FOR
PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Carter Associates , Inc.
In accordance with Contract No . 0725 2007-2009
This Work Order No . 16 is in accordance with the existing AGREEMENT dated November 6th
2007 between Carter Associates , Inc . (SURVEYOR) and Indian River County (COUNTY) . This
Work Order No . 16 amends the agreement as follows :
SECTION I — PROJECT LIMITS
This Work Order No . 16 is for the SURVEYOR to perform all related field and office Surveying
and Mapping services in connection with the County Roadway Improvements , planning , design
and Right of Way Acquisitions of 43rd Avenue between 8th Street and 18th Street. Limits include
side streets to limits of design and acquisitions .
SECTION 11 - SCOPE OF SERVICES
As agreed upon between Carter Associates , Inc . and Indian River County , the SURVEYOR
shall provide Professional Land Surveying services to complete all tasks as outlined in this Work
Order No . 16 , and per the attached proposal , cost outline ( Exhibit A) , and Right-of-Way check
list ( Exhibit B) .

SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT
1 . Project shall be completed as follows :
a . Time of 80% - 90% PRELIMINARY review submittal shall be 60 calendar Days from
notice to proceed .
b . Time of FINAL project completion shall within 30 calendar days of receipt of the
County' s review comments .
2 . Deliverables -The SURVEYOR shall provide the COUNTY :
a . PRELIMINARY REVIEW . 80% - 90% product for County review and comments . One
( 1 ) paper " hardcopy" . County shall strive to provide review comments within 30
calendar days of preliminary submittal .
b . FINAL two (2) paper hardcopy signed and sealed sets and one ( 1 ) signed and
sealed MYLAR sets in adequate recording fashion (I . E . Borders) . Sheeted and
model space (as applicable) AutoCad drawing file and PDF formats on CD .
c. Survey set/sheets shall include a cover sheet with location sketch , survey
certifications , and related title and project No ; a survey notes/legend/abbreviation
sheet , survey horizontal control sheet(s ) , right-of-way plan view sheets and/or as
indicated in the County supplied Right-of-Way map " checklist" .
d . Work Product and digital versions are to prepared and submitted so that the
COUNTY or other consultants can readily use it for the design and analysis of the
area , as defined . It shall contain all information necessary for third -party surveyor to
independently recreate and/or utilize the survey work . It is acknowledged all final
products become property of Indian River COUNTY and will be available for use by
the public at large .
a . The SURVEYOR' S work product shall meet or exceed the minimum standards as
defined by Sections II , III and IV or the COUNTY will not approve the SURVEYOR' S
request for payment .
SECTION IV — COMPENSATION
The COUNTY agrees to pay , and the SURVEYOR agrees to accept, for the above described
services rendered as identified in Sections I , II , and III of this Work Order No . 16 , for a total lump
sum fee of $40 , 220 . 00 .
All and/or any additional services not described hereon shall be pre- approved by the
COUNTY . Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the contract year(s ) of November 6th , 2007
through November 5th , 2009 .
All invoicing shall include project number (0858) , Work Order No . (WO16) , Contract Number
(0725) , itemize man-hours and materials expended to complete the scope of services .
Payments shall be in accordance with the original Professional Surveying and Mapping /GIS
Services Agreement , contract No : 0725 with the COUNTY and as stated in Section II , III and IV
hereon .

The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein .
IN WITNESS WHEREOF the parties hereto have executed these presents this 2nd day
of December 2008 .

1 . Review and apply approximately 117 property owner title reports including legal descriptions ,
encumbrances and all associated documents provided by Indian River County .
2 . Field survey and office preparation of road right of way map of the subject right of way route for
recording per Chapter 177 and Chapter 472 , Florida Statues and Chapter 61G17 , Florida
Administrative Code and furnished Indian River County Right of Way Map check list.
3 . Prepare Section and One-Quarter Section certified corner records for submittal to Department of
environmental Protection .
4 . Prepare and include a survey control sheet of the project field data .
5 . Prepare and include a property parcel owner' s data table .

INDIAN RIVER COUNTY RIGHT OF WAY MAP CHECKLIST
All Right-of-way projects shall adhere to Chapter 177 & 472 , F . S . and Chapter 61G17 , F .A. C . as
minimum and be certified as such . R/W Maps shall be prepared/submitted in the form of a certified
R/W Map will all applicable certifications and criteria met.

❑ Parcels shall have a parcel identification number in numerical order corresponding with
the Table of Ownership .

❑ Show stations , deflection (or bearing ) of all intersecting section , '/4 section lines to
baseline (either field or calculated as applicable) .
❑ Show monumented baselines and all references thereto .
❑ Verify that computed distances and bearings are supported by field measurements .
❑ Verify and/or monument significant elements of Baseline alignment shown . ( i . e . curves ,
bearings and P . I . Stations)

❑ Ensure that significant elements of all curves are shown and are correct . Also show
curve data on all sheets where they appear.
❑ Provide 100 ' stations on the map product on the controlling baseline and at the
beginning , ending of R/W project , including all bends , PC ' s , PT' s , PI ' s .
❑ Provide and monument stations in the field at maximum 1 , 000 ' intervals , on the
controlling baseline and at the beginning , ending of R/W project , including all bends ,
PC ' s , PT's , PI ' s . For projects under '/z miles stationing at 500 ' intervals .
❑ Stationing in field minimum 500 ' beyond beginning and ending of project.
❑ Baseline (equate stationing to intersecting or matching State or County Jobs by Station
and job number when available)
❑ Projects shall be in NAD83/90 State plane coordinate system and bearings reference
shall be shown and must be clearly stated . Values shall be place on key baseline
monumentation .

❑ As applicable to the project - show all proposed R/W lines in distinct bold line type .
❑ Show/plot all Murphy Reservations and/or release lines (including document of record)
via graphic or dimension which ever is obtainable .
❑ Show names of subdivision streets , plat book and page recording — also , show Lot and
Block number and Dimensions , Exceptions , Easements , and Vacated portions .
❑ When property lines break the Right of Way on curve , show tangent bearings or chord
bearings .
❑ Show any replat of a subdivision — show old subdivision when necessary to clear up
former acquisition or old title references — use system of dashed lines , dotted figures ,
etc . , with ties shown and name and plat book reference indicated
❑ Stations and Angle Ties to ALL Existing Major and Minor Side Streets ( Public Road
R/W) as surveyed and/or calculated .
❑ Show field located P . R . M . /major property corner or parcel ties by station/offset to
baseline/control line .
❑ Current and/or historical Street Names .
❑ Any buildings within 25 feet of proposed R/W line shall be dimensioned to R/W line . If
proposed R/W line is unknown , obtain buildings within +/- 25 ' of existing RNV . Locations
of buildings and large structures can be obtained from field and/or photographic /
planimetric methods , note shall be placed on plan as to origin .
❑ Show and identify all easements on detail maps by width , use and ownership (where
applicable) .
❑ Any other items as to meet the goals and objectives established by the County .
