENT dated November 6th,
2007 between Carter Associates, Inc. (SURVEYOR) and Indian River County (COUNTY). This
Work Order No. 9 amends the agreement as follows:
SECTION I — PROJECT LIMITS
This Work Order No. 9 is for the SURVEYOR to perform SEVEN (7) Boundary and
Improvement Location Surveys directly related to the CR510 R.O.W., Right — of — Way
acquisition applications.

SECTION II - SCOPE OF SERVICES

Attached proposals from Carter Associates, Inc.
As agreed upon between Carter Associates, Inc. and Indian River County, the SURVEYOR
shall provide Professional Land Surveying services to complete SEVEN (7) Boundary Surveys.
Surveys shall delineate the Boundary lines as per deed of record (supplied by the COUNTY);
locate all above ground and apparent underground improvements. Underground improvements
that are not readily visible or obvious are not included in these tasks.

SECTION III — TIME FOR COMPLETION & DELIVERABLEWWORK PRODUCT
1. Project shall be completed within thirty (30) business days from Notice to Proceed.
2. Deliverables SHALL BE CERTIFIED TO Indian River County and shall include five (3)
signed and sealed 24" x 36" AND (3) reduced 81/2" x 11" — "legible" versions (for
recording), electronic copies in AutoCAD 2007 and PDF formats.
SECTION IV —COMPENSATION
The COUNTY agrees to
pay,
and
the
SURVEYOR agrees
to accept,
for
services rendered as
identified in Sections I, II,
and
III of
this
Work Order No. 9, a
lump sum
fee
of $12,200.00

The AGREEMENT is hereby amended as
specifically
set
forth herein. All remaining sections of
the AGREEMENT shall remain in full force
and effect,
